    Risk: A blank video display can result in a loss of critical safety information, such as the speedometer, warning lights, and rearview camera image, increasing the risk of a crash.

    Fix: The video display control module software will be updated by a dealer, or through an over-the-air (OTA) update, free of charge. Owner notification letters are expected to be mailed July 14, 2025. Owners may contact Cadillac customer service at 1-800-333-4223. GM's number for this recall is N252500680.

    General Motors, LLC (GM) is recalling certain 2023-2024 LYRIQ vehicles. The bracket that anchors the left-rear and center-rear seat belts may have been improperly welded to the seat frame.

    Risk: An improperly welded seat belt bracket may not restrain an occupant as intended, increasing their risk of injury during a crash.

    Fix: Owners are advised not to use the left-rear and center-rear seating positions until the recall repair is completed. Dealers will replace the rear seat cushion frame, free of charge. Owner notification letters were mailed January 17, 2024. Owners may contact Cadillac customer service at 1-800-458-8006. GM's number for this recall is N232425220.

    Fix: The vehicle cockpit unit (VCU) software will be updated by a dealer or through an over-the-air (OTA) update, free of charge. Owner notification letters were mailed March 16, 2026. The left or right front stabilizer bar bracket bolts may have been improperly tightened, which can allow the bolts to loosen and separate from the vehicle frame. A detached stabilizer bar or bracket can damage the vehicle's high voltage cables or EV battery coolant lines.

    Risk: Damaged cables or coolant lines can result in a vehicle shutdown, causing a loss of drive power and increasing the risk of a crash.

    The vehicle's instrument panel automatically dims excessively at dusk and dawn. When it does this, the speedometer and other information in the dashboard are not visible. There is no way to adjust the brightness any higher and no way to disable this auto dim feature. This is, to my knowledge, a known software bug that has persisted for close to two years without any effort by GM to rectify this safety issue.

    When approaching sunset, or when travelling under bridges, the dash dims excessively and is barely readable. I have to take my eyes off the road for extended periods of time to be able to see the information. The excessive dim does not reset itself, when the ambient lighting returns to normal, for a prolonged period of time. Complaints to the dealer service department, GM EV Concierge, and OnStar have not had any effect on resolving this. Tnis is an ongoing problem since delivery of the vehicle.

    Unable to get display lighting bright enough to adequately read instrumentation when coming out of garage into low atmospheric lighting. Have instrument display lighting rheostat turned up to maximum but sensors don't allow the display to brighten. The vehicle lighting sensor is not blocked. The rheostat should be able to turn the display to full bright, but isn't designed that way. Definitely a safety issue. Cadillac dealership says there is nothing they can do about this situation, but acknowledge it's a widely reported complaint. The manual rheostat should be able to turn the display to full bright by selecting the maximum setting. Unfortunately it's not designed that way.

    While we're driving the vehicle the adaptive cruse control was on. As we approached a vehicle ours started to slow down from 55mph to where it almost braked all the way about 2mph. The vehicle in front of us braked completely and was stopped. As soon as they were stopped, Our vehicle tried to rapidly accelerate into the back of it where we had to intervene and slam on the brakes. If we didn't do this there would have been an accident due to our adaptive cruise control failing. This has happend twice to us. Taken the vehicle to the dealership and they cant replicate the problem so there is nothing to fix. This is something that needs to be Investigated before an accident occurs.

    The contact owns a 2024 Cadillac Lyriq. The contact stated while driving approximately 35 MPH, the Forward Collision Avoidance: Automatic Emergency Braking system unintentionally engaged. No warning light was illuminated. The vehicle was taken to a dealer to be diagnosed; however, the dealer stated that the cause of the failure could not be determined. The dealer was unable to duplicate the failure. The vehicle was not repaired. The manufacturer was notified of the failure. The failure mileage was approximately 3,400.

    On or about 11/14/24 Car found completely dead and won't start in garage. Car had about 50% charge in main battery pack and 12V completely dead. Called OnStar service and they had vehicle taken to dealership on flatbed. GM app reports Vehicle Issues: "Electric Drive Unit, Lithium Ion Battery, Electric Drive Unit, and 12V battery require your attention now." Vehicle purchased on or about 8/18/24 and at time of failure car had about 385 miles. Car kept garaged when not in use in pristine condition. Dealer reports as of 11/23/24 they do not know the problem with the car. I have been without use of my only car since 11/14/24.

    I am submitting this follow-up to document new information and continuing concerns regarding my 2024 Cadillac Lyriq EV, which experienced sudden unintended acceleration during a parking maneuver. GM and its claims administrator (ESIS) have now completed their review and issued a denial letter on October 31 2025, stating “no defects were found.” However, the denial included no supporting technical findings, analysis, or data explaining how that conclusion was reached. Despite multiple written requests, GM has not provided the following key event-data modules: – Brake System Control Module (EBCM/iBooster) export (brake pressure, pedal position, brake override flags) – Propulsion/Drive Control Module data (torque request vs actual torque and accelerator input) – Front Camera Module metadata (timestamps and event record IDs) Additionally, several data discrepancies remain unresolved — including mismatched timestamps between the ASCM and FCM reports and camera images that do not correspond to the actual crash scene. On October 29 2025, GM publicly announced Customer Satisfaction Program N252521980, covering certain 2023–24 Cadillac Lyriq and 2024 Chevy Blazer EV vehicles for a Brake System Control Module software update due to possible brake-control performance issues. Because my incident involved loss of braking response, this recall appears directly relevant. I have requested confirmation whether my vehicle is affected and whether this module was analyzed in GM’s investigation, but have not received a reply. I am requesting that NHTSA review GM’s handling of this case to determine whether the engineering review was complete and whether the vehicle’s BSCM or related systems could be implicated in unintended acceleration or brake override failures.     While driving northbound on [XXX], south of Oklahoma City, at normal highway speed, at night, in the rain (on [XXX]), this EV lost all power to the wheels and the regenerative braking ceased to function. It appears that all flow of electrons from the batteries to the drive motors, and from the regenerative brakes to the batteries stopped. I still had dashboard lights, steering, and conventional brakes. I was able to safely coast to a stop on the highway shoulder. I set the parking brakes, put the “transmission” into “park”, and turned off the power button. I then restarted the car, and all was normal again. I continued my drive from Austin, TX, to Evergreen, CO with no additional incidents, arriving on [XXX]. This is reminiscent of the “blue screen of death” in old Microsoft desktop computers — fixed the problem by cycling the ON / OFF switch. Unfortunately, doing this when driving at 75 mph is less than ideal, and is a real potential safety problem.     While driving through Wisconsin on [XXX] with Super Cruise activated in 70MPH traffic the car provided me with driver's side collision warning failure notice and then a forward collision warning failure notice, but the car continued to be operating normally. A short time later a brakes overheating warning came on while the car still seemed to be operating normally and maintaining speed. I immediately disengaged the cruise control to check out the situation and the car instantly locked on the brakes, and we could see smoke and smell the burning brakes. I pulled to car to the side of the road, and we exited the car concerned there might be a fire coming from one of the 4 smoking wheels. The rotors were red hot. We remained a safe distance from the car until the brakes stopped smoking. After it appeared safe, I reentered the car and determined the brakes (parking brakes) were fully engaged and the car would not move. We contacted OnStar for assistance and waited. After a significant time (it seemed like quite a while) I suddenly heard the sound of the parking brakes electrically releasing. Shortly after that I started and tested the car again and it would move so we slowly drove it off the busy interstate to the next exit a short distance away and continued working with OnStar to obtain a tow to a Cadillac dealership. After working with OnStar for several hours and determining that there would most likely be no tow truck coming, we decided to test drive the car around the area now that the brakes had cooled off. We ended up renting a 'safety' car that my wife would drive, and I would try to drive the car back to Minnesota to our dealer. The car operated normally for the drive home, but we did not use any cruise control just in case. It was my belief that a system communication issue told the brakes to engage and that since the Super Cruise was engage the motors just added more power to over-come the drag on the brakes until they over heated.     I am writing to report an issue with the windshield durability on my 2024 Cadillac LYRIQ. Over the past three weeks, I’ve experienced three separate instances where small rocks struck the windshield while I was driving, each impact resulting in a crack or chip. In my experience with previous vehicles, similar impacts would rarely, if ever, result in such immediate damage, which raises concerns about the material or design of this particular windshield. This frequent damage has quickly become costly and frustrating. Windshields should withstand small impacts to a reasonable extent, and this does not seem to be the case here. I am also reaching out to Cadillac to address my concerns, but I wanted to report this issue to NHTSA in case other drivers are experiencing similar problems. Please let me know if there is further information I can provide. Thank you for looking into this matter.

    After dark, the rear camera is inactive. Also after dark, sometimes the backup lights do not come on in reverse. According to [XXX] , this is due to a defect with the rear electrical harness because an active license plate light circuit interferes with the camera and backup lights circuits.     Ref: NHTSA ID# 24V589/ GM Recall N242453471 Record shows that the above recall was completed on August 27, 2024. Yesterday while I was reversing my 2024 LYRIQ out of my garage I experienced multiple false automatic braking. Braking was so frequent that, I thought, driving that car at that time was unsafe and pulled it back into the garage and took my other car. It was raining at that time. False braking has happened in the past but not to a point where driving is nearly impossible.

    On September 9, 2025, while slowly turning into a parking space at approximately 7 mph, the vehicle suddenly experienced unintended acceleration. There were no warnings, alerts, or Safety Alert Seat vibrations prior to the surge. The vehicle rapidly accelerated forward, struck a parked car, went over a curb, hit a tree, and finally came to rest after colliding with the side of a building. GM has since taken possession of the vehicle for forensic analysis.

    Each time it rains and the car is in reverse, the emergency braking system kicks in repeatedly, and a red light flashes on the screen. The car believes there is an object or person behind the car; I would imagine because the rear camera can see the rain. It is difficult to even move the car at all, and is a major safety concern. The last time this occurred, I was backing out of my driveway at a slow speed. This also occurred backing out of a parking spot at a slow speed. I hesitate to drive this car in rain because of the issue. In speaking with the dealership's repair personnel, this occurs on every Lyriq. In fact, there was another Lyriq in for that complaint at the same time. I cannot attach photos or a video, as doing so would be unsafe. However, it can be easily re-created in any Lyriq.

    What component or system failed or malfunctioned, and is it available for inspection upon request? Multiple components of our driver assistance software have failed, beyond what we can check in the fields above. Most recently, we have experienced two major failures. The first failure occurred when the car triggered the emergency parking brake on its own while we were driving in traffic at 30 mph. We narrowly avoided being rear ended. This is the incident I've detailed in the fields below. The second failure occurred when the adaptive cruise control failed to brake as we approached gridlock traffic. We again narrowly avoided collision on multiple fronts. We were going approximately 45 mph with roughly 2000 miles on the car. We have the service records, but the selling dealer and Cadillac engineering claim these issues are now fixed. How was your safety or the safety of others put at risk? We were well within less than a car length of collision in both of the situations described above. This car is a large SUV, it would presumably cause significant damage to cars it hit. Has the problem been reproduced or confirmed by a dealer or independent service center? Yes. They verified failures and out-of-date software in our vehicle in this last service visit. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? This vehicle has been directly inspected by Cadillac engineering through our selling dealer. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? The warning lights we experienced included the warnings that the system had engaged and "service driver assist system." There was no warning when the adaptive cruise control failed.

    The headlamps in the Cadillac Lyriq are supposed to be self leveling, maintaining a consistent throw throughout a drive. My left headlamp droops significantly after 10-15 minutes of driving, failing to throw its beam more than 15 feet ahead. This poses a a safety risk because it decreases my ability to see pedestrians or road hazards. This has not been reviewed by a Cadillac dealer yet because it is a known issue to which GM had no current solution. There are no warning signals that appears in the car’s screen. The two pictures below show the headlamps at start up and after 15 minutes of driving. The droop in the left lamp is noticeable. The pictures were taken from the same spot and at the same angle.

    on Sept 23, 2025, I was rear ended by a driver going between 50-60 mph. I am concerned that some safety features were potentially not working at the time of the incident. I will detail the information below: Background / Original Service Issue: When I initially brought my vehicle to Titus-Will Chevrolet GMC Cadillac for service, I observed that multiple critical safety systems were nonfunctional, including, but not limited to: Forward collision alert sensor All cameras (rear, front, top down) Blind zone steering assist Lane departure warning and other associated warning sensors The dealership kept my car for two days and then informed me that a "module" needed to be ordered for repair, but stated that the vehicle was considered “drivable” and would call me when the part came in. I noticed some of the safety features had resumed working, however some were still non-functional. At no point was I told there were any potential safety risks, nor which safety systems were still not functional. I was not given any paperwork, as they were holding it until the module came in. Crash Event: On 09/23/2025 at 5:37 pm, I was involved in a crash. At the time of impact: The forward collision alert sensor did not function I received no warning of the impending collision No airbags deployed. This may have been directly related to the previously identified and unrepaired safety system issues.

    Driver assist forward continues to have issues after being at dealer for issue for 4 weeks for repair After getting car back from dealer for the first three days the error did not come up. Now the error comes and goes daily. Issue happens when approaching stop lights stop signs where there is nothing in front of you starts vibrating seat and slams on breaks. Same stop sign as yesterday but today it vibrates warnings Makes not sense.

    Headlights start dropping as car is driven. Every 10 to 15 miles must pull car over and restart as you can not see further than a car’s length in front of you. Cycle car off and then back on the headlights return to normal for another 10 miles. Also seems that some of the individual lamps start pointing upward blinding oncoming drives even with most of the lamps are pointing down in front of the less than a cars length. Seems the headlights are making adjustments be are not correct until you do a power cycle.

    At least 10 times over the past three months the vehicle’s feature of one pedal driving disengages without a reason or code. When the this happens the vehicle accelerates rapidly. This has happened on local streets and on the highway and often results in a near miss collision. I am concerned this will result in a serious injury or death.

    Headlights on vehicle “droop” after driving 20 minutes or more. They start off okay and then become non existent l, pointing directly down toward the pavement in front of the vehicle and you can’t see more than 1 car length in front of you. You must turn the vehicle off and then back on to reset the headlights to drive. This is a huge safety issue for driving at night.

    Continuing problem with both front and rear emergency braking. Primarily when it is raining and vehicle is put in reverse violent and unexpected emergency braking will occur for no apparent reason. This will happen several times. Also, when backing out of driveway on a slant, rear emergency braking seems to sense the street and brakes are applied violently. This situation has also happened going into driveway on a slant for front emergency braking. Have informed the dealer several times of this problem and they say that Cadillac has not fix. Very distressing and dangerous for whip lash of adults and children.

    I was driving on Saturday September 3 at 5 pm over the Chain Bridge entering Washington DC. Traffic was heavy and the auto brake went on when I was about 15 feet from the car in front of me. I was using one-pedal driving and when I pushed on the gas slowly we started slowly moving forward. I was traveling around 3 or 4 miles an hour and the car would not slow down when I took my foot over the pedal. One pedal driving apparently stopped working. I then applied the brakes gently and the brakes did not work. I then pushed the brakes hard and still no brakes worked. I tried pumping the brakes and nothing worked. The car gently ran into the car in front of me causing no damage to the car in front and he kept on his way. My front grill was cracked and bumper was slightly damaged. The signal on the dashboard said parking brake engaged which it was not. While the auto safety brake engaged minutes earlier when it was not needed it did not work when the car was drifting uncontrollably into the car in front of me.

    I have had two incidents with the automatic braking system occur in a short period of time. The Lyriq is not a car that I drive very often so when the incidents occurred in the last 2 of the 3 times of driving the car it makes me scared to drive the car again. The first incident occurred while driving at approximately 35 mph. The road I was on had 4 lanes with a turn lane in the middle dividing the 4 lanes with 2 lanes on each side of the turn lane. The road carries moderately heavy traffic as it has many businesses (fast food restaurants, strip malls, etc.) I was driving in the left lane when a truck in the on coming traffic entered the middle turn lane and a vehicle ahead of me entered the lane to the right of me. Neither vehicle crossed the line of the lane I was in. This is normal traffic for the type of road I was on. I know there have been reports of rain interfering with sensors in the past, but this day was clear. My vehicle flashed a red collision warning and engaged the automatic braking system. This was scary as heck. It was very jarring to me and my passenger. Next incident I was parking along a curb. So this time I was driving under 10 mph. I was backing up and there was a hand rail along on a walkway on the curb. There was no way I would hit the railing as it was several feet from the roadway. There was no warning in my car when it engaged the automatic braking system. Even though I wasn’t traveling very fast it was very jarring and my neck was still hurting from the last incident when it engaged the automatic braking. When the car engages the automatic braking system it sounds as though it has hit something or someone has hit you. It’s very confusing. I reported previous incidents to my dealer and they said that the sensors were sensitive and they supposedly did some sort of fix, but frankly I’m concerned what could happen as these incidents have quite seriously been a pain in the neck for me.

    I purchased a brand new Cadillac Lyriq on July 1, 2024, and have encountered several severe issues that compromise the safety and reliability of the vehicle: 1.Display Failure: The display frequently flickers, flashes, and sometimes completely blacks out, causing the loss of critical information such as speed, backup camera, navigation, and other vital systems. This makes the vehicle unsafe to drive, as it is impossible to monitor speed and other essential functions necessary for safe operation. 2.Battery Overheating During DC Fast Charging: The vehicle’s battery consistently overheats during DC fast charging sessions, causing the charge rate to drop significantly (from ~190 kW down to 14-20 kW). This overheating issue not only drastically extends charging times but also raises serious safety concerns, particularly during long road trips in hot weather. The vehicle has repeatedly experienced these issues, making it unreliable for long-distance travel. 3.Charger Overheating and Instability During Home AC Charging: The home AC charger provided with the vehicle becomes excessively hot while charging the Lyriq. Additionally, the charging rate fluctuates erratically between 0-3 kW, further complicating the charging process. This overheating of the charger at home raises concerns about the safety of parking or charging the vehicle in my garage, as it may pose a fire risk. These issues have been reported to my dealership, who have attempted repairs without success. The problems persist, and the vehicle remains unsafe and unreliable for my family’s needs. I am deeply concerned about the potential for these issues to lead to more severe safety incidents if not addressed promptly.

    The contact owns a 2024 Cadillac Lyriq. The contact stated while driving at approximately 3 MPH as she was entering her garage to park the vehicle, she depressed the brake pedal, however, the vehicle continued forward and crashed into a tool chest located at the back of the garage wall. The contact stated that the front of the grill and the front bumper were damaged and had to be replaced. The contact stated no warning light was illuminated. The contact stated that failure reoccurred when she was parking the vehicle in a parking lot. The contact had taken the vehicle to a local dealer, where it was diagnosed and determined that the ABS system had failed and caused a loss of braking ability. The contact was advised by the dealer that the failure was related to NHTSA Campaign Number: 24V589000 (Service Brakes, Hydraulic). The contact had the recall completed at that time. The manufacturer was informed of the failure. The failure mileage was 3,000.

    The vehicle is a Cadillac Lyriq. Following an over the air software update pushed by the manufacturer, the vehicle began displaying a high voltage system error message and lost the ability to DC fast charge. The high voltage components themselves are reported to be functional, the issue appears to be caused entirely by the software update rather than a hardware failure. The vehicle has been at an authorized dealership service center since August 14, 2026, awaiting a resolution. The dealership has confirmed the fault is present and is consistent with a known issue caused by the manufacturer's software update, but has not been able to repair it independently, since the fix requires a corrected software release from the manufacturer. As of this writing, the manufacturer has not issued the corrected update and has not provided a timeline for when one will be available. This creates a safety risk because the vehicle cannot be relied upon for longer trips or situations requiring DC fast charging, which can leave the driver stranded or unable to reach a destination or charging option within a safe range, particularly in hot weather conditions common in this region. The warning message and loss of fast charging capability appeared immediately after the software update was applied, with no prior symptoms. It is unknown whether the manufacturer has independently inspected this specific vehicle beyond the dealership's diagnosis. The high voltage system and charging hardware remain available for inspection upon request. Multiple other owners of this same model have reported the identical issue following the same software update, suggesting this is a widespread defect rather than an isolated incident.

    On August 9, 2026, at approximately 3:40 a.m., while driving, the vehicle suddenly lost all propulsion and became completely disabled on the roadway. The vehicle remained powered on but would not move forward or backward. At approximately the time of the failure, Cadillac/OnStar Advanced Diagnostics generated a critical Electric Drive Unit warning instructing immediate service and reported diagnostic code P0A78. Additional vehicle diagnostic warnings involved the electric drive unit/high-voltage propulsion system. I immediately contacted roadside assistance but was unable to obtain a workable towing solution for several hours. The vehicle ultimately had to be privately towed and has remained undriveable since the incident. The loss of propulsion occurred unexpectedly while the vehicle was being driven, without advance warning that would have allowed me to safely plan for the vehicle to become disabled. I am concerned that the same sudden loss of propulsion could occur at higher speed, in traffic, at an intersection, or in another location where an unexpected loss of motive power could create a serious crash risk. The vehicle requires diagnosis and repair for the Electric Drive Unit/P0A78 condition.

    Cruising at 65mph in moderate mixed traffic when vehicle began maximum braking with no action from me and no forward obstacles. Several following vehicles had to take drastic evasive action to avoid collision. Very lucky this resulted in no injury or damage. Could have been much, much worse.

    Right rear taillight and brake light fail to light intermittently. This is a known defect on many other Lyriqs. When this failure occurs, the vehicle is not compliant with state laws regarding having working taillights and brake lights, and could result in a moving violation ticket. Cadillac should be required to recall and repair/replace these lights.

    When in reverse abrupt automatic braking occurs. Safety hazard with abrupt stop around other vehicles, hard braking causes driver and passenger to pitch forward. No warning. Automatic Braking disabled, sensors checked, settings adjusted by dealer. Dealer says there is nothing they can do. General Motors contacted but no action taken to date.

    While driving in one-pedal driving mode at the high setting, an error message appeared and one-pedal driving suddenly shut off and switched to a more standard drive mode where engine braking is disengaged. This happened while drive was engaged and stopped at a red light. The car briefly lurched forward and the car engaged an emergency brake to prevent the car from rolling into the vehicle in front of me. This incident was unpredictable and resulted in an unexpected change to the drive mode I had selected which could have lead to a collision.

    Vehicle engage emergency stop in forward direction for no apparent reason. Leaving dead stop 2nd car at traffic signal. Huge problem as may cause rear end collision. I have disengaged all auto brake features. Had previously already disabled reverse auto braking feature. I was at a residential intersection. Single lane each direction with left turn lane. I was the 2nd vehicle in line. There was a crosswalk however there were no pedestrians, bicycles, etc. in the crosswalk or on any sidewalks of roadways. The left turn green arrow illuminated and the vehicle in front began turning. I began to follow and reached no more than 5-10 mph when "bang", the emergency stop engaged. I actually thought I hit a pole or object. Terrifying for my wife in the passenger seat also. The vehicle behind us was able to stop in time, this time. However, drivers do not expect the vehicle in front of them to begin moving the emergency brake for no reason. This is a dangerous defect which must be addressed.

The vehicle was taken to the dealer, and it was suggested to deactivate the Back Over Prevention and Cross Traffic Alerts while attempting to reverse. The vehicle had not been diagnosed or repaired. The manufacturer was made aware of the failure by the dealer. The failure mileage was 145.

    Driving my car on the highway at speeds of about 65, and the charging port door suddenly opens. When I slow down then the door closes on it own. Seems like the housing for the door isnt made tight enough to stay closed during highway driving.

    We encountered consistent loss of or erratic headlight operation. The behavior was that lighting would simply fail on one or both sides of the car, often along two-lane roads with curves in the road where it is critical to judge distance to oncoming traffic. Other times the lights would dim and wash out the ability to clearly see the road ahead. The dealer has replaced the headlights three times already. An associated lighting module was also replaced more than once. Note that this issue is still present so we are unable to use the vehicle now after dark.
