I slowly turned right into a parking space, braked to stop but the car didn't stop and instead lurched, jumping forward over the parking curb and hitting a small wall. My foot was on the brake pedal the entire time. It happened at 9:00am on a clear day. I was alone in the car and uninjured. Damage to the car was extensive and estimated at nearly $40,000. As a result, car was totaled by the insurance company.

I was pulling slowly into my driveway and suddenly the car began accelerating on it's own. I braked and it would not stop. I ended up running into my electronic gate and hitting the side of my neighbor's house. Safety was put at risk because I could have hit a person or moving car had this not happened in my driveway. I could have caused more substantial damage to my neighbor's house. I will report this to the dealership and have it inspected. There were no warning lamps or messages or warning symptoms.

Vehicle: 2023 Toyota RAV4 Prime Plug-In Hybrid Component/System: Digital instrument cluster / dashboard display Description of the problem: The digital instrument cluster intermittently and unpredictably goes completely blank while the vehicle is being driven. When this occurs, all dashboard information is lost, including vehicle speed, fuel and battery level, warning indicators, cruise control status, and tire pressure information. The screen remains entirely black and provides no information to the driver. Safety risk: This failure makes the vehicle unsafe to operate. The driver cannot see speed or critical warning information, and multiple safety features become functionally disabled due to the lack of display feedback. The screen has gone black while driving, which significantly increases the risk of an accident and poses a serious safety hazard to the driver, passengers, and others on the road. Timeline and progression: The issue first occurred in October 2024. Initially, it happened infrequently (about once per month) but has progressively worsened and now occurs nearly every other drive. Restarting the vehicle does not reliably restore the display. Cold weather may be a contributing factor, but the failure also occurs under normal conditions. Reproduction and inspection: The problem has occurred repeatedly and with multiple drivers. The vehicle has been taken to a Toyota dealership twice for this issue. No repair was performed, and the concern was dismissed. The vehicle is available for inspection upon request. Warnings or alerts: There are no warning lights, messages, or alerts prior to the failure. The display turns completely black without warning.

The digital instrument panel in front of the driver is blank and will not turn on. This happens intermittently at start up for up to an hour after starting the car. This causes the driver to not see alerts/warnings, current speed, tire pressure, gas gauge levels, etc. When contacting two Toyota dealerships and Toyota corporate, I was told that there is no remedy and that I essentially cannot do anything until they find a remedy. Toyota has left me with an undrivable car with no solution to the problem.

Distracting connected services spam. When the car is turned on, the center console displays a "safe driver" warning, then after a number of seconds, a message instructing driver to sign up for connected services for navigation appears and won't display a default screen or last screen displayed until driver selects another display option. I was never on the navigation menu, nor do intend to ever use the navigation menu, and yet the solicitation appears. This is not only annoying, but distracts the driver from other information that the driver intends to see on the center console. If Toyota motor sales wants to not allow navigation except for subscriptions, it should be limited to when a driver presses the navigation button, but not distract the driver by spamming when the center console should be displaying whatever the driver has selected and forcing the driver to "cancel" by finding the button on the center console to what was originally displayed when the car was turned off or a default screen of user choice.
- 10/28/2024· UNKNOWN OR OTHER
A Toyota advertisement for an optional subscription service, called Drive connect, takes over the display randomly, including when I am using the display for directions with GPS. There have been many instances where I have been using the display for directions, when this banner takes over the screen, and I have to abruptly reach over and turn it off so that I can continue following the directions. I have missed turnoffs because of this. And I do not think that it is safe to have to reach over suddenly to switch the display back to GPS again. I don't see any reason why the driver should not have a choice when it comes to the displays default behavior, especially when following directions on maps with GPS

Driver was parking the car in a parking lot at a very slow speed. The car suddenly shot forward at a very high rate of speed, requiring evasive steering from trees and a fence, until the car crashed head-on into another vehicle. The driver is very certain the brakes only were applied during the entire time the car shot forward. Airbags were not deployed. Bystanders were within six feet of the car at one point during the acceleration, prior to the crash. There were no injuries. Earlier, on the day of the crash, the car had a sudden, strong, yet brief acceleration of the vehicle during normal braking while approaching a rotary, unlike normal ABS. ABS itself seemed unneeded (the approach to the rotary was well within the speed limit, on dry condition roads). This issue occurred twice in the two months prior to the accident. There were no known warning messages in the car or the phone app for the car in any of these cases or at the time of the crash. The manufacturer has been notified of the crash, a product liability case has been opened, and an engineer was sent to look at the car. A data download from the airbag event data recorder (Bosch CDR) was done, a report was generated, and the report was provided to the car owner. No data in the formatted report is provided regarding Vehicle Speed, Accelerator Pedal, Cruise Control Status, Brake Torque, etc. Unformatted hexadecimal data is included in the report, without a data schema to decipher the data. It is not known if any other analysis or information about the car was reported by the engineer, beyond the Bosch CDR report. The owner is not aware that the problem has been reproduced or confirmed by anyone involved, or of any other inspections than the one described here. Manufacturer has not yet provided the owner with a report response. The car is currently at an auto repair shop and no repairs have begun. The shop is awaiting an appraisal by the insurance company before doing the repairs.

The contact inspected the vehicle and became aware that the electrical wires underneath the driver's side front seat were exposed, and the cover was in the floorboard. Additionally, the dashboard panel on the front passenger's side had detached and was hanging down. The failures occurred two weeks after the recall repair under NHTSA Campaign Number: 25V595000 (Electrical System) was performed. The dealer was made aware of the failure but provided no assistance. The vehicle was not diagnosed or repaired by an independent mechanic or dealer. The manufacturer was made aware of the failure, and a case was filed. The failure mileage was approximately 34,000.

I am reporting a systemic pattern of safety reporting negligence, property damage, and data manipulation by Toyota of Woodbridge. On March 26, 2026, the dealership performed a backup camera recall (25TA13) but completely failed to address or notify me of a second open safety recall regarding the instrument cluster (Recall 25TA08). Consequently, in early June 2026, the unaddressed defect manifested: the digital dashboard screen went completely black while operating, creating an immediate highway hazard. The vehicle was returned to the dealer to address this screen failure. Sometime between June 10 and June 17, 2026, while the vehicle was in the shop's care, the passenger seat trim panel was physically broken and sheared off by technicians. On the June 17, 2026, invoice, the dealer officially logged Job #3 as a completed Multi-Point Inspection (MPI). However, during a subsequent visit on July 2, 2026, the Customer Service Advisor openly admitted that the June 17th inspection was never actually performed, proving they falsified the safety record, and could only display the outdated March 26th data on his screen. This pattern culminated during the July 2nd visit, which was for a diagnostic check regarding vehicle sounds. The dealership used this visit to aggressively demand an immediate $1,100 tire-and-alignment package based on hidden telemetry. When I demanded the physical MPI report to verify their claims, my requests were ignored and the telemetry and MPI reports were only provided after the new tires were installed. When I saw that the telemetry reports and MPI did not justify the damage they verbally claimed, I asked for and was given a full refund and my old tires were put back on the vehicle. Upon taking delivery, the vehicle's Tire Pressure Monitoring System (TPMS) was flashing a dashboard safety fault. Ourisman Toyota Woodbridge ignored a Toyota Corporate mandate to contact me by close of business July 7, 2026. The vehicle is available for inspection.

The digital dashboard (instrument cluster)on my 2023 Toyota RAV4 Prime, suddenly went completely black. This included the speedometer, warning lights, fuel gauge, and all other driver information. I was unable to monitor my speed, vehicle status, or receive safety alerts — a serious risk while operating the vehicle. I have not authorized repairs yet. I brought the issue to Toyota’s attention, but since the vehicle is out of warranty and over the mileage limit, the dealership is requiring a $200 diagnostic fee just to evaluate the problem. There has been no formal decision from Toyota yet on whether they will cover the cost. After researching online, I discovered that numerous other owners of 2023 RAV4 Prime vehicles are experiencing this same dashboard blackout issue, suggesting a broader defect. This is not a result of wear and tear, and it directly impacts safe operation of the vehicle. I am submitting this complaint because this is a safety-critical system failure that should not occur in a new vehicle. If this is a common defect, it warrants investigation and potentially a recall. Toyota should not place the burden of diagnosis or repair on consumers for what appears to be a growing, safety-related manufacturing issue.

On the night of [XXX] at approximately [XXX] PM, I was driving from Tracy, California to Lancaster, California via the [XXX]. The freeway had no traffic at the time, and I set my cruise control to maintain a speed of 75 mph for a safer ride. While driving in the left lane near Santa Nella, my car suddenly deployed the ABS brakes. The steering wheel locked, and the car began to slip—its tires locked as if on a roller coaster going downhill. The driver’s side then rolled over and crashed. Fortunately, the road was empty, and I did not hit anyone during the accident. I thought I was going to die, but I am lucky to be alive. suddenly Abs brakes deployed steering wheel lock right side this reason I have very accident. I believe the car has manufacturer defects. First, the door handle on the driver’s side is positioned too close to the driver’s left arm. Second, the center console on the right side—intended for resting the driver’s arm or storing extra items—repeatedly hit my ribs during the accident. During the crash, the driver-side door handle struck my left arm multiple times. Orthopedic surgeons reviewed my X-ray and found that my forearm was fractured in three places. They recommended surgery. The center console also struck my right ribs repeatedly, causing swelling and pain. Fortunately, my ribs were not broken. My chest & ribs swelling and pain right side shoulder I have pain my head & neck pain My car was declared a total loss. I respectfully request that these issues be investigated, as I believe another driver could sustain similar injuries in a future accident involving this vehicle model. On [XXX], I was pulling into a parking spot in a parking lot [XXX] in San Francisco. When I engaged the brakes, the vehicle did not stop and continued moving forward. The vehicle accelerated a bit and climbed over the parking lot barrier, onto the pedestrian sidewalk, and into the street despite continued brake depression. The right side of the vehicle made contact with the parking meter on the sidewalk for street parking. The brakes were depressed and engaged the entire duration of the incident, and the vehicle did not stop until impact with the parking meter. Standard safety features were enabled (ie. pre-collision system, road sign assist). Dynamic radar cruise control is disabled. I reported this incident to the Toyota dealership, whom sent a vehicle safety inspector, and I emphasized my concern for the safety of the vehicle. Per Toyota’s investigation report, Toyota concluded the vehicle was mechanically functioning properly at the time of the incident and the vehicle is overall safe to drive. In regards to recal 25TA08/25TB08, Toyota has yet to release a fix for the very critical digital instrument cluster going blank forever. I got a notice in the mail that they had a software update for me but when I was able to schedule an appointment with the dealer, the dealer advised that within 10 days of Toyota releasing the “fix” they rescinded/repealed the “fix”. So by the time I got the letter giving me hope that my car would live, I realistically had 2 days to get it in to get updated with a fix that probably wouldn’t have worked. With such a critical part/software issue, I feel Toyota is massively dragging their feet and I am driving a ticking time bomb and possibly having this car become unsafe to drive and potentially useless all together.

My 2023 RAV4 Prime is impacted by the 12.9" display screen issue currently under recall. After experiencing two incidents of my screen going blank, my dealership advised that my car was not safe to drive and I have been driving a dealer rental car for over 10 weeks now. This is not a satisfactory solution for me for multiple reasons: 1) I no longer have access to the features of my vehicle that led me to purchase this particular vehicle, namely it's size and being a plug-in hybrid (i.e., reduced fuel costs); 2) my partner is not allowed to drive the rental vehicle which limits its usefulness for trips together unless I want to do all the driving; 3) I have been advised by the dealership I am not allowed to put my pets in the vehicle, which further limits its usefulness since I have to seek alternative transportation to take them for veterinary care, etc. Further, I am forced to return to the dealership every 30 days to renew my rental agreement. I have been given no information as to when the remedy will actually be available for my specific vehicle.

The instrument cluster (combination meter) failed completely and without warning, going entirely blank. This eliminated all speedometer, warning light, and indicator functionality. The failure began occurring intermittently before becoming persistent. No warning lamps or messages preceded the failure — the cluster simply went dark. The loss of speedometer functionality made the vehicle unsafe to operate, as the driver had no means of monitoring vehicle speed. The loss of all warning indicators eliminated visibility into any safety-critical vehicle alerts that may have been active during the failure periods. The problem was confirmed by the dealership and is the subject of Safety Recall 25TA08 (Combination Meter May Go Blank). The vehicle was brought to a Toyota dealership where the recall was documented and the repair was eventually completed. The replacement part required VIN-specific production and programming by Toyota, resulting in an extended out-of-service period of nearly two months at the dealership, and approximately five months from the first occurrence of the failure. During the out-of-service period, the vehicle could not pass state inspection and was legally inoperable. The component has been replaced and is no longer available for inspection.

On December 29, 2025, the 12.3-inch digital instrument cluster on my 2023 Toyota RAV4 Prime went completely blank upon startup. This failure immediately eliminated my ability to see the speedometer, fuel and battery levels, tire pressure warnings, and all critical brake and hybrid system malfunction indicators, rendering the vehicle completely unsafe and illegal to operate on public roads. I immediately brought the car to Wondries Toyota in [XXX] , where it has remained ever since. The dealership confirmed the failure is related to Toyota Safety Recall 25V595 (Toyota internal code 25TA08). The service department advised me that the vehicle is unsafe to drive, but they are unable to repair it because the required replacement combination meter is on national backorder with no estimated time of arrival. As of late March 2026, my vehicle has been sitting out of service at the dealership for approximately 90 days. Because Toyota has failed to provide a timely remedy for this severe safety defect, my vehicle has been abandoned on the dealer lot for a quarter of a year. Furthermore, this prolonged, unmaintained inactivity introduces secondary safety and vehicle integrity hazards. The plug-in hybrid high-voltage battery has been sitting idle and degrading for three months, and the fuel in the system is destabilizing. I am filing this complaint because Toyota's failure to supply replacement parts for a massive safety recall within a reasonable timeframe has permanently compromised the safety, use, and value of my vehicle, and I am currently proceeding with a Lemon Law repurchase that vastly exceeds the 30-day out-of-service threshold in [XXX] . Lastly, we got this RAV4 Prime new and have had all the services done at the authorized dealership since day one. The primary instrument display started to go out when it was cold, below 50'F. This could be mitigated by running the engine or running the defrost for a period of time to warm up the area around the display. When it was too cold this winter, that was no longer effective and the display would not come on at all. The car would drive and all of the instruments displayed on the screen would work (cruise control, etc.), but there was no way to know how fast I was going and if there were any warning lights illuminated. Running the car long enough without the display until the environment warmed, the display would come on, showing the animated start up sequence, then showing whatever speed and other car conditions were current. It took three tries for the dealership to finally see this issue, because they would put the car in the warm garage before working on it, and the display would come on. The dealership now has the car, and is seeking a replacement display, saying it is an electronic issue.

There is an active recall on this car with no available remedy. Upon starting the car, the instrument panel/instrument cluster screen remains completely black and doesn't turn on. This means there is no way to see: (a) speedometer, (b) odometer, (c) tire pressure, (d) available fuel range, (e) available EV range, (f) check engine lights or other emergency alerts, (g) lane assist, (h) drive menu settings, (i) temperature outside, (j) what gear the car is in, (k) whether the car is in EV mode or HV mode, (l) average fuel economy, (m) whether the parking break is engaged, (n) whether all seatbelts are fastened, (o) whether any doors or trunk is open, (p) battery status, and (q) oil status. To be clear, Toyota is well aware of this issue and has sent out the recall notice. Upon taking my car to the dealership, they confirmed that this is the active recall issue and there is no available remedy. If the instrument panel has not yet gone blank, there is a software update that can be given; however, if the instrument panel HAS ALREADY gone blank, there is ABSOLUTELY NOTHING THAT CAN BE DONE TO REMEDY THIS. The dealership would not release my car to me as it is not drivable. The recall has been active since SEPTEMBER 11, 2025 and there is no remedy available FIVE months later.

I'm experiencing an intermittent malfunction of the combination meter (speedometer), as described in NHTSA Recall 25V595. I contacted my local dealer, who confirmed no resolution was available at this time. I contacted Toyota customer care who also confirmed no resolution at this time. They indicated my only option was to seek a Toyota loaner vehicle and then request reimbursement for that expense after returning the car. I received this recall notice from Toyota in November 2025. They indicated on this notice that they estimate the remedy may be available by December 2025. It is now February of 2026 with no remedy identified. As indicated in this recall notice, driving my vehicle without a functioning instrument panel is unsafe - "increased risk of crash or injury" Per the last paragraph in the notice, I am filing my complaint with NHTSA because I feel they have failed to provide a remedy to this defect within a reasonable time, as this is a crucial safety related system, and I cannot safely operate this vehicle without a functioning instrument panel.
