Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2020-2021 Avalon, Avalon Hybrid, Corolla, Highlander, Highlander Hybrid, RAV4, RAV4 Hybrid, Lexus ES350, Lexus RX350, Lexus RX450H, 2021 Sienna Hybrid, Lexus ES250, 2020-2022 Camry, Camry Hybrid, and ES300H vehicles. A short circuit may develop in the Occupant Classification System (OCS) sensor, preventing the front passenger air bag from deploying.
Risk: An air bag that does not deploy during a crash increases the risk of injury.
Fix: Dealers will inspect and replace the sensor as necessary, free of charge. Interim letters notifying owners of the safety risk were mailed in February 2024. Some final remedy owner letters were mailed October 31, 2024. Letters will continue to be mailed in phases through January 2026. Owners may contact Toyota customer service at 1-800-331-4331. Toyota's number for this recall is 23TB15/23TA15 Lexus 23LB03/23LA03.

Our car in used for a TURO rental car. The TURO system has unlisted our income making vehicle until this is fixed. It's been over a year and we are losing money every day. We are considering legal action. Hope this submission helps. Thank for your help

I purchased this vehicle new in June 2021. It is the hatchback model. Prior to that I drove a motorcycle since 2015. I noticed right at the beginning that the headlights don't illuminate far enough ahead at night for what I consider safe. My ability to see off into the distance ahead of the vehicle is far less than what I would get from my motorcycle, which doesn't make any sense to me because this car has LED headlights which are supposed to be better. I live in Washington, DC and have family in New Jersey that I visit regularly. There are two ways I can go. The first is Interstate 95 which has a speed limit of 65 but I set my cruise control at 60. When I come to an area where there is no street lighting and there are no other cars around which are helping illuminate the roadway, I am overdriving my headlights. This is not safe. The second route is taking Route 50 to the Annapolis Bay Bridge, then getting on Route 301 North towards Middletown, Delaware and then New Castle, Delaware, over the Delaware Memorial Bridge and then Route 295 towards Burlington County where my family lives. Route 50 is 65 mph until you get to Annapolis where its reduced to 55, then stays 55mph all the way to Route 301, and then Route 301 is 55 mph, until the Delaware Memorial Bridge. When I get to Route 301 North there is very little other lighting on the road other than my headlights. There are far fewer cars than on Interstate 95 so I get very little illumination from passing cars. Even if I reduce my speed to 50 mph, I am still over-driving my headlights. There has to be something wrong with how the headlights have been positioned. They are not shining for any reasonable distance ahead of the car. I have gone to the Toyota dealer in Bethesda, MD where I purchased it, Jim Coleman Toyota three times to tell them about it. Each time they have told me that there is nothing wrong with the lights. But I don't know what they did to check them.

Car was in dealership twice in one week for jerking while driving dealership said it was abc and was so called “fixed” both times only had my car for a little bit back before I had to take it back . 3rd time in the same week my transmission went out on me while on the highway with young kids in the car and almost caused a wreck. Car got towed in was confirmed it was my transmission and a dozen other codes going off . Toyota is lucky that the semi that was behind me when this happened did not plow into me

On December 20, 2023, a safety recall was officially generated for my 2021 Toyota Corolla. It has been over a year, and there is currently no remedy for this recall. I was originally notified that there would be an option to fix this by the third quarter of 2024. However, that was not the case. On December 26, 2024, I contacted the service center at Sam Leman Toyota in Bloomington, Illinois by phone, and they confirmed that there is still no remedy for repair.

On December 12, 2025, I was in a head on car accident in my 2021 Toyota Corolla nightshade. I was slammed into the steering wheel which caused injury. My car has over $14,000 worth of damage to the front yet my airbags did not deploy. I have taken my car to Toyota several times over the past few years and every time I’ve asked about a remedy for this recall I’ve been told there is no remedy as early as June or July 2025 I had to bring my car in for a manufacturer defect with the engine that Toyota fixed for free. I asked them about the recall on the airbags. They again told me there was no remedy.

I purchased my Toyota corolla SE 2021 in May 2021 with 11 miles on it. I received safety connect trial for one year. It was so unsafe. I believe there was an invasion of privacy because after requesting my data held at Toyota Corp., I read of times of install/deactivation that were not proper in accordance of my purchase and my understanding of the set up at time of purchase. It appears this safety connect was already connected before my purchase, which is a safety issue and is a violation of privacy and lead to data breach. With my connected device, iPhone, acting crazy and miss reading my locations and redirecting, etc., I was on my way to Apple in December 2021 with 2016 miles on my car. I believe the remote connect was being controlled by others as up to five devices are allowed on the module. GPS worked accordingly obviously, because when I exited the interstate, immediately my engine stalled out at 50 miles an hour in the rain. I was scared to death and found the closest gas station to pull over and stop my engine. About 30 minutes later, I continued to apple, 1 mile away. After restarting my car, I was alerted to be safe and contact my provider. I found out that that meant that my control module was blown. This has affected me in a few ways. Maps have read I am in China, South Florida, north, Florida, Georgia, California. I also went to the local Toyota for an oil change and tire rotation in October 2022 and learned that I needed a tire replacement and the pressure was down to 15 pounds. No light appeared. If the communication module is destroyed, then all of the safety features are, as well. No lights alert, no Blindspot warning days, automatic emergency, braking has failed as well, as I recall when it worked and it is not even sensing a vehicle or anything in front of it. Also the red signs that display, stop, certain miles per hour, etc. are reading incorrectly. Furthermore, Toyota wants to disclaim all warranties associated.

I am writing to formally file a complaint regarding the ongoing issues related to safety recall 23TA15 for my 2021 Toyota Corolla LE. The recall, issued in December 2023, involves a defect in the Occupant Classification System (OCS) sensors in the front passenger seat, which may have been improperly manufactured. Unfortunately, Toyota has failed to address this recall in a timely manner, significantly impacting my ability to use my vehicle safely and as intended. My local dealership has confirmed that my car is affected by this recall and that the OCS sensor is not functioning. However, despite the recall being issued nearly a year ago, the dealership has informed me that they are unable to obtain the necessary parts to repair the issue, and they do not expect to receive them until at least the end of December 2024—if not later. As a result, I have been unable to use my car's passenger seat in a safe and fully functional manner, effectively rendering the car unsuitable for my needs. This prolonged delay in resolving a critical safety issue is unacceptable, and I am concerned for my safety and the safety of others until this issue is resolved. I kindly request that the NHTSA investigate Toyota’s handling of this recall and take appropriate action to ensure that affected vehicle owners, including myself, are provided with timely repairs and necessary parts. I am also requesting clarification on any steps I can take to expedite the repair process. Thank you for your attention to this matter. I look forward to your prompt response.

The contact owns a 2021 Toyota Corolla. The contact stated that while the vehicle was parked in the driveway with the engine off and attempting to enter the vehicle to clean the interior of the vehicle, the vehicle rolled away going downward out of the driveway. Due to the failure, the vehicle tugged the contact while rolling away. The contact stated that the vehicle independently came to a stop in the middle of the roadway. The contact sustained back, feet, and injuries to both legs, requiring medical attention. A police report was not filed. The Insurance Provider retrieved the vehicle at the scene. The vehicle was not diagnosed or repaired. The manufacturer was not notified of the failure. The failure mileage was unknown.

The contact owns a 2021 Toyota Corolla. The contact stated that while driving in a parking lot, the clutch pedal failed to function as intended. The clutch pedal was depressed and failed to return after being released. The contact used his foot to raise the clutch pedal. No warning lights were illuminated. The vehicle was taken to the dealer, where it was diagnosed and determined that the slave cylinder had failed. The vehicle remained at the dealer unrepaired. Upon further investigation, the contact related the failure to the six-speed manual transmission. The manufacturer was notified of the failure and assisted with the labor cost. The failure mileage was approximately 58,000.

My car shows that the vehicle needed to be serviced, in checking the Toyota app I saw there was a warning for the adaptive lighting system/electrical concern. The warning light had been on for some time, thinking I was due for regular maintenance like an oil change I had ignored it for a bit. Everytime I would start the car this warning would appear. I too the car in for service, they also did a 20k maintenance service and kept the car overnight overnight as they were unable to locate the issue same day. The following day I was advised they couldn't determine the issue and they cleared the code.

While driving my car I noticed a sloshing noise like water, I thought it was a bottle of wiper fluid until I opened and closed my passenger rear door. The door was full of water, I looked it up online and found tips to loosen a plug on the bottom of the door to let the water drain out. I did this and it was pouring out of the door. I called for service, took the vehicle in, asked about damage to electrical inside the door like speakers and locking systems. The man explained due to the way the door is made IF any water got in it would not affect these systems. They advised the plugs can get full of debris and to loosen the plugs after heavy rain. This has now happened several times, I have to check all my doors and drain them after it rains. Note there are three plug holes, all have plugs so no water can freely drain on it's own.

I am filing this complaint regarding a serious defect in my 2021 Toyota Corolla (Manual). At approximately 42,000 miles, the clutch slave cylinder failed. This defect caused hydraulic fluid to leak and contaminate the clutch assembly, resulting in significant damage and loss of proper function. Despite the fact that this issue stems from a defective part, the dealership has refused to cover the repair of the clutch assembly, stating that the clutch is not covered under warranty. Instead, they are requiring me to pay for the repair out-of-pocket, which would not have been necessary if the damage had not been caused by the defective clutch slave cylinder. I believe this is unfair and unsafe, as the failure of a clutch slave cylinder directly impacts the drivability and safety of the vehicle. This is a significant safety issue. A defective clutch slave cylinder can lead to sudden clutch failure, loss of vehicle control, and increased risk of accidents. This is not a wear-and-tear issue but a manufacturing defect that compromises the safety of the vehicle. I respectfully request that NHTSA investigate this matter to determine whether similar failures are occurring in other Toyota Corolla vehicles of this model year. If this is a widespread defect, I urge NHTSA to take appropriate action, including a recall or requiring Toyota to cover repairs.

While driving the vehicle the brake hold system was activated at a red light. While proceeding through the light within (about) 5 minutes the vehicle indicated a brake hold malfunction. An odor of smoke was also present, at that point I turned off the vehicle and inspected the origin; the trunk. Upon opening the trunk the right side had a sizeable fire growing towards my assets. Police did arrive on scene, an adjuster has examined the damages, however this has not seemed to be replicated. I believe the risks associated with this incident are quite evident. There was no collision.

The safety of any passenger in the front seat is potentially affected as the defect can either cause the front passenger airbag to fail to deploy in an accident or cause it to deploy when it shouldn't, either way causing harm to the passenger in the front. This probem was widely reported in the press in 2023 and 2024. One article I read said that Toyota claimed it would start issuing the recalls for repair in Feb 2024 but that still had not happened by the time I bought my 2021 Corolla in April 2024. I bought it as a used car from a local dealer who shared the info about the recall which was on the CARFAX report too. I am reluctant to have family or friends in the front passenger seat because of the risk of injury in the case of an accident of accidental deployment of the airbag. This reduces the value of my car significantly, as it is required by law to be safe for my passengers as well as myself. Very disappointing behavior from a company that used to be reliable.

At around 0750 AM on 20Nov2023, the sun roof exploded while turning roughly 15 MPH onto an onramp of a highway. Nothing hit the car from the outside. The sun roof glass shattered and fell onto the sun roof cover. Glass could have fallen inside as well as onto me and could have obstructed my vision, causing a crash. No pot holes or noticeable bumps were struck prior to or after the malfunction. Car was at a red stop light for a minute or so prior to the malfunction. The temperature was around 45 degrees Fahrenheit when the malfunction occurred. The heat was not turned on in the car at any point. When speaking with the dealership service center, they said no on else has come in with this issue. When doing a quick google search, up to 5 individuals from 2022 have had the same issue with varying toyota models and years, including 2020 SUVs. The car will be taken into the dealership for inspection tomorrow, 21Nov2023. No warning lights related to the problem appeared.

The problem seems to occur when activating the Full-Speed Range Dynamic Radar Cruise Control (DRCC) feature, as detailed next. During a recent trip, the car behaved in a potentially dangerous, life-threatening manner when the DRCC function was activated. Specifically, the car seems to increase the RPM of the engine to dangerous levels when going down a slope even if there is a speed limit set using the cruise control. In other words, the car does not change gear properly to keep the RPM in normal ranges. The engine’s RPM getting close, entering the red zone indicated by the RPM tachometer, and staying there even for seconds may cause an engine explosion which at highway speeds can be extremely dangerous for the passengers and other nearby vehicles on the road.

I recently purchased a 2021 Toyota Corolla SE. After a few days the "Engine Maintenance Required - See Dealer" notification appeared. See as it was purchased very recently, I assumed this was the oil maintenance light, and reset it. Later that day, it came back, and I reset it a different method under the assumption I did it wrong. The next day it was back, and that is when I realized it must be something else. So I started doing some research and found lots of other people have had this same issue: 2020-21 Corolla, greater than 30k miles, usually less than 60k, and while the Check Engine Light *DOES NOT TURN ON* the car is throwing an error code: P268115 - Engine Coolant Bypass Valve. Sure enough, I have the codes pulled and I have that code showing up. So I took it to the local Toyota Dealership. They stated that the issue was not a warrantable, so I would likely have to pay out of pocket. After a slight fuss, they agreed to fix it as a "one-time goodwill gesture, performed in the interest of customer satisfaction..." So in the end, they did fix it; I did still have to pay for a diagnostic of the issue which is frustrating, especially since I was dead on in my analysis of the issue. To answer the guideline questions: Engine Coolant Bypass Valve Leaking engine coolant could cause the engine to overheat, which can lead to fires, not exactly ideal. It can also cause electrical shorts, leading to more issues. Yes. Lancaster Toyota confirmed the issue. Yes, Lancaster Toyota confirmed that the part was faulty. They appeared shortly after buying the car. There were no error lights like I'd expect, but there was the constantly appearing "Engine Maintenance Required - See Dealer" message which the Owner's Manual states is simply the 5000 mile Oil Change warning. Hidden behind this message is the error code P268115.

While attempting to change a flat tire on a 2021 Toyota Corolla Hatchback, two wheel studs sheared off completely, leaving the wheel unsafe to drive. My wife was stranded alone in the mountains with no cell service. She managed to loosen three lug nuts, but the final two were seized. When a passerby assisted, both seized studs snapped. The spare could barely be mounted. The vehicle has been serviced exclusively by Toyota dealers since new. Lancaster Toyota later replaced the broken studs free of charge, yet simultaneously claimed that another dealer, Frontier Toyota, had “switched the tyres.” so Lancaster are not responsible. That claim is false — Frontier’s invoice (Sept 25 2025) clearly shows only an oil change and inspection, no tire or wheel service of any kind. Replacing the studs at no charge while denying responsibility suggests an attempt to quietly resolve what may be a dealer-induced over-torque or defective-stud failure. The broken studs were not returned to us despite multiple requests — effectively withheld by the dealer, which constitutes unauthorized retention of customer property and prevents independent verification of failure cause. Toyota Corporate (case #251103002581) was notified but refused to investigate, referring the issue back to the same dealer. This complaint is being filed so NHTSA can document a potential wheel-stud safety defect or service-related failure and dealer mishandling of critical evidence that could conceal a wider safety problem.

The navigational systems shuts off in the middle of driving. The screen will go completely black making it unsafe when using navigational directions. The system also shuts off at the slightest pot holes. Windows also won’t work when trying to put them down from the drivers side

After bringing my 2021 Corolla into a certified mechanic that I personally trust is the most skilled and talented for Toyota brand, as a specialist for asian make vehicles. I was told that my unit was given a old battery despite the year of my vehicle. The safety of starting the engine was discovered 3 days ago. When I couldn't start the Car. I brought it to mountain view auto mechanics, in mountain view, CA. The mechanic I spoke with visually scans the engine and said that everything was not tampered with, but requires a replacement battery, by the Toyota dealership; any of the locations. Because the car is a 2021 it falls in the category of standard brand new warranty. A diagnostic fee has been said by Toyota of Stevens creek of the amounts of 230.00$ to find the issue. Even though I in with in warranty. I work at Quiktrak inc. as Field Inspector/authorized inspector my id# is CNP995 I perform inspection audits for vehicles and vessels all in the northern areas of California. I am complaining about my vehicle being un-safe for long term use, because of contraindications of how my car is. For the record. I have not modified any thing that would void my vehicles basic warrenty. Nor the CA emissions warenty

Around 1900-2000 miles of ownership, 6 months after purchase, I was driving on I95 in the rain, 65 MPH, traffic in West Palm Beach, Fl, on my way to Apple(Mall). Upon exit, my engine completely stalled out. I pulled off road, not smoothly at all, turned off the car and it restarted 30 minutes later so I resumed my travels. The next day, my vehicle alerted me that the communication module had blown, not in those words. I had called Toyota and told the representative of the spoken alert and she informed me to go to dealership. They were clueless how to fix and reset battery. This communication is vital for all modules to perform safety features. Today I had a large screw in tire and no alerts of low pressure. Tire read 12 lbs pressure. My lane departure/correction doesnt work half of the time. My attorney has informed me that he hears there are concerning issues involved and noncompliance has led to him filing suit today. Software Source code leaked and left open on GitHub for 5 years has compromised the safety of all connected customers and their devices.

Apparently my car's Engine Coolant Bypass Valve is broken and it's a common case with my car's model that's made after 2020. Seems like it there's a need a recall for such a thing since it's a defective part they knew about since my car was screaming at me for an engine maintenance. However, the check engine wasn't on. I went to Toyota at Elk Grove, CA and knew what the issue was pretty quick and charging me $700 to get it replaced. I feel like it's intentionally being handled poorly so they keep charging me 700 every few years to replace a $80 part. As well my story is very common with other customers of Toyota Cars made after 2020. I wanted to report this since that's potentially harmful to put out a car that could have engine problems easily. Especially it's a fairly newer car. Also, they have yet to replace the part for the known recall on this vehicle.

The problem of windshield wipers going off either once, or at varying speeds has not been able to be reproduced by my Toyota dealership. It goes off right when I turn the car on 50% of the time and is very inconsistent. Wipers typically go off after using the turn signal, making long turns (such as into a parking spot), and driving on long curved roadways. It has been inspected and nothing has been found. There are no warning lights or messages.

Hi, This car has had a recall since 12/20/23. Toyota still has no fix for us. I have this car on the TURO rental platform, but they unlisted my car because of this recall. I want to file a complaint on Toyota for not offering me a fix and killing my business. Is they any way to put a hold on this until a fix comes out? Since January 2024, my vehicle has been recalled. They said there would be a remedy by Q3. We called Toyota Dealership in Irving TX and they said next month. Primewell PS830 205/55/R16 - These tires were discontinued by the manufacturer one or two years ago and replaced with an upgraded tire because of tread skid and wear issues. The tires were on a Certified Used Toyota Corolla I purchased on September 29, 2023 from Toyota of Lincoln Park, Chicago, IL. I mentioned on the test drive that the brakes were making noise at every stop, with an occasional thud sound. The salesman, Dustin Dolan, first told me the noise was because the car hadn't been driven recently, then that the noise was normal and then that he agreed the sound wasn't right. He said he'd have the brakes "looked at." They weren't. The car consistently made skidding noises when braking. The dealer continued to tell me the brakes were fine and the tires were fine because they would never sell at car with unsafe tires. I also found a slit in the rear passenger sidewall. Took car to a reputable dealer. They found the tires with uneven wear and choppy condition. They also discovered rusted rotors. They repaired the brakes and replaced the unsafe, discontinued tires with new tires.

Engine Maintenance Required message came on. Took the vehicle to the dealership and was told that it was the Cooling Bypass Valve. The service advisor said he knew what the problem was before even looking at the vehicle but still wanted to run a diagnostic and charge $800-$900 for the repair. I was told my vehicle was no longer under warranty.

Headlights are defective. There is a "V" shaped dark spot in the headlights of the vehicle when on low beam. The significant unlit "V" shaped area does not allow the driver to see objects in the roadway which come up on a driver rather quickly, resulting in collisions. Toyota is aware of the defect, but will not act on it. Toyota Claim # 220811002276. There have been two collisions involving items in the roadway due to this headlight defect. Total out of pocket costs on the damage have been in excess of $1,500.

I have brought my car to Toyota of New Orleans (the same place I bought it) regarding an issue where if my kids are in the backseat unbuckled the seat belt alert does not ring to indicate that the seatbelts are not hooked also if I undo the seatbelt while driving it does not alert either. Also, when I am driving and nobody is in the backseat, and nothing is on the seats etc. the seat belt alert will begin to ring. I have record this on my phone and showed Toyota they have checked my care twice and I let if at the dealership for 3 days and they cannot duplicate the problem I explained it happens at different times however they said they see the video but can't get it to do it at the shop. so now I am stuck with a car that is a safety concern for me and my children.

Got in the car this morning and the check engine light came on. Needed to get to work so started driving. About 5 minutes later car started shuttering and reduced engine power displayed. Had to do 20 MPH all the way to work. Did not make for a good morning. The lane keep assist and other safety features were turned off as well. Not very safe when driving.

I-20 0400. TRAVELING IN THE RIGHT LANE. 18 WHEELER AHEAD WAS PULLED TO THE SHOULDER. I MOVED OVER TO THE LEFT LANE. WHILE MOVING OVER THERE WAS METAL IN THE ROAD(LEARNED FROM STATE POLICE LATER FROM THE 18 WHEELER). UNABLE TO AVOID THE OBJECT THE RESULT WAS A BLOWN TIRE. THE DESIGN OF THE LEFT HEADLIGHT IS MADE SO ON A TWO LANE ROAD THE ONCOMING TRAFFIC WONT BE BLINDED. HOWEVER, WITH AN INTERSTATE IF IN THE RIGHT LANE THE LEFT LANE HAS A BLIND SPOT. THE HEADLIGHT IS 1/2 THE LENGTH OF THE RIGHT HEADLIGHT. PLEASE PLEASE LOOK INTO THIS. I WAS LUCKY WITH ONLY A BLOWN TIRE MORE SEVERE DAMAGE OR LOSS OF LIFE DUE TO LACK OF VISIBILITY ON DIM SETTING IS FRIGHTENING.

I am making a complaint about my 2021 corolla SE because I am constantly running into issues that cost and are not my fault as to why these issues have come about. I bought my car in March 2021, in May 2021 my car hit a bump in the highway and cracked my rim and bust my tire. Luckily I have alloy and tire coverage so I did not have to come out of my pocket. Next, my two front tires are constantly wearing down on the inside of the tread making the wires be exposed and causing me to have a blow out or needing to replace my tires at least 4 to 5 times since I have had my car. My car only has 71,000 on it to need that many tires replaced. I go and get my car serviced where I purchased my car at the toyota dealer so If I needed and alignment or tires rotated they would have let me know that and the only time I was informed I needed an alignment was when I got all new tires. Then, in November of 2022 my front lighting sytem when completely out ( signals lights, blinkers, brake lights, etc. I went to get it checked on and the dealer made me feel as if I had an entire electrical issue going on with my car and I would have to pay $160 x2 in order for them to just figure out what was wrong with my practically new vehicle. They came back and explained it was only a fuse and the fuse is $5 but still attempted to charge me for diagnostics. After this encounter I had to be ugly just to not come out of pocket for a large sum. At that visit there was an "engine malfunction" light on as well that they did not mend and continues to blink to this date. Prior to my last servicing appointment last week my entire radio system went out ( backup camera, radio, gps system, bluetooth). I was told at the dealer that I would have to pay $160 for them to diagnose my issue and and then pay for the repair. I asked my service manager if there were any recalls on my car for these issues and he stated no but as for the 2022-2023 models there are recalls. I need help with this.

Engine coolant bypass valve malfunctioned at 34xxx miles, causing “engine maintenance required” this could cause the engine to get too hot or not hot enough and affect the internal components of the engine. I was sitting idle in my car. The two cars crashing icon came on even though I was in the parking lot doing nothing. There was also issues with the other indications where I would use the back arrow on my steering wheel to clear the issue and it would not clear. Seems unsafe and scary.
