    Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2021-2025 Sienna Hybrid vehicles. The bolts for the seatbacks on the third-row seat assemblies may not be tightened properly. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) numbers 202, "Head Restraints," 207, "Seating Systems," and 225, "Child Restraint Anchorage Systems."

    Risk: An improperly tightened seatback may not adequately restrain the occupant during a crash, increasing their risk of injury.

    Fix: Dealers will tighten the seat assembly bolts, free of charge. Owner notification letters were mailed April 1, 2025. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's number for this recall is 25TA05.

    Incident Description (Firestone Destination LE3 – P235/65R17) I purchased four new Firestone Destination LE3 tires (size P235/65R17, DOT IW2 8ILE31) and within the first week three of them developed noticeable bulges in the sidewalls. I returned to the tire service center, and they replaced the three tires due to the bulging. The following week, new bulges appeared again in the sidewalls on multiple replacement tires, for a total of six affected tires. Sidewall bulging creates a risk of sudden tire failure and loss of vehicle control, and I do not feel the tires are safe to drive on. One service center stated this was “normal from driving on roads,” but I disagree due to the number of tires affected in such a short time. The condition has been repeated and confirmed by multiple service centers. The tires remain available for inspection upon request. There were no warning lamps or messages before the bulges appeared. The issue began immediately after installation of the first set, and the problem persisted with the replacement set. In my assessment, the repeated sidewall bulging indicates a potential structural or manufacturing defect.

    Hello, leader. I bought my car (Sena Platinum Edition) for three months. Due to a mistake in the appearance, I need to replace the engine now. My new car will definitely depreciate if the engine is replaced, so I hope Toyota headquarters can refund me in full. I need to buy a new Sena! My house is full of Toyota cars, no matter what the result of Toyota Company's handling? I will still choose to buy a Toyota! Thank you!

    Looked online to see if other Sienna drivers had the same issue with the hybrid system. Brakes are accelerating a bit even when already pressing on the brakes. It has happened enough times to be concerning. We have not taken it to the dealearship as of yet but will be getting in touch soon , now that we know it's a known issue. No warnings from vehicle.

    Dear Sir/Madam, I am writing to formally raise a complaint regarding a serious and ongoing issue with my 2024 Toyota Sienna XLE AWD, purchased in April 2024 in Illinois. From the very first day, the trunk door has not functioned properly — often failing to close, randomly opening, or requiring manual force to shut. Despite repeated service visits across multiple states (IL, SC, NC) and investing considerable time, fuel, and lost income as a contractor, this issue persisted for over 18 months, creating safety concerns and inconvenience for me and my family. Key Issues: Trunk door opens on its own – even while reversing, creating serious safety risks. Malfunctioning power close mechanism, requiring manual closure frequently. Unreliable performance: door would close, then immediately re-open. Visited Toyota service centers at least 6 times across 3 states. Each visit took 2–3 hours, costing me work hours as a contractor. Customer service experience has been poor, especially with manager Patte, who: Failed to schedule proper service appointments. Directed me without coordination. Refused to escalate my request or connect me to her manager. Dismissed my request for compensation by saying, “It’s your issue.” Please help me on this issue. Thanks in advance.

    When a lever is pulled, the second row seat can move freely back and forth on a track until the back of the seat is returned to the upright position. I was driving at highway speeds and had to brake suddenly. The seat was at the back of the track with the lever pulled, and it slammed into the back of the driver seat, hurting my back. It stuck there putting pressure on the seat (which could be felt) until I stopped and had to use force to make it slide back.

    The driver side power sliding door failed to detect my [XXX] and ended up squishing it and we struggled for over a minute for the door to open.     Keeps bottoming out suspension is specification is stated at 5.6 inches and I measured 3.3/4 unloaded never get a response on my complaints from my last repair and resolution on what they admitted that suspension was too low and wouldn’t put it in writing only saying that vantage mobility saying that it’s height requirements are as stated by vantage but no mention of ground clearance that I wanted them to verify that it’s specifications are stated at 6.5 inches and i measured 3 3/4 empty then they tell me not to go by specs and I’m a retired from wright Patterson Air Force base in the engineering department with logistics command and I’m like really guys you have to to have a specifications for a engineered system and they can’t even call back after they admit you have to go by specs! And then the authorized repair specialist tells me i had a bad brace on the front tire that was welded wrong and didn’t even mention it on the invoice or reported it to vantage mobility to call me about anything I had concerns about ?

    Wobbling vibration on exterior mirror, driver side. It looks like incoming vehicles shake sideways. This happens on any bumpy road and it is easier to observe at highway speeds. Purchased on [XXX]. Vehicle is available for inspection. The distorted image resulting from mirror wobbling affects driver's awareness of surroundings, visibility gets worse at nights/rain due to blurred lights. This can lead to an accident. My regular dealerships in Milford, CT have confirmed the issue but have not been able to provide a fix. They say the vibration is normal. I do not believe so as the passenger side mirror never has issue like that. I have first noticed this issue a couple of weeks when driving on highway. Posting my case online has gotten at least 4 people reporting the same issue with the Sienna since 2021 in multiple trims.     On rare occasions (about once every 500 miles), when uniformly applying force to the brake pedal, the deceleration suddenly ceases and the vehicle seems to jerk forward. To a passenger, it feels like the driver's foot slipped off the brake pedal, but the driver continued to provide the same uniform force to the brake pedal and lost all deceleration. Typically, the driver is startled and presses significantly harder on the brake pedal jerking all occupants forward. The brakes re-engage and continue the deceleration at a much higher rate. Toyota addressed this problem with TSB 4724 which involved reprogramming the brake actuator assembly. This corrected the problem for about 3 months, but it is now occurring again. When the car was taken to the Toyota dealer again, the technician stated that TSB 4724 was cancelled, he could not duplicate the action (even though he was told it only happened on rare occasions), and claimed that the "brakes function properly as they should." Fortunately these loss of braking incidents have only occurred on dry pavement so that the driver did not lose control of the car when slamming on the brakes to compensate for the loss of deceleration. Coming down a hill on a snow covered road could have caused the vehicle to slide into an oncoming lane. It is my speculation that there is a null spot that exists during certain circumstances which causes there to be a "gap" between when this hybrid vehicle transitions between the dynamic hybrid brake and the friction wheel brake. Both braking systems independently work fine, but the transition between them (on rare occasions) does not occur continuously. This causes the driver to experience an unexpected loss of braking which, under the right conditions, could cause the driver to loose control of the forward motion of the car. As an engineer, I feel it is unacceptable for a safety system (braking) to not properly operate 100% of the time! On a Toyota owner's site, others have seen the same.

    We had a complete tire blowout of the rear driver tire in the desert, the temperature was 108° ambient. Toyota did not mention to us nor did the dealership that the car does not have a spare tire. We were 50 miles from any service Including cell service, I had to walk a mile to find cell service meanwhile two of our kids experience heat exhaustion. It took four hours to get the Vehicle towed. In that time our children experience heat exhaustion and had to be taken to the hospital. This is a huge oversight part to not make the customer aware that the vehicle especially family vehicle does not have safety equipment like a spare tire. In the desert this is a life and death issue that was not addressed by the dealership nor Toyota. All that is given is an inflation kit. We were not told by the dealership that a card did not come with What turns out to be essential equipment in the middle of a desert.

    When crossing a one-way road with oncoming traffic from the left, a vehicle that has recently passed might be triggering the forward collision avoidance system which cuts off throttle control to the Sienna, even when the passed vehicle is well out of the way. It is unknown if the collision avoidance feature is being triggered as there are no warning lights or messages in the display. This results in the Sienna having no accelerator response and sitting “dead in traffic” sideways in front of oncoming cars. This this is the second time this issue has occurred and today I was nearly t-boned by on-coming vehicles as there was no throttle response to move the car forward. This issue has been reported in 4th generation Sienna (‘21-‘24) forums on Facebook. This issue puts at risk vehicle occupants as well as other vehicles on the road.

    Three (3) separate systems failed. 1. Upon seeing cars ahead of us slowing down, my husband pressed the breaks to slow down to avoid collision. When he pressed on the breaks, instead of slowing down or stopping the vehicle, the brakes failed to engage, and caused a surge that compelled the vehicle to lurch forward. The lurch propelled our vehicle into collision with another vehicle. 2. Also, the forward collision warning system failed to alert or engage the breaks. Our vehicle was stopped by the impact of hitting the other vehicle, not by its own breaks. 3. The automatic breaking system also failed to engage at all prior to impact.

    The contact owns a 2024 Toyota Sienna Hybrid. The contact stated that while driving at an undisclosed speed, the hatchback independently opened. No warning lights were illuminated. The vehicle was taken to a dealer where it was diagnosed and determined that the hatchback needed to be recalibrated. The vehicle was repaired; however, the failure persisted. The vehicle was taken back to the dealer where it was diagnosed and determined that the hatchback needed to be recalibrated. The vehicle was repaired; however, the failure persisted.     I have never been in an accident in my 31 years of driving. However, I am worried that this van is going to cause me to be hit. Several times I have pulled out to make a left or right turn and my gas pedal does NOTHING. I can push it clear to the floor and NOTHING!!! Cars end up slamming on their brakes and honking at me but there's nothing I can do. This is EXTREMELY scary and a HUGE HUGE HUGE manufacturing flaw. There needs to be a recall for this to be fixed before people are injured.

    The 3rd row seats do not remain stowed when passing over a speed bump or dip in the road. Depending upon the speed of the vehicle, the seats might jump up several inches and almost return to the fully stowed position with a loud thud. The seats then bounce up a few inches from the fully stowed position and remain unsecured. It's possible that the seats might unexpectedly fully open at higher speeds.

    Rear Tail Lights Failed... Brake and Signals work. This was repaired through the dealership. 4 connectors and housing replaced invoice shows part 909890-10942. I am writing to express my concern regarding repeated tail light failures on my black Toyota Sienna that appear to be caused by wiring connection issues. This defect creates a serious safety hazard for my family, other motorists, and pedestrians, particularly in rural areas where there is little to no street lighting. I live in the country, where many roads are unlit and visibility at night is already limited. Because my vehicle is black, it blends into the darkness when the tail lights fail. Drivers approaching from behind may not realize my vehicle is on the road until it is too late to stop safely, significantly increasing the risk of rear-end collisions. This issue is especially concerning because I frequently transport children in my vehicle. Their safety depends on other drivers being able to clearly see my vehicle at night. A tail light failure caused by a wiring defect could place them at unnecessary risk of serious injury or death if another vehicle is unable to see us in time. The defect also puts other drivers at risk. Motorists traveling at highway speeds on dark rural roads rely on tail lights to judge the presence, distance, and speed of vehicles ahead. Without functioning tail lights, they may have little or no warning that another vehicle is in their lane, particularly around curves, hills, or during poor weather conditions. This appears to be more than a routine bulb replacement issue. If the root cause is an unreliable wiring connection, the lights may work intermittently or fail unexpectedly, making it difficult for owners to recognize the problem before driving. An intermittent electrical failure is especially dangerous because drivers may believe their lights are operating normally when they are not. Started a case with Toyota 260715001670

    Moving about 55 miles per hour on right lane of I-95 the Jeep in front of me braked abruptly. I braked hard but as my Sienna was quickly slowing down to stop, the brake pad felt spongy when it was slower and as I pressed even harder, it seemed to have stopped decelerating at that slower speed and that was when my Sienna slammed into the back of the Jeep. Happened today on July 15, 2026 at around 4:24 EST. I will also file this same report to Toyota. My wife and I are at the hospital getting checked, but my Sienna looked undrivable.

    My child [XXX] ) engaged the automatic locking retractor (ALR) and as it wound back in it wrapped around his neck. By the time we got to him it was tight enough that it was difficult to move or rotate or slide the seatbelt over his head, and we ultimately cut the seatbelt to free him. In addition to the actual safety incident, I was very surprised to learn that this was even possible. I had never heard of this safety issue, and had absolutely no game plan or awareness about this issue. I saw the SBIR program addressing innovation in ALR systems which is exciting! However I would also strongly encourage some sort of awareness campaign or warning messaging to raise awareness and ensure parents keep belt cutters in their cars.     Rear foldable seats do not lock down into compartment. They bounce up when you stop just a little hard. Cable has already broken when this occurs. Anything laid on top of these seats when folded will be ejected inside of car. Hitting driver in the head. This is very dangerous. Already brought to dealer and complained and nothing was done. They said that's the way it is. Someone will die if this problem is not taken care of.

    The power liftgate on my 2024 Toyota Sienna consistently fails to complete a full close-and-latch cycle. When activated, the liftgate begins its closing sequence normally but then aborts the cycle during the final stages of latching. The system emits an audible beeping signal and the liftgate reverses direction, reopening approximately one-quarter of the way. This failure occurs repeatedly when the vehicle is stationary with no external obstruction present. Multiple repeated attempts to close the liftgate using standard operation (key fob, interior switch, and exterior switch) result in the same failure pattern. The system does not complete a secure latch. Eventually, the liftgate can only be fully closed manually with physical force, bypassing the powered closing function entirely. This condition occurs intermittently at times but has become frequent and repeatable enough to prevent reliable use of the power liftgate system. No obstructions are present during operation. The local Toyota dealership service department observed this and indicated a lift gate adjustment is required, noting it is a known issue Toyota Customer Care has indicated they have no documented record of a known defect relating to this condition. However, multiple independent owner reports and dealer-level discussions indicate similar behavior may be occurring in other vehicles of the same model line, suggesting a potential systemic issue that may not yet be formally recognized. This malfunction raises potential safety concerns, including but not limited to: failure of the rear liftgate to fully secure and latch, increased risk of the liftgate remaining partially open or unlatched without clear driver awareness, and reliance on manual force to override an electronic safety system designed for automatic secure closure. I request that this complaint be documented for investigation and review to determine whether a defect trend exists in the power liftgate system of the Toyota Sienna platform.

    On June 5 th the vehicle was going approximately 50 miles per hour in the middle lane. I placed on the brakes suddenly and car lost power and stalled out. Unable to restart vehicle for 5-10 minutes on the freeway. Significant safety concern due to vehicles sudden stall out in mid the highway traffic and unable to restart car. Almost hit from behind as well as others that followed from behind. On Jun 7th vehicle was taken to the Toyota dealership and car checked by special mechanic and reported that they were unable to reproduce the stalling or issues with restarting the vehicle.Dealership feels there is nothing wrong with vehicle. There was 2 warning messages on vehicle when stalled suddenly 1-Charge 2- Fluid

    When going down hill while braking, the brakes will release causing the car to lurch forward and then they grab again. It’s intermittent and Toyota has a TSB on it but the dealership with not fix it because they can’t duplicate the problem. It can cause people to take their foot off and of the brake pedal and trying to apply it again. Which can cause someone to rear end another car.

    The lift gate strut bent when I tried to close the rear door, then when I tried to open the door to see what the issue was, the strut exploded. The parts went flying and almost hit me in the face, I could feel the air on my cheek as it blew past me. The spring and metal parts inside the strut shot all the way across my cul de sac, and the noise it created sounded like a gunshot. My neighbors came out of their houses to see what happened because it was so loud. We have an appointment at the dealer tomorrow to get it looked at. If I was standing any closer the spring would have hit me in the face; my children luckily were in the car already, if it happened a minute earlier it could have hit them. I noticed after the fact that the gaps in the back door are uneven when it’s closed, and I wonder if the door is installed unevenly and that put pressure on one side more than the other causing the strut to break. I can’t take a picture of the broken strut because I am too afraid to open the rear door in case the other one explodes.

    I am the original owner of a 2024 Toyota Sienna Hybrid (VIN: [XXX]). The vehicle has required two significant engine repairs within the first 57,000 miles of ownership. Both repairs were performed at authorized Toyota dealerships under warranty. INCIDENT 1 — ENGINE REPLACEMENT (10,000 MILES): At approximately 10,000 miles, the engine developed severe knocking. The dealership determined the engine was defective and performed a complete engine replacement under warranty. INCIDENT 2 — OIL LEAK (57,000 MILES): At approximately 57,000 miles, the engine developed an oil leak between the oil pan and the timing belt cover. The dealership repaired the leak under warranty. The dealership informed me that this repair carries only a one-year warranty on the work performed. The fact that the engine required complete replacement at only 10,000 miles indicates a manufacturing defect. The second engine issue at 57,000 miles suggests a pattern of powertrain reliability problems. I contacted the Toyota Brand Engagement Center (Case #XXX) and spoke with both an advocate and a supervisor. Toyota declined to offer any extended powertrain coverage and would only agree to review future issues on a "case-by-case basis." I am concerned about the safety and long-term reliability of this vehicle's powertrain given the recurring nature of these engine failures. A complete engine failure at 10,000 miles on a brand-new vehicle is not normal wear and tear — it is a defect. I am reporting this so that NHTSA is aware of potential systemic engine issues with the 2024 Toyota Sienna Hybrid.     In general the braking system on this vehicle is poor. The van does not seem to want to stop unless you apply a lot of pressure on the brake pedal. I have taken the vehicle to the dealership who explained to me that the braking operation is normal. I replied “so the poor brake design was intentional”. The dealer said the braking system takes a little while to get used to. I have contacted Toyota corporate 5 times to express my concern about the brakes. The customer service representatives say that they will pass my concerns along. I personally own 6 vehicles. The brakes on this Toyota are by far the worst. It is not even close. I do not feel like this vehicle is safe. This is a minivan that is used to move families around. I do not feel safe in this vehicle and most importantly I am worried every time my wife drives this vehicle that someone will slam on the brakes in front of her the van will not stop in time. Toyota is the world’s largest auto maker, they can and should produce a better braking system than what is available with this van. This is unacceptable with braking technology available at this time in automotive history. Their lack of response to my concerns in also disappointing. Thank you for hearing my concerns.

    I was at a complete stop. There were no obstacles around me or oncoming. When I pressed the accelerator nothing happened for a full 3 seconds, until my vehicle jolted forward at an excessive speed. Brakes were not working.

How was your safety or the safety of others put at risk? -The engine went out at 77k miles in the middle of driving with no warning. Could have caused a serious accident. We have always done proper oil changes. The rods separated from the pistons and punched a hole in the engine block. Has the problem been reproduced or confirmed by a dealer or independent service center? The dealer diagnosed it and our mechanic said this has happened with Toyotas too recently Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? -no Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? -no warnings or messages before the failure of the engine.

    While applying the brake at the stop light. (25 to30 mile speed limit zone) The Toyota hybrid sienna start to accelerate for split 5 or more seconds. I have to apply the brake harder even though I was already applying the brake to slow down because the light was red and the are several cars in front of me. For the first time I was scared thinking about rear ending some one in front of me or the Toyota Lexus incident where it accelerated and everyone died flashing through my mind. It was not a good feeling.

    I notice the driver seat had a clip that popped off unto my carpet, when inspected it came off the seat rail. While moving my seat back using the electronic seat lever a white clip with two metal bearings was sticking out, I notice the ball bearings were not in place and had came off the white clip. Yes, it is available for inspection. This pose a safety concern as the seat rails will not function correctly when moving seat or using the memory seat buttons for each driver. I have not brought the vehicle to a dealer to inspect as there is a estimate fee involved. I am currently out of my 2 year toyota care warranty. I am not seeing any warning lamps or messages besides the seat making a clicking noise when I move the seat back and forth.

    Stepping on brakes - Car slows then accelerates a few seconds and lunges forward a couple of feet. Extreme safety issue. This occurred yesterday with my sister in the car and we almost rear ended the vehicle stopped in front of us. I have complained to the Cumberland Toyota dealership in the past and was told if they cant recreate the issue and it doesn't happen all the time there is no way to fix the issue. Yesterday I was almost in an accident because of this faulty design/anomaly associated with my car. This lunging forward is not suppose to happen and I want it fixed Toyota needs to come up with a resolution before it kills me. I have purchased this vehicle new 2024 and it has been happening since purchase - NOW its really bad

    There are no messages/warnings. The vehicle will surge at very low speeds such as when I am coming to a stop, trying to avoid a person in a parking lot etc. It was SO scary this evening as I accelerated into a very very busy intersection...I pressed on my brakes harder and it was instant - but about 1 second (which is a LOT in this scenario) later it then slowed instead of accelerated. I have owned the vehicle almost a year. I will be bringing in for inspection this week and let the dealer know. I have put on about 11K in miles and would say this has happened a dozen times thus far with this evening being the scariest one yet. I figured it was just something normal for a hybrid but as I researched online - many people have this issue. TOO SCARY for me and dont want to hurt anyone or myself.

    The driver's side mirror vibrates when the vehicle is at speed. I've been to two dealers who say that it's normal, and everything is intact and installed correctly. When driving, the mirror vibrates a lot, which is a safety issue when trying to use the side mirror during changes, etc.

    The interior dash lights, when turned up to max brightness, take several seconds to read when making adjustments to the climate controls. These dim controls pause a hazard as it takes your attention away from driving. The dealer told me that they could not make them brighter. The issue is at night and day times.

    When I am rolling to a stop, or lightly pressing in the brakes, the car will occasionally jolt forward and I have to slam on the brakes. It is not every time but has happened many times since buying the car new. I brought it into the dealership and they could not replicate it, but it makes me so scared to drive. This could result in an accident or the hitting of a pedestrian. No warning lights.

    The third-row seat does not lock in the stowed position. As one goes over speed bumps etc. it bounces up and down. The is no positive lock to ensure the third-row seat is secured in the stowed position. In the event of a head on collision the seat may fly out and forward either killing or injuring the passengers.
