Tesla, Inc. (Tesla) is recalling certain 2024 Cybertruck vehicles equipped with a service-installed off-road light bar accessory. An incorrect surface primer may have been used to attach the lightbar to the windshield, causing the light bar to loosen and detach.
Risk: A detached light bar may fall, creating a road hazard for other vehicles and increasing the risk of a crash.
Fix: Tesla Service will inspect the light bar and install an additional mechanical attachment or replace the light bar using tape to adhere the light bar to the windshield as well as an additional mechanical attachment as necessary, free of charge. The contact owns a 2024 Tesla Cybertruck. The contact stated that while driving at a moderate speed on a windy day, a metal piece, approximately 2-3 ft. long and located on the side of the sunroof, detached from the vehicle. The contact managed to recover the piece that detached from the vehicle. The vehicle was taken to a Tesla service center, where the failure was acknowledged the failure, and the contact was provided an estimate for the repair. The contact stated that the failure was a manufacturer's defect and that the repair should be free of charge. The vehicle had yet to be repaired. The failure mileage was approximately 11,000.

The contact owned a 2024 Tesla Cybertruck equipped with Pirelli Tires, Tire Line: Scorpion ATR, Tire Size: 285/65/R20, DOT Number: N/A. The contact stated that while driving at 70 MPH, he heard a loud bang, and the vehicle veered to the left. No warning light was illuminated. The contact attempted to steer right, but the vehicle failed to respond. The vehicle struck another vehicle in the left lane before both vehicles crashed into the median barrier wall. The contact's vehicle then spun into three right lanes and came to a complete stop on the right shoulder. The air bags were deployed. Upon inspection, the contact discovered that the rear driver's side rim was fractured. The contact and three passengers were taken to the hospital via ambulance. The contact sustained third-degree burns on his arms as a result of the air bag deployment. The second passenger had cuts on the head from the seat belt. The third passenger had cuts and bruises on the face from the seat belt. The fourth passenger sustained no injuries. A police report was filed and stated that the failure was caused by the rear driver's side tire blowout. The vehicle was towed to a tow yard and was deemed totaled by the insurance company. The manufacturer was not made aware of the failure, but the contact obtained the event data report and summary. The failure mileage was approximately 8,504.

I believe there was an issue with the ABS system that kept me from stopping the vehicle, as speed was increasing while going downhill with the brake pedal pushed hard. I was forced to make the choice to steer the vehicle into a mountainside to stop it before I ended up over the side of the mountain road. The vehicle then tipped over, coming to a stop.

While driving on a local road and thank fully at speed of 20mph vehicle lost all power, in few seconds of alerts steering failed as well as brake failed. Car slowly coasted to stop and died completely. Steering and brakes were completely unresponsive all of a sudden while driving. Was only able to get out of car via emergency exit. If this was on a highway at highway speed am very positive it would have been a disaster.

The newest Tesla vehicles coming out of the factory with an unannounced HW4 computer upgrade, dubbed AI4.1, are seeing them short-circuit and die down shortly after purchase. In some case, the new AI4 computer gives up the ghost after several hundred miles, rendering not only the FSD feature, but the cameras and GPS navigation hence range estimates unavailable, too. I believe it's a safety defect and the VIN was used to be able to submit the form, it seems to affect most new Tesla vehicles. Thank you

I was driving in a rain storm and the wiper started to malfunction. Would only clean half the windshield. Then shut off completely. Made a weird noise and started smoking in the front.Started working for on wipe then did a 360 on front end of the car. Wiper blade flew off and wiper arm Would not stay in position. Was able To get home but had to get the car towed to Tesla.

When driving during a snow storm, the headlights of the Cyberteuck become covered in snow and ice. The main headlights that produce most of the light are just under the frunk in an inset groove. Because of the nature of the design it catches snow it collects in that location. Because of the LED headlights there’s not enough heat to melt away the snow. At night your lights slowly fade away until you can barely see anything. Photo is not my vehicle but my friend’s who reported same problem.

On Saturday, [XXX], vehicle was at a T-intersection, in normal drive mode (not FSD) waiting for traffic to pass to enter roadway from side road. Vehicle suddenly started to go backwards. Had to press on the brake to stop. Rebooted the car and the error happened again. Tried to reverse from the intersection to be able to stop the vehicle in a safe location, but steering (drive by wire) was not working. Had to drive with one foot on the brake and one on the electronic throttle to go backwards, even though vehicle was in drive model. Display read 'Traction Control Disabled', 'Adaptive Rider Control Degraded', 'Stability Control disabled', and 'Emergency braking not available'. Tesla service was requested and vehicle was transported to Tesla facility in Tampa. I am very concerned that had this failure occurred while on interstate could have resulted in a serious accident. I am submitting this safety complaint as the vehicle owner of a Tesla that was involved in a collision on November 15, 2025. Although I am the policyholder and registered owner, I was not the driver at the time of the incident. My husband, [XXX] , was operating the vehicle, and Tesla Autopilot/FSD was engaged when the loss of control occurred. At the time of the incident, it was raining heavily, though visibility and road conditions were still within what Autopilot typically handles during normal operation. While driving under these conditions with Autopilot activated, the vehicle suddenly exhibited unexpected and erratic steering behavior, causing it to lose stability and veer out of control. The deviation happened rapidly and without sufficient warning or opportunity for my husband to intervene manually. The resulting collision appeared to be the result of an Autopilot system malfunction, not driver error. Key concerns include: Autopilot’s failure to maintain proper lane position Sudden abnormal steering input originating from the system Possible sensor, perception, or weather-related detection failure Lack of adequate warning or disengagement alert prior to the loss of control Tesla Insurance later determined the driver to be “principally at fault,” but this conclusion did not consider the possibility of Autopilot system failure under heavy rain conditions and does not reflect the sequence of events as experienced by the driver. As the vehicle owner, I am requesting that NHTSA review this incident as a potential safety defect involving Tesla’s Autopilot/FSD system. I am concerned that this malfunction—particularly under adverse weather—may also affect other Tesla owners and could indicate a broader systemic safety issue. NHTSA has created a safety risk by forcing FSD to have extreme monitoring. While in FSD I have to go back to normal driving to change the station on the radio or look at my phone. This is something I do in my non fsd vehicles all the time. The reason for me purchasing FSD is to be safer. Your ridiculous overreach may actually kill me.The entire purpose is to reduce distracted driving, yet your ridiculous rules force an unsafe environment. I hope whoever pushed these restrictions realizes they are accountable for the crashes and deaths they create for forcing people out of FSD.

In cases where design constraints prevent a single CHMSL, two identical lamps are permissible, provided they are symmetrically mounted above the other stop lamps. 2. Cybertruck's Rear Light Bar: The Cybertruck’s rear light bar does not appear to comply with this standard, as it presents a continuous light bar rather than a distinct, centrally mounted CHMSL or two identical, high-mounted stop lamps.

While exiting the Chick-fil-A drive-thru lane, I lightly pressed the accelerator to move into the next lane. Suddenly, the vehicle jerked forward very quickly, as if I had floored the accelerator. At the same time, the display showed "Emergency Braking in Progress." It felt as though the truck briefly braked and then rapidly released, causing unexpected acceleration. No collision occurred, but it was startling and very scary.

The contact’s father owns a 2024 Tesla Cybertruck. The contact’s father stated that while driving at an undisclosed speed, the Autopilot function became inoperable. The contact stated that while engaging the Autopilot function and attempting to make a turn in a residential area, the vehicle unexpectedly accelerated and collided with a tree. The air bags did not deploy during the incident. As a result of the crash, both the contact’s father and nephew sustained injuries and received medical attention at a local emergency room. The contact’s father sustained a rib injury and bruising, while the contact’s nephew suffered an ongoing back injury due to the incident. A police report was filed. The vehicle was towed and taken to a body shop. The dealer was contacted; however, the vehicle was not diagnosed or repaired. The manufacturer was made aware of the failure, and a case was opened. The approximate failure mileage was 1,500.

Component/System Failed: The primary component failing is the Front Overhead Console Assembly (P-R-N-D manual gear selector/shifter and hazard light switch). The assembly detaches, rattles, and fails to remain securely affixed to the headliner. The defect has been present since delivery (Aug 2024, 208 miles). Safety Risk: The overhead console contains the crucial backup gear selector and the hazard light button. Should the vehicle’s primary touchscreen fail (rendering the main gear selector unusable), reliance on this repeatedly detaching or faulty manual shifter compromises the driver's ability to safely operate the vehicle (Park, Reverse, Neutral, Drive) or activate emergency hazard lights. Problem Confirmation/Inspection: The failure is confirmed by the manufacturer (Tesla Service Centers). Tesla attempted to fix the overhead console defect at least four times. The defect was confirmed again on October 22, 2025 (11936 miles), where Tesla noted only a "slight adjustment... as a temporary fix" and stated a "Permanent fix will be performed on the upcoming visit". The vehicle and its components have been inspected repeatedly by Tesla technicians. Warning Lamps/Symptoms: The primary symptom is the physical rattling, looseness, and detachment of the overhead console/gear selector. The center console can come off at ant time that the windshield visor is detached from the console. This has happened to me several times while I was driving and it caused the cybertruck to shift out of gear and has changed the gear to neutral and park on different occasions. The first time this happened while I drove, I was driving down the street and the car came to a hault when the gear shift cover came loose. This could have caused a serious accident, but fortunately it didnt. This problem existed from the time of delivery & we had to make our 1st service apt on the day we took possession of the truck. This happened several times afterwards and tesla was unable to fix this in 7 attemp

We received a notice from Tesla about a recall involving the cant rail. We reached out to Tesla and they said we must go to a Tesla dealer and the mobile team will not be able to do this repair. We are in South Dakota and the nearest dealer is 6 hours away in Colorado. We asked Tesla to give us a name of a local shop who can do the repair and they said we must go to Tesla. They are unwilling to handle this recall.

On [XXX], [XXX] sat in the left rear-seat of a Cybertruck that was involved in a solo-vehicle collision. [XXX]'s impact injuries were minor. The truck caught fire. The right rear door remained mechanically openable, but its ordinary operation depended on low-voltage electronic buttons located inside the cabin and on the exterior pillar. Those buttons failed after the crash. A man tried to rescue the occupants, but he could not open the doors because the buttons failed and the truck lacks exterior handles. The "armor" glass made it difficult for the rescuer to break open the windows. The mechanical backup releases for the rear doors are concealed beneath the map pocket liner at the bottom of the doors — not feasible to find or use in the smoke, heat and chaos of a post-crash fire. This obscure release left the rear seat occupants with no practical means of egress. [XXX] died of smoke inhalation and burn injuries because he was trapped inside the Cybertruck. On [XXX], [XXX] was a right rear passenger in a 2024 Cybertruck. The driver lost control and struck a tree with the driver side against a wall. The truck immediately caught fire. The operation of the doors was powered by a small battery pack which was no longer operational after the impact. The passenger side doors operate off of that battery. Without power, the passenger doors could not open, trapping the driver and passengers in the Cybertruck. The fire moved quickly to the passenger compartment and towards the trapped passengers. The design of the electric doors required passengers who are not familiar with the vehicle to find the location of a manual release hidden from view beneath a panel to remove that panel and pull the manual release mechanism. There are no instructions present to explain to a passenger how to access the manual release. The truck has no exterior door handles. The only means to open the door from the outside was a button located at the base of the B&C pillars. When the electric system failed, that door release button became useless. A rescuer arrived but had no way to open the passenger side doors from the outside. The truck’s “armor glass” windows made forcing entry extraordinarily difficult. The rescuer was able to break the passenger side window with repeated strikes and attempted to rescue [XXX] by extracting her through the front passenger window but was unsuccessful due to the heat from the fire. She was forced to retreat back to the rear seat. She survived the initial collision with no injuries and was fully conscious. She was unable to escape the Cybertruck and was killed due to smoke inhalation and thermal injuries. The failure of the vehicle’s design to provide emergency egress in the event of crash, power loss, and fire created a foreseeable and unreasonable risk of occupant entrapment and death in the event of a collision resulting in power loss. The contact leased a 2024 Tesla Cybertruck. The contact stated while driving 25 MPH, the windshield wipers turned off. The contact stated that the indicator was inoperable. While depressing the left turn signal, the blinker was slow to respond. While driving on a narrow street, the vehicle swerved to the right. There were no warning lights illuminated. The contact received a recall letter related to NHTSA Campaign Number: 25V170000 (Structure). The vehicle was taken to an independent mechanic who referred the contact to the dealer. The contact called an unknown dealer in Las Vegas, NV, but the vehicle was not diagnosed or repaired. The manufacturer was called, but there was no response for two hours. The contact eventually spoke to a representative who suggested that a service appointment should be scheduled using the App. The failure mileage was approximately 5,000.

In a rain situation the wind shield wiper is not operating when it’s pouring rain outside or even a drizzle. The wiper never goes off. It has to be manually turned on for the windshield wiper to actually work. Also, the lighting on the vehicle is very dim because the vehicle pretty much only operates off of two tiny fog lights, and in a low visibility area there’s not enough lighting to actually see the road and bicycle is biking on the roadways even in a dark situations, it’s the same thing not enough lighting to actually see anything on the road.

Took vehicle in for service they replace the wiper motor under warranty with the faulty one that is more unsafe than the previous one. And they refuse to fix it. Research shows they knew about this for months, yet continue to risk owners safety by replacing working units with faulty ones. Then have it taken or leave an attitude.

We were driving on the freeway yesterday September 3rd at 9:30pm and my passenger side door swung open while on the entrance ramp to highway 24. My infant was sitting in that passenger side in her carseat and thankfully she wasnt injured. A car could have collided with that door or it could have hit anything outside resulting in extreme damage, injury, or death. Upon inspection of the door latch, the door latch is almost completely unhinged. The screw cam off, and the latch simply swivels and does not lock the door or hold it in place. We had to drive home with our hand physically holding the door closed. My driver side door also seems to not be fully latching - though it is not in as bad condition. But upon driving it, you can feel an airgap with the sound at higher speeds which suggests to me that this door latch is also at extreme risk of failure. These two doors are the only ones commonly used by us. But given they are both seemingly failing, my guess is that all four door latches are defective and at immediate risk of causing harm, injury, or death to those Cybertrucks on the road with the same latch.

The left sail applique was nearly disconnected from the vehicle, so I removed it the rest of the way manually before it could fly off while driving at speed. The right one is still a little loose, but doesn't seem to be in danger of flying off. The model number stamped into the piece that came off indicates that it was replaced during recall work in Nov 2024 as part of Safety recall 24V-457, however because work had been done on the tonneau cover by Tesla in Jun 2026, I believe it exacerbated the problem causing the part to detach enough to be dangerous. They did not mention the problem to me at the time even though the vehicle was specifically brought into the service center prior to warranty expiration to check for covered issues. Tesla is now quoting $1190 and refusing free repair because the mileage is 53,500. I asked for, but did not receive a copy of the final estimate with the updated parts listed for $1190. I did upload a preliminary estimate where they showed the parts that would have been needed if the vehicle had the new rails installed.

Received a “critical error” message on the screen and was advised to pull over. After doing so and putting the vehicle in “park” I was unable to take the vehicle out of park or move it at all. This would have been very dangerous if it occurred in an unsafe neighborhood or busy highway. Couldn’t even take it out of park for the tow truck driver to get it onto the flatbed so he had to use “skates” to pull it onto the truck. There were many different error messages appearing on the screen related to different systems in the car so I suspect some sort of electrical defect caused multiple systems to fail simultaneously. Subject: Tesla Cybertruck Recall – Out-of-Pocket Wrap Costs I own a Tesla Cybertruck (VIN: [XXX] ) affected by the recall on Cant Rail Assemblies. Tesla requires the replacement and welding of new assemblies. My truck is custom wrapped by Rogue Films, a Tesla-recommended supplier. Rogue Films quoted $850 to re-wrap the affected areas after the recall repair. Tesla is only offering $300, leaving me to cover the remaining $550. This cost is a direct result of Tesla’s recall, not elective customization. In addition, Tesla does not provide a loaner vehicle during the lengthy repair process, adding further burden. I am filing this complaint because customers should not have to pay out-of-pocket expenses to remedy a manufacturer’s defect under recall. Large driver's touchscreen in the Cybertruck gets really, really hot. Brought it to Tesla for a service today (August 5th, 2024) and they said the screen is working as expected. They showed me another vehicle and it also had the 110 to 125 degree screen. This can burn skin if you're older and lacking nerves or a child (which Tesla encourages to use the screen with their video games). As this is a design defect and not a one-off part issue, I'd personally like to see this recalled. There are other people complaining about the same issue on this very new vehicle -- [XXX] Worth noting that this high temperature is produced in the lower center area of the screen. Tesla Service says it's due to the computer processor being located in that area with no way for the heat to dissipate (ie, a TV would have room behind it to dissipate the heat, but the Cybertruck has that area blocked). On XXX at approximately 5:15 AM, I was driving my 2024 Tesla Cybertruck (VIN [XXX], ~47,300 miles) on an interstate trip to [XXX] with Full Self-Driving (Supervised) engaged. I heard a loud pop from the front of the vehicle, immediately followed by an alert stating the left front turn signal was malfunctioning and that I needed to take over control. FSD disengaged and I was forced to manually drive in pre-dawn darkness with a failed turn signal until the next charging stop. The fault then behaved intermittently: it cleared on its own, returned, cleared again, and I resumed FSD for much of the trip. Then, with roughly 100 miles remaining, the left turn signal malfunction warning returned while the vehicle was actively driving itself, again forcing manual takeover and leaving me without a functioning left turn signal in traffic. This is a serious safety defect on multiple levels: (1) A failed turn signal prevents me from lawfully and safely signaling lane changes and turns to other drivers. (2) The failure occurred WHILE the automated driving system was operating and depends on functional signaling to execute its own lane changes — yet the system had been driving with a component that failed audibly and without warning. (3) The intermittent nature means the vehicle can re-enter a hazardous state at any time, without warning, at highway speed. (4) I cannot get a service appointment immediately, so I must continue operating a vehicle with an unrepairable intermittent signal failure, exposing me to citations and collision risk. This vehicle has an extensive repair history including body panel adhesion defects related to Tesla’s 2025 cant rail recall, and this electrical failure continues a pattern of defects across multiple systems. No warning preceded the pop. The failure is reproducible in the sense that it has recurred three times in one day. Service records and buyback correspondence with Tesla are available on request. On May 15, 2025, at around 11:30 AM, my Tesla was involved in a collision while Full Self-Driving (FSD) was engaged. The incident occurred at the Tesla West Covina Dealership. The vehicle struck a barrier wall without any collision warning, suggesting both a failure of the sensors and FSD system. After the incident, Tesla staff directed me to their West Covina Service Center. A service advisor reviewed the dashcam footage with me and acknowledged the crash appeared to result from FSD malfunction. I was told Tesla will cover the repair. However, when I returned on May 26, 2025, I was informed that the dashcam footage was no longer available. I did not delete it and suspect it may have been removed by Tesla personnel. They also then claimed they will not cover any repairs. I have written to their legal department to formally request the video footage and any other data they have regarding the crash. Despite follow-up, Tesla has not responded or released any data. I am reporting this as a possible safety defect and mishandling of evidence. I request NHTSA investigate: The failure of Tesla’s FSD and collision avoidance system. The disappearance of crash footage after review by Tesla staff. Tesla’s lack of response to requests for data and documentation. Thank you for your attention.

The PCS (Power Conversion System) failed on my Cybertruck as it has on thousands of other trucks. It's an issue for me because the nearest supercharger station where I can charge is 45 miles away. I have to budget my daily driving so I don't run out of charge before reaching the supercharger. If I run out of charge, the truck will be inoperable until the service center can fix the vehicle. Right now the service appointment is 24 days from now.

My 2024 cybertruck has a PCS2 onboard error. It has failed but Tesla has told me it’s under warranty however if I was past the warranty it wouldn’t be covered. This is a widespread issue known on 2024/2025 cybertruck. Nearly all VINs are affected.

I paid Tesla to perform a safety inspection of my Tesla Cybertruck after I purchased it. During that inspection, Tesla determined that the vehicle's as built configuration was **not safe** and required me to purchase a newer revision of the Power Conversion System (PCS). The required PCS revision did not exist when my vehicle was originally manufactured, meaning the vehicle could not have been built with that revision. The original PCS installed in my vehicle has a widespread failure rate among Cybertrucks. When the PCS fails, the vehicle is unable to charge, leaving it stranded. I believe this is a safety defect that warrants investigation by the NHTSA. Tesla's own words on their invoice state that the as-built configuration was determined to be unsafe, and required me to purchase a newer revision of the PCS (1777777-T2-G) to address the safety issue.

The vehicle has experienced an issue with its power conversion system (PCS2) that leaves AC charging unavailable. Based upon online forums and social media, this issue is clearly widespread amongst early VIN model Cybertrucks. The scope of the issue has lead to delays with repair. My vehicle has experienced this issue since early June 2026, and I am still awaiting repair. This is a safety concern because it could plausibly leave the vehicle stranded due to lack of ability to charge the vehicle. While Tesla and third party superchargers are widespread, they are not everywhere and cannot allow a vehicle to travel to most roads in the United States. There have been circumstances where, if AC charging was not an option, I would have been stranded in a desert and surely would have died of excessive heat exposure. With the PCS2 failure, this issue a plausible risk.

I am leasing a 2024 Tesla Cybertruck and have experienced multiple serious and unresolved safety issues. When driving at speeds of 50–60 mph or higher, the Cybertruck begins to shake aggressively, particularly in the front wheels. The vibration makes it difficult to maintain control. I took it to the Tesla dealership, and service staff confirmed there is a known malfunction with the Cybertruck tires. They informed me they are waiting on new tires to arrive, but offered no timeline and sent me home with the issue unresolved. No loaner vehicle was offered, no workaround was provided, and I am still driving a vehicle Tesla knows is defective. In addition, the rear windows malfunction frequently. When I close the rear doors, the windows often jam or freeze. This concerns me because the Cybertruck is built entirely with electronic door systems—there are no manual overrides. If power is lost during a crash, occupants may not be able to exit. This is even more alarming in light of recent reports of Cybertruck-related deaths. Multiple individuals have died in crashes where the battery was damaged or the vehicle caught fire. Power loss caused the doors and windows to become non-functional, and due to Tesla’s armored glass, bystanders couldn’t break the windows to rescue the victims. These people burned alive while trapped inside. This is not a theoretical issue—it has happened. Despite these risks, Tesla has not issued a recall or urgent fix. I’ve made multiple efforts to resolve these problems, but Tesla’s delays, denials, and lack of proactive safety measures are deeply negligent. This is a dangerous vehicle, and I am requesting that NHTSA immediately investigate both the front-end tire instability and the entrapment risk caused by electric-only doors and unbreakable windows. I no longer feel safe driving this Cybertruck and believe others are also at serious risk.

The car would stop charging at home and an error showed up about two months ago. After reporting to Tesla, they did a diagnostic over the air and concluded that the battery (HVB) needs to be replaced. The car is at the shop for over a month, and they are still waiting on parts.

I am filing a safety defect complaint regarding my Tesla Cybertruck. Water entered the truck bed outlet area and caused corrosion to the mounting bolts and surrounding components, resulting in Tesla determining that the entire bed wall panel requires replacement. The bed outlet is designed for outdoor use and exposure to weather conditions. In my opinion, water intrusion into this area represents a design defect because moisture can enter the electrical outlet assembly and associated mounting hardware, leading to corrosion and component deterioration. My concern is that continued corrosion could potentially affect the integrity or functionality of the electrical outlet system and create an electrical safety hazard for owners using the bed power outlets in normal outdoor conditions, including rain or vehicle washing. Tesla inspected the vehicle and confirmed water ingress and corrosion were present and advised that replacement of the bed wall panel was necessary to correct the condition. I request that NHTSA review whether the design of the Cybertruck bed outlet and surrounding components adequately protects against water intrusion and corrosion during normal vehicle use.
