Supplemental Safety Description I am submitting this supplemental statement to provide a complete and consolidated description of the ongoing safety concern affecting my vehicle. Vehicle Behavior and Safety Symptoms During normal driving conditions, the vehicle consistently exhibits directional instability, including pulling to one side and difficulty maintaining straight-line tracking. Steering control feels unstable, and the vehicle experiences abnormal harshness and vertical movement over the road surface. The vehicle often feels difficult to control and demonstrates behavior consistent with lateral misalignment, including a crab-walking sensation. During rapid left and right steering inputs, the vehicle exhibits abnormal and asymmetric body movement. One side of the vehicle body rises excessively while the opposite side compresses, resulting in excessive body roll and uneven suspension response. This abnormal dynamic behavior creates a strong perception of instability and increases the risk of loss of control or rollover during steering maneuvers. Additionally, overall traction appears reduced. Under wet or slippery road conditions, the wheels are more prone to slipping, further compromising vehicle stability and steering control. Repair History and Dealer Involvement The vehicle has been presented to four different Toyota authorized dealerships for the same steering and suspension-related concerns. Multiple wheel alignments, steering calibrations, inspections, and road tests were performed. Despite these repeated repair attempts, the condition persists. Each dealership reported that no abnormal condition was found and stated that the vehicle was operating normally. In at least one instance, a dealership declined to continue further repair attempts while the safety-related condition remained unresolved. Independent Review and Technical Findings After repeated unsuccessful repair attempts, I conducted an independent review to better understand the cause of the on

Vehicle Cannot Be Safely Driven – Structural / Suspension Defect Suspected – Dealer Claims “Repair Completed” Despite Unequal Wheelbase My vehicle is currently unsafe to operate, due to the following critical issues: ⸻ 1. Left–right wheelbase inconsistency The front and rear wheelbase measurements are not equal side-to-side. This is a strong indicator of structural deviation, such as: •Subframe misalignment •Control arm mounting point displacement •Deformed suspension components •Manufacturing or assembly error in chassis geometry ⸻ 2. Front caster angles remain abnormal •Left caster: 3.9° •Right caster: 4.9° •Cross-caster difference: 1.0° A 1° difference exceeds manufacturer tolerances and causes: •Vehicle drifting •Poor straight-line stability •Uneven steering return •Potential instability during emergency braking This is a safety-related steering geometry defect. ⸻ 3. Real-world driving confirms unsafe behavior The vehicle cannot maintain straight-line travel, the steering wheel is off-center, and the car exhibits instability at normal speeds. This makes the vehicle unsafe to drive. ⸻ 4. Despite these unresolved issues, the dealership insists the vehicle is “repaired” and demands that I pick it up. They refused to provide frame measurement reports or explanation for the unequal wheelbase. Returning a vehicle with unresolved steering and chassis geometry defects constitutes unsafe vehicle delivery. ⸻ Conclusion Based on unequal wheelbase, abnormal caster geometry, and inability to drive safely, this vehicle likely contains a structural or assembly defect. I formally request investigation, suspension geometry verification, and required action from the manufacturer, as this is a matter of public road safety.

  • 11/26/2025· SUSPENSION

I was driving to work when after running across a minor pothole I heard a clunking sound. It turns out it's the top cap for the front coil-over Bilstein shocks had broken allowing the shaft and the bump stop to protrude through the upper mounting plate. The coil did its job of keeping the vehicle suspended, but the shock piston has no ability to alleviate any stresses put upon the coil. Dealer states that because there was an outside contribution, i.e. a pothole, which had been driven around and over many times before, wasn't warrantable. There was no more than standard bump and sway than when driving around as usual. There was no shudder as though I was going too fast for the defect in the road. I dont know if there was pre existing stress damage from a recent collision(opposite corner of truck) or if the was a defect with the part. The top cap is made out of aluminum rather than steel, which I find disturbing after learning this is not the first to have broken. This vehicle has only been off pavement in gravel driveways and parking lots. I hope you can investigate this thoroughly.

The vehicle was being operated in 4-wheel-drive, mode 4H, and when turning to the left at low speed, there was a grinding noise from the front of the vehicle and it would not proceed. The vehicle had to be put back in 2-wheel-drive, mode 2H, put in reverse, backed up a short distance, and then put back in drive, and then it was able to proceed. This is likely the CV joint issue reported in other 2024 and 2025 Toyota Tacomas. The dealer has been notified, and the vehicle will be taken in for service.

My 2025 Toyota Tacoma has an ongoing issue where Apple CarPlay frequently disconnects or fails to connect altogether. This happens both at startup and randomly while driving. • 10/20/2025· FORWARD COLLISION AVOIDANCE,LANE DEPARTURECrash

While driving on a dry, clearly marked two-lane highway in Montana, I had Lane Tracing Assist and Active Lane Keep/Track Assist turned on. My hands were on the steering wheel, speed was within the limit, and visibility was clear. The truck unexpectedly steered itself out of the lane toward the right-side guard rail. There were no audible or visual lane-departure alerts, no “steering assist canceled” message, and the Pre-Collision System did not apply braking before impact. The vehicle struck the guard rail. The system failed on multiple safety layers — no lane warning, no steering correction, and no automatic braking — under normal conditions (dry pavement, well-painted lane lines, daylight). I believe this represents a malfunction or defect in the driver-assist software or sensor calibration for Toyota Safety Sense 3.0. I request NHTSA investigate similar incidents for 2024–2025 Toyota Tacoma models equipped with LTA/LDA and Pre-Collision System to determine whether a systemic defect exists.

  • 10/11/2025· ELECTRICAL SYSTEM

My 2025 Toyota Tacoma SR5 has an ongoing issue where Apple CarPlay frequently disconnects or fails to connect altogether. This happens both at startup and randomly while driving. The problem occurs across multiple iPhones, different cables, and even after ensuring that both the phone and the truck’s software are updated. Many other 4th-generation Tacoma owners (2024–2025) are reporting the same issue online — CarPlay will drop connection, the system freezes, or CarPlay will not appear as an available input for long periods. Because CarPlay is a key part of the hands-free system for navigation, calls, and messaging, these disconnections cause drivers to interact manually with their phones, which creates a potential safety hazard. Toyota dealerships acknowledge the problem but currently have no permanent fix. It appears to be a system-wide defect in the Audio Multimedia 6.0 system used in the new Tacomas. I’m submitting this report so NHTSA can investigate and, if appropriate, prompt Toyota to issue a software or hardware recall to address the defect.

•What component or system failed or malfunctioned, and is it available for inspection upon request? I purchased a 2025 Toyota Tacoma from Carver Toyota of Columbus, IN, 47280 brand new with only 15 miles. I also purchased the 100,000-mile bumper to bumper warranty that covers third party parts. After driving the vehicle for 5,000 miles, I noticed problems with Transmission or the Powertrain System whenever I would shift from 2nd to 3rd gear, down shift, shift to reverse, and would slip acceleration whenever the vehicle is sitting idle. •How was your safety or the safety of others put at risk? This has impacted my safety and the safety of others by not properly shifting into gear. When accelerating it engages harshly from 2nd to 3rd gear. I can’t properly accelerate whenever the vehicle is idle; thus causing safety concerns when operating this vehicle. •Has the problem been reproduced or confirmed by a dealer or independent service center? Yes, on [XXX], I took the vehicle into to Carver Toyota Service Center with only 10,639. My warranty covers 100,000 miles bumper to bumper. •Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? The Service Member with Carver Toyota “test drove” and confirmed that the vehicle shifted harshly from park to reverse and from 2nd to 3rd gear. The vehicle fluids were checked and were found to be low. The fluids were “topped off” and the vehicle was returned back to me within one day of my delivering it to them. •Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? At the time of my reporting this issue with Carver, there were no warnings or error codes indicted by the vehicle. However, this is a well known issue with this make and model of the Toyota Tacoma. I got pieces of glass into my eyes [XXX] Video of the Event This is what I sent to Toyota and the Dealer Toyota of Pharr, as I get ready to take it in for repair but they told me to make a claim on my insurance, when it was literally a defect with the glass Warranty Claim – Defective Sunroof Shattering (2025 TRD Pro) To Whom It May Concern, I am the owner of a 2025 Toyota TRD Pro currently under the New Vehicle Limited Warranty. While driving, the sunroof on my vehicle shattered spontaneously without any external impact. I have video evidence clearly showing the glass cracking, with no rock strikes or other outside causes. This failure I feel is a defect in materials, not outside damage, and therefore falls squarely under Toyota’s New Vehicle Limited Warranty. The warranty specifically excludes glass damage only when caused by external objects; it does not exclude spontaneous failures like this one. For my safety and the integrity of the vehicle, I am requesting that this repair be processed under warranty without delay. I’m risking water to enter the vehicle which will cause more damage. Please document and submit this claim to Toyota Corporate. I also feel I need to help the public of future Tacoma TRD Pro buyers and owners and file a formal defect complaint with NHTSA, as this type of spontaneous glass failure represents a significant safety concern. I respectfully request written confirmation that this will be covered under warranty and ask that you proceed with the repair promptly. Thank you for your immediate attention to this matter. Fourth generation Toyota Tacoma interior door handles -- 2024, 2025, 2026 -- suffer from an engineering or manufacturing defect. These handles are breaking during normal use, by the thousands. This is a serious safety risk, as drivers with limited mobility, or even fully-abled drivers can be entrapped within the vehicle during an emergency. A regular pull, let alone a hard pull, routinely snaps the mechanism. Toyota dealer has 1000+ backlog of repairs. This is a serious safety risk and a recall is critical. Drivers will die if this is not addressed; it is merely a matter of time.

  • 09/03/2026· UNKNOWN OR OTHER

Few months back, the driver side door handle "broke" on the inside. There is some plastic piece/mechanism that attaches the door handle to the cable on the inside of the door that allows the user to open the door, with resistance from the cable. That piece broke, so it made the handle loose and flimsy, unable to open the door from the inside. Toyota said that was a known issue and that the parts were on backorder, so they just swapped the handle from my back passenger driver side door with my front driver side door. Well last week, the swapped handle also broke. So, I have to roll my window down and open the door from outside to get out as neither of my left side door handles work. Toyota still doesn't have an eta of when parts will ship to replace the pieces, and they aren't offering me a resolution in the meantime.

The inside driver-side door handle broke, leaving me unable to open the door from inside the truck. I had to crawl across the truck and exit through another door. This is a serious and immediate safety hazard. The driver must be able to exit the vehicle quickly in an emergency. If there were a fire, accident, or other emergency, a broken interior door handle could trap the driver inside the vehicle and potentially result in serious injury or death.

Equipment Failure. The interior driver-side door handle mechanism failed and snapped completely off during normal operation while trying to exit stationary vehicle. The plastic hinge pin/bell-crank component fractured internally under standard hand pressure. This component failure completely prevents the driver from exiting the cabin from the inside, creating an immediate emergency entrapment and life-safety hazard. The manufacturer (Toyota) currently has the replacement door handle assembly on a national backorder with no available ETA, forcing consumers to operate a vehicle with a compromised emergency exit route. The vehicle currently approximately 3429 miles

I think it’s a serious safety issue for Toyota not to include a trailer brake controller or at least a connector to install an aftermarket trailer brake controller on my truck. This has been mentioned numerous times by customers. My truck is rated to tow 6,400 with no way to add a trailer brake controller. I don’t see how Toyota can legally sell this truck without at least giving an option to install an aftermarket controller, even though Toyota doesn’t recommend it.

The interior driver's-side door handle assembly on my 2025 Toyota Tacoma failed during normal use while attempting to open the door from inside the vehicle. There was no excessive force, impact, accident, or other damage that caused the failure. There were no warning lights, messages, or noticeable symptoms prior to the handle failing. The failure creates a safety concern because the driver's door can no longer be opened normally from inside the vehicle using the interior door handle. In an emergency where rapid exit from the vehicle was necessary, such as a collision or vehicle fire, the inability to use the driver's interior door release could delay or prevent the driver from exiting through that door. The vehicle was brought to an authorized Toyota dealership, which inspected and confirmed the failed interior door handle assembly. The dealership determined that the assembly requires replacement and is covering the repair under the Toyota factory warranty. The replacement assembly has been ordered; however, I was informed that the part is currently on backorder with no estimated date of availability. The failed component remains installed in the vehicle and is available for inspection if requested. The vehicle remains in use while awaiting the replacement part. I am submitting this complaint because the failure involves the mechanism used to exit the vehicle and therefore appears to present a potential safety issue rather than simply a cosmetic or convenience-related defect.

I purchased a brand-new 2025 Toyota Tacoma TRD Off-Road. From the first day, the steering wheel was off-center and the vehicle vibrated while driving. At around 500 miles, I noticed severe instability and abnormal tire wear. A wheel alignment report showed: Thrust angle: +0.15° (spec: 0° ± 0.05°) Rear toe: Left 0.23°, Right -0.10° (difference: 0.33°, spec ≤ 0.15°) Front caster: Both sides out of specification (shown in red on report) Driving symptoms include: Cannot maintain straight-line driving; requires constant steering correction. Excessive body roll during lane changes or turns. Continuous vibration of the vehicle and steering wheel on smooth roads. Vehicle bounces and momentarily loses control on uneven surfaces. Steering wheel rotation does not match front wheel steering angle. When stationary, turning the steering to full lock causes the body to rise; releasing it causes it to drop. Attempts to repair at Toyota of Manhattan (June 2025 – August 2025) were unsuccessful. The dealership repeatedly refused to perform proper inspections, provide reports, or acknowledge the defect, despite multiple appointments and a case opened with Toyota corporate. The defect presents a serious safety risk: reduced vehicle stability, increased stopping distance, loss of steering control during maneuvers, excessive and uneven tire wear, and abnormal fuel consumption (13.5 MPG with no load). The problem exists from new and has not been corrected.

The interior door handle on my vehicle broke, and this creates a safety risk because occupants — including passengers, especially children in rear seats who may not know an alternate way out — may be unable to open the door and exit from inside. This is particularly dangerous in an emergency such as a collision, fire, or submersion, or in a situation where a passenger needs to exit quickly for personal safety, such as during an abduction or assault. This defect could also delay first responders during an extraction. The door broke again immediately after picking up the vehicle.

The smoke alarm sustained visible smoke damage and its exterior housing was melted, which we believe indicates the alarm was exposed to significant heat/smoke without triggering, or triggered only very late in that exposure. We believe this represents a serious safety equipment failure: a smoke/fire detection system that did not provide any early warning during an active fire, in a small space. This delay in detection could have resulted in serious injury or death. Additionally, we received no safety briefing regarding the hazards of the propane system at the time of vehicle handover. The vehicle overview video provided to us contained no safety information regarding the gas system, and we later discovered it depicted a different vehicle unit than the one we were actually provided, raising questions about whether this vehicle's specific gas/stove configuration and safety equipment had been properly reviewed or tested prior to rental. We made a best guess at cause in speaking with the responder fire department, but we cannot know for sure. To be clear, there was no issue with the Toyota Tacoma itself. We are submitting this complaint to ensure NHTSA is aware of this potential safety defect in the smoke alarm system and/or propane stove. Front passenger interior door handle is a fragile component. Mine failed with little to no force where the cable disconnects from the handle. It has rendered the ability to exit the vehicle via the handle impossible, and Passenger must roll down window and reach for the exterior handle to exit. I’ve had my vehicle at the dealer, and being sent back with vehicle not fixed due to parts being on backorder. No ETA is able to be provided.

The drivers door handle inside the truck has failed leaving me unable to open the drivers door unless I roll the window down and open the door from the outside. I took the truck to a local dealer and they let me know this is a very common problem on the truck and the parts to fix it are on nationwide back order. Was sent home with the vehicle knowing the door is inoperable unless opened from the outside. This poses a serious safety risk in the event of an accident or emergency where one needs to exit the vehicle quickly.

I am reporting a severe safety hazard on my 2025 Toyota Tacoma caused by incorrect alignment specs and an uncorrected extreme toe setting. On a recent service visit, alignment inspection showed a Total Toe of +1.62 degrees (OEM target is approximately +0.10 degrees). The alignment machine database loaded incorrect tolerance specs (+1.37 to +1.87 degrees displayed as acceptable green range), leading technicians to leave the extreme toe-in completely unadjusted. In less than 2,000 miles of normal driving since a prior inspection, both front tires suffered extreme, rapid toe wear down to the internal cord layer. Operating a vehicle with +1.62 degrees total toe causes severe lateral tire drag and creates an imminent threat of a catastrophic tire blowout at highway speeds, presenting a significant vehicle safety defect.

The contact owns a 2025 Toyota Tacoma. The contact stated that while stationary and attempting to exit the vehicle from the driver's side, the interior driver's side door handle fractured, preventing the opening or locking of the door from the inside. The handle remained physically attached to the door panel but was inoperable. The vehicle was taken to the local dealer to be inspected. While at the dealer, it was discovered that another Tacoma at the dealer had a similar interior door handle failure. The vehicle was not repaired. The manufacturer was notified of the failure. The failure mileage was 4,476.

Driver side front and passenger door handle broke internally. The door is unable to be opened from the inside. This puts occupants at risk of not being able to exit the veichle in an emergency or safely from the inside. The problem was fixed for the front after several weeks due to back order of the part. Per my service representative at Toyota, it was a safety concern so they paid for a rental while they waited for the part and repaired the veichle. The mechanism broke without warning and suddenly after normal expected use.

On several occasions in the previous two months, I have been driving during heavy rain. On the first occasion, while high speed wipers were on, they suddenly stopped working. I slowed down, but was able to get them going again prior to pulling to the shoulder of the highway. I had to switch to the slow wiper speed to get them going. I switched back to the high speed wipers due to heavy rain, and kept stopping each time. It continued this cycle and stayed on after about 15 attempts. Today, on [XXX], I drove down [XXX] from Flint, MI towards Detroit at approximately [XXX]. The wipers again quit working and I had to switch the wiper speed to get it working again while at full highway speed during a deluge of rain and nearly had to stop in a construction zone with no shoulder and heavy traffic, nearly causing a collision until I was able to get the wipers to begin working again. On a previous occasion, I turned them on. When the rain stopped, I was unable to stop the wipers, but was eventually able to stop them by carefully moving the intermittent delay wheel to a position between on and off. It appeared that there may be a short in the wiper circuit. This was a definite safety issue in that my attention was taken away from driving to try to fix the wiper situation. In speaking with my service adviser at Randy Wise Toyota in Grand Blanc, MI, he went out to their lot and tried to duplicate the problem, which he confirmed happened on that same Toyota Tacoma model. I attempted to get this repaired under warranty. Their response was that there was nothing they could do since it appeared to be in the design of the wiper system. My opinion is that this is a serious safety issue which needs to be addressed immediately. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

The XM radio fades out when ever you go under a tree and on its own when going down the road. It says that it is trying to locate a satellite! The dealership has replaced the radio and the antenna and checked the antenna cable, with no improvement! They said that they can’t do anything else about the radio problem. Toyota said to file a complaint with NHTSA. Toyota's case number is 250707000269. The truck is 3 weeks old! With 453 miles on it.

2025 Toyota Tacoma SR5 - 8038 Miles The passenger on the right rear side tried to exit the 2025 SR5 Toyota Tacoma, and the interior door handle would not engage. It was confirmed with the Toyota dealership: PERFORMED INSPECTION AND VERIFIED R/R INNER DOOR HANDLE IS INOPERATIVE. FOUND THE INTERNAL RETURN SPRINGS DISENGAGED FROM THEIR RETENTION POINTS AND HAVE LOST PROPER TENSION DUE TO INSUFFICIENT SPRING RETENTION/IMPROPER FACTORY FITMENT. CONDITION IS CONSISTENT WITH COMPONENT MATERIAL FATIGUE AND RETAINER DESIGN FAILURE, RESULTING IN LOSS OF HANDLE RETURN FUNCTION AND LATCH ACTUATION. RECOMMEND REPLACEMENT OF R/R INNER DOOR HANDLE ASSEMBLY TO RESTORE PROPER OPERATION. The part is on back order due to this being a constant issue. They have informed me that they are not sure when they will have the part. They told me that there are even customers who have been waiting for over a month so far. Driving around with the inability to open the doors from the inside is a trap hazard! Based on multiple complaints and reports, Toyota should issue a recall for these vehicles.

The interior driver-side door handle/release mechanism on my 2025 SR5 Toyota Tacoma failed without warning. The vehicle has low mileage and was being used normally. When attempting to open the driver door from the inside using ordinary force, an internal component appears to have detached or broken, leaving the interior handle completely inoperative. The handle simply hangs loose. Vehicle and failed component are available for inspection. This defect creates a significant safety risk because I cannot exit the vehicle through the driver door from inside the cabin. The only way to exit is to roll down the window, reach backward, and pull the exterior door handle. This requires an awkward twisting motion and causes shoulder pain. In an emergency such as a crash, fire, or water intrusion event, my ability to quickly exit the vehicle could be compromised. The defect also affects my ability to care for my young son, who has a medical condition and may require immediate assistance or rescue medication. If an emergency occurred while driving, I could be delayed in reaching him because I cannot promptly exit through the driver door as designed. A Toyota dealership inspected the vehicle and confirmed the failed interior door handle mechanism. However, the replacement part is on national backorder and unavailable. I was informed repairs will likely take more than a month and that the waiting list reportedly exceeds 2,000 vehicles. Numerous Tacoma owners have reported the same failure, suggesting a widespread defect. Despite reports of widespread failures and lengthy repair delays, there has been no recall, service campaign, or proactive communication from Toyota warning owners of the defect or providing guidance while awaiting repairs. There were no warning lights, messages, or advance symptoms before the failure. The handle functioned normally until it suddenly stopped working during routine use. The defect occurred without misuse, excessive force, collision, or damage.
