Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2022-2026 Toyota, Lexus, and Subaru Solterra vehicles equipped with a Panoramic View Monitor (PVM) system. Please see the recall report for a complete list of models. A software error may cause the rearview camera to freeze or display a blank screen when the vehicle is in reverse. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 111, "Rear Visibility."
Risk: A rearview camera that fails to display an image can reduce the driver's view behind the vehicle, increasing the risk of a crash.
Fix: Dealers will update the parking assist software, free of charge. Owner letters were mailed January 2, 2026. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's numbers for this recall are 25TB13 and 25LB06. Subaru's number for this recall is WRE25.

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2023-2024 Toyota Venza, 2023-2025 RAV4 Prime, RAV4, Highlander, GR Corolla, Crown, 2024-2025 Lexus TX, LS, Toyota Tacoma, Grand Highlander, and 2025 Lexus RX, Toyota Crown Signia, Camry, RAV 4 Plug-in Hybrid (PHEV), and 4 Runner vehicles. Due to an error in the instrument panel software at vehicle startup, the instrument panel may fail to display vehicle speed, brake system, and tire pressure warning lights.
Risk: An instrument panel display that does not show critical information can increase the risk of a crash or injury.
Fix: Dealers will update the instrument panel software over-the-air (OTA) for non-PHEV vehicles, free of charge. For PHEV vehicles, dealers will inspect the instrument panel assembly, and either replace it, or update the software, free of charge. Owner letters were mailed December 5, 2025. Additional letters will be sent, anticipated in May 2026. This is a phased recall. Owners may contact Toyota's customer service at 1-800-331-4331. In March 2025, pulling out from parking lot into traffic and accelerated but vehicle slowly moved forward even though accelerator pedal was more than halfway down. Luckily, the oncoming vehicle had time to slow down and avoid hitting my vehicle. This is the same problem that Toyota Grand Highlander Platinum owners have been experiencing. This is a safety hazard especially when your accelerating into traffic and the vehicle basically craws forward.

On 9/24/2025, while stopping in front of house to back into driveway, main 14 inch display screen went blank. The forward camera was not displayed. Upon putting vehicle in Reverse gear, display screen remained blank and no reverse camera was displayed. After shifting vehicle into Park, display screen continued to be blank not showing main control interface. Main 14 inch display screen failure. Loss of forward, surround and reverse cameras. Loss of control display.

When starting from a stop, putting foot on the gas pedal the car won’t move. Hesitates for a longtime. It has happened numerous times since purchased new. The manufacturer stated that engineering is looking into it, with no definitive answers. No warning lights. It just won’t go, at times which is a safety concerning especially when crossing with oncoming traffic. Unresponsive.

We have had 2 windshields crack on the Lexus TX350, with very low impact to the windshield by very small road debris. Both windshields require(d) replacement due to long migrating cracks. There are several issues with the windshields on the TX350 per online searches. The windshields are a laminated acrylic one that are prone to cracks requiring replacement. This is a common issue on the TX350. The windshields are on long back orders for a genuine Lexus part. We are having our second windshield replaced on XXX at the Lexus dealer. I am of the opinion that this issue should be looked into by NHTSA for a potential recall and improved manufacturing process for future original equipment and genuine Lexus replacement. Our TX350 will be available for inspection on XXX at Parker Lexus in Little Rock Arkansas. The approximate cost to replace the windshield is $1850.00. We are not claiming the windshield on our insurance for this replacement for fear of potentially being cancelled by our insurance company. Please research this issue at your earliest convenience and feel free to request more information from me if needed to help in the assessment of the problem. We look forward to hearing from you asap. Air Conditioner Condenser failed after only 12,000 miles on a brand new 2025 Lexus TX 350 Vehicle. Leak caused AC to only blow hot air. After having the vehicle serviced, the Service Advisor indicated that the condenser had a leak which was cause by road debris and would not be covered under the warranty or purchased additional extended warranty. The vehicle is exclusively driven on highway and paved roads. Researching the symptoms, many other now frustrated Lexus Owners are complaining about the same issues and lack of Lexus Support. The placement of the condenser behind the wide- open grill, make the condenser prone to failure. Repair cost estimated at over $2,000 for a $70,000 car, less than 6 months old.

The vehicle looses power and hesitates to accelerate from a stop start. This happens when The HV Battery State of Charge (SOC) is below 30%. •The vehicle is in catalyst warm-up mode (cold start). The Motor Generator (MG) is in charge mode. As a result of this scenario I have almost been T boned 3 times while attempting a left turn from my home across a dual carriage lane. And again nearly rear ended because of the lack of power or hesitation. To me this is a big safety concern that Lexus do not have a credible fix for.

The electronic throttle control, vehicle speed control, and powertrain/transmission control system failed. When I press the pedal to accelerate there was no power. However I did not lose power to the vehicle as I can see the vehicle was in Drive mode but was not moving forward. I was on a decline hill and the decline helped me go downwards before a truck could hit me. This total loss of acceleration has occurred exactly 2 times within a 4-week period. The vehicle is currently located at the Lexus dealership service center and is available for immediate inspection upon request. My wife and small children were in the vehicle when the accelerator pedal went completely dead while driving. The sudden drop in speed in active traffic almost caused us to be catastrophically rear-ended by an 18-wheeler semi-truck. After one minute of zero throttle, the vehicle violently jerked, locked us into a highly dangerous 5 MPH limp mode on the road, jerked a second time, and then suddenly resumed normal speed. My family's lives were put at extreme risk. No. The vehicle is currently at the authorized Lexus dealership for inspection, but the service department states they cannot find any diagnostic trouble codes (DTCs) and have not yet reproduced the intermittent failure on a test drive. The vehicle is currently under inspection by the authorized Lexus dealership service department. It has not yet been inspected by the manufacturer's regional corporate specialists, police, or insurance. No. There were absolutely no warning lamps, check engine lights, dashboard messages, or symptoms prior to either of the two failures. The loss of acceleration happened instantly without warning. The vehicle's computer system failed to store any error codes despite the severe mechanical jerking and the sudden 5 MPH speed restriction.

We were driving down the highway and the vehicle started shaking. I immediately slowed down as I felt we had a flat. There was notification on the screen. I moved to the right onto the shoulder. Only after we were completely stopped did I see a low tire warning come on display. I slowly got out as the traffic was heavy. I could see the outer section of the tire came apart from both sides. As we were in a dangerous location someone called the police. Two police cars pulled behind us and provided us some safety. We agreed to swap out bad tire for spare as we were in a very dangerous location and needed to get vehicle moved before someone ran into all of us. We did look at the highway to ensure no objects were in the road. I didn’t see any while driving, but we wanted to verify to protect other drivers. There also weren’t any pot-holes in highway. We inspected tire after taking it off and there were no screws, nails, objects, or damage to the center tread/sides. The tire seemed to separate fairly cleanly on both sides. Tires were Continental Cross-contact 255/45 R22 107V LX20 (Eco plus) To repeat, no Warning Lights came on vehicle before tire came apart. I have sent message to my insurance company. No one has inspected tire and it is in my possession.

I am reporting two separate but related fuel-system defects on my 2025 Lexus TX500h (VIN [XXX]) that raise safety concerns and appear to be widespread across Lexus TX and NX hybrid models. FUEL FILLER DOOR FAILS TO OPEN: The electronic fuel filler door intermittently fails to open when the release is activated. It sometimes requires multiple attempts, and at times will not open at all, leaving the vehicle unable to be refueled. This creates a safety risk of being stranded with no ability to add fuel, including the risk of running out of fuel on the road or being unable to refuel in an emergency. The failure appears to worsen in cold weather. This is a widely reported problem on 2022-2025 Lexus NX and TX hybrid vehicles and has been attributed to a faulty electronic fuel-door lock actuator (referenced by owners and parts suppliers as OEM part number 77030-F6020). Owners are frequently forced to use a manual emergency release located behind an access panel in the cargo area in order to refuel. FUEL TANK CANNOT BE FILLED TO CAPACITY: On a near-empty tank, the fuel pump nozzle shuts off after only approximately 8 gallons, despite the tank's rated capacity of approximately 17 gallons. Adding additional fuel requires repeated, prolonged attempts and often only a few more gallons can be added. This significantly reduces usable driving range relative to what the vehicle's fuel gauge and range estimate imply, creating a risk of unexpectedly low fuel and reduced range in situations where refueling options are limited. Other owners have reported the same behavior, and in at least one documented case a Lexus Field Technical Specialist diagnosed the cause as a deformed/defective fuel tank requiring a redesigned replacement. Both conditions are present on my vehicle. I am reporting them so that they are documented at the federal level. No crash, injury, or fire has occurred. Vehicle Information Make: Lexus Model: TX350 Luxury Year: 2025 Mileage: 580 miles VIN: [XXX] Summary of Complaint At just 580 miles, my brand-new Lexus TX350 Luxury's AC failed on first use. I heard hissing and saw oil residue under the hood. The Clearwater, FL dealership diagnosed it as a punctured AC condenser due to “road debris” and quoted $2,000 to repair. The car had no visible damage, and I asked to keep the part for independent analysis but was denied—despite offering to pay in full. Photos show the vehicle in flawless condition. Lexus initially refused warranty coverage and claimed a front tire may have kicked up a rock, despite all tires being behind the grille. Eventually, after repeated contact with Lexus corporate and both dealerships (in Jacksonville and Clearwater), they agreed to cover the repair—but still refused to return the damaged part. Several service advisors admitted this issue is common. The condenser appears unprotected behind the grille, making it vulnerable to debris. Given the low mileage and lack of damage, this points to a design flaw. Lexus’s inconsistent handling and denial of access to the part raise concerns. Request for Investigation Please investigate this as a potential systemic design defect. The vulnerability of the AC condenser can lead to repeat failures and high repair costs, especially in hot climates where AC is essential. Lexus has asked one owner if he would agree to being a guinea pig and allow them to add a screen behind the grill. y 2025 Lexus TX500h experienced sudden and complete AC failure at fewer than 25,000 miles, 18 months after purchase. The selling dealer diagnosed a refrigerant leak from the AC condenser and denied warranty coverage citing road debris impact damage. No photographic documentation of a discrete impact event or specific leak point was provided to support the denial. This failure is consistent with a documented, systemic design vulnerability in the Lexus TX platform. The vehicle's wide-open front grille design allows road debris direct access to the AC condenser, which sits as the frontmost component in the cooling stack with no protective barrier. Multiple NHTSA complaints document identical failures on TX platform vehicles at extremely low mileages including 580 miles and 900 miles, with dealers uniformly denying warranty coverage. Operating without AC in Texas summer heat constitutes a safety hazard, particularly for child passengers. I am requesting NHTSA investigate this as a systemic design defect pattern across the TX platform. Attached photos include: (1) photos provided by the dealer as their evidence of "impact damage" — none of which document a specific leak point or impact deformation zone; and (2) photos taken by the owner of the condenser on the Westside Lexus loaner vehicle provided during this service visit, which show comparable fin condition and debris accumulation to the denied vehicle. The loaner vehicle photos demonstrate that this condenser condition is consistent with normal operation in this environment, not discrete impact damage.

On our 2025 Lexus TX350, less than 1 year old, and under normal driving conditions, our AC stopped working suddenly. Upon inspection at the dealership, they determined a rock or road debris came through the front grill and impacted the condenser, which caused a major refrigerant leak and ac to stop working. I believe this was due to a poor design of the lower half of the front grill, it’s wide open and offers little to no protection to ac and engine components. They should have a better honeycomb or similar grill to protect components. The refrigerant leak is an environmental and road safety issue, and living in hot climates like southern Nevada, when it’s 110 degrees outside, the heat inside the vehicle with no AC is a safety issue for children, adults, etc. The dealership did not cover repairs under warranty, and quoted $3,500 to repair either with cash or submit insurance claim.

2025 Lexus TX350, approximately 27,000 miles. Vehicle exhibits rotational clicking/vibration from front drivetrain during acceleration while turning. Dealer is attributing issue to brake pad and rotor wear. Lexus TSB L-SB-0033-24 Rev1 documents a known front drive shaft assembly defect on this platform. TSB applicability check has not been performed. Requesting investigation into whether this defect extends to 2025 model year production.

The contact owns a 2025 Lexus TX350. The contact stated that while driving 50 MPH, the brake pedal was depressed and felt spongy, and the vehicle failed to respond as intended, causing the braking distance to be extended. The vehicle was taken to the dealer, where the contact was informed that the brakes were in good working condition; however, the failure persisted. The contact stated that while attempting to raise the folded passenger's side third row seat to the upright position, the back of the seat became stuck to an unknown part behind the seat, causing the seat to fail to respond as needed. The vehicle was taken to the dealer, where the contact was informed that the unknown part had been replaced, and the seat could be raised; however, the failure persisted. Additionally, the contact stated that fuel tank door had failed to open intermittently while pressing the release button on the steering wheel, forcing the contact to release the fuel tank door by engaging the emergency release manual button inside the trunk. The dealer was notified of the failure, and an appointment was scheduled for a diagnostic test. The vehicle was not repaired. The manufacturer was notified of the failure. The failure mileage was approximately 3,000.

I would just like to say that the info instrument panel in my Lexus does not light up satisfactory to see when there’s daybreak. I think this is a safety hazard when you can’t see the numbers because of the sunshine. I think this is a safety issue that should be addressed by Lexus anytime you can move the instrument cluster info and you cannot read the numbers then there is a problem. I haven’t seen where many people have complained about it, but I do know that mine is not the only one that has this problem. Thank you.

While my 2025 Lexus TX was parked in my residential garage, the vehicle suddenly locked itself with my [XXX] and [XXX] children and my phone inside. The doors had been unlocked prior to placing them in the vehicle. Without warning, the vehicle locked all doors and windows. The key fob failed to unlock the vehicle electronically or manually. My husband and I attempted for over 30 minutes to gain access while the children remained trapped inside before calling the dealership for technical help. They instead transferred us to Lexus roadside assistance. Lexus roadside assistance advised calling [XXX] to break the windows or otherwise enter the vehicle forcefully. However, before we could call, my husband figured out how to get a narrow opening to access the vehicle through the trunk to enter the interior and lower the windows in order to remove the children safely. The vehicle was taken to the dealership for inspection. This malfunction created a serious occupant entrapment safety hazard involving infants. Basically, the instrument cluster lighting is very, very poor. Very difficult to read when driving, especially in a day break. You must turn off your daylight running lights to make it a little bit brighter. This is a problem.

While stopped in traffic with my foot firmly on the brake pedal, my vehicle began accelerating forward unexpectedly. The vehicle seemed "possessed". It was as if the gas pedal was fully on the ground while my foot was still on the brake, so the car was lurching and seemed to be fighting itself. The vehicle continued moving forward and would not stop despite sustained and hard brake pressure. I was rapidly closing distance on the vehicle in front of me and narrowly avoided a collision by inches. The vehicle only stopped after I shifted it into park and then I shut the car off. This was a terrifying and dangerous event. The vehicle accelerated against brake input and created an immediate collision risk. I reported this incident to my local Lexus dealership (Lexus of Fife, WA). However, the dealership minimized the event in their service documentation and did not accurately record my description of unintended acceleration while braking. At that time, I was informed that Lexus had issued a Technical Service Bulletin (TSB) related to this issue but has not issued a recall or broadly notified owners. If the manufacturer is aware of this defect and has not initiated a recall, I believe this poses a significant public safety risk. This issue presents a serious safety concern involving unintended acceleration despite brake application. I am concerned that other drivers could experience similar incidents, potentially resulting in injury or fatal accidents. I suspect that accidents have already resulted from this. I am requesting investigation into this defect. Owners should be notified and this should be recalled immediately.

We leased the Lexus at the end of May 2025. My husband I drove the car. It was nice. My husband and I are in our early [XXX] . Meaning we are very capable of driving. We both ride motorcycles, snowmobiles, dirt bikes,4 wheeling and we also own 1957 truck, 2024 corvette, and we even both drove our own corvettes at the spring mountain in Nevada. That being said this car is a nightmare. I don’t need a car to take over my driving, braking, and anything else. Whether it’s driving 75-80mph and you are on a major interstate and it decides that the sweeping curve is dangerous while I’m passing a semi or a string of cars! I don’t need it to slow down because it thinks too fast of a curve?!?!?. I took it to Brookfield Lexus for over a week I spoke with service told them what I dislike about the car. The one service tech said,”. You are not the only one that dislikes all of the safety features”! I asked him can you remove all of the safety features, “ he said not sure but we will do what we can”! When I picked up my car on Dec 20,2025 they showed me that only a couple of safety features can be shut off. The other ones have to be shut off manually everytime you start the car!!!!! I should not have to go into the computer and manually shut other features off because it’s back to flashing, beeping, and any other annoying features to put me in danger because it’s telling me you are going to have an accident!!!! It’s a distraction that is a problem. My daughter drove it home with our small trailer in a snowstorm last Sunday . My daughter was changing lanes and when she did your auto braking took control and she almost went in the ditch. With the trailer on the back it started to slide because it was overcompensating which put my daughter, the car, my trailer, and anyone else near her in danger. I spoke with Lexus Corporate and filed a complaint. They said there was nothing wrong with the car. There is a major problem with that car.
