On [XXX], I was driving my Blazer up to our local Museum to grab some information. The Museum is about a mile and a half from my house. When I had left my house, the car was driving like normal. I pulled into a parking spot at the Museum, and I went to shut the vehicle off, and I thought maybe the check engine light had come on but wasn't completely paying attention to the dash. About 10 minutes later I went to restart the vehicle to leave and I noticed the check engine light on. Since the vehicle seemed to be driving okay I thought I could make it home and check the car's computer to see what the issue was. As I was leaving the parking lot I stopped at the end of the parking lot to look before pulling out onto the road and the Blazer said shift to neutral and restart the vehicle. When I did that the vehicle would not start. It would just crank and crank and then it would finally start for a few seconds. I was going to back my vehicle up out of the road but each time I would try the car would die. I am very thankful this did not happen on the highway or even on the freeway. A friend and myself had to push my vehicle back into a parking spot. The DTC code is a P023F. When this happens, you can't technically shift the vehicle into neutral to push it out of the way because the car has to be running to shift. I was able to force it into neutral, however. I made an appointment with Awesome Chevrolet in Chehalis WA and had to have it towed to the dealer. They stated it was the fuel pump module. I had at that time 18,388 miles on the odometer. Unfortunately, I no longer feel safe with this vehicle in case it happens again. Where I am hours from home or on a 2-lane road or freeway. The fuel pump module failure appears to be a known issue with GM vehicles. The fuel pump module was replaced and was able to be replaced under warranty. I hope that it does not happen again. For about a week and a half my car had made a high pitched wheezing/hubbing sound when started cold. This occurred at all temperatures cold and warm. Yesterday December 13th I went to run some errands and about a mile into my drive my 2021 3lt 3.6 V6 Chevrolet Blazer lost the ability to power itself. There was no electrical problem however the power train lost power all gears drive and reverse. I was able to get off the roadway without incident. I scan my check engine light and had several check engine lights including a P 0796.

My 2021 Chevrolet Blazer experienced a radiator failure at approximately 42,000 miles. The radiator and cooling system malfunctioned, causing coolant loss and triggering a warning light on the instrument panel. The failed radiator is available for inspection upon request. This defect put my safety and the safety of others at risk because a compromised radiator can lead to engine overheating, sudden loss of power, or even steering assist reduction while driving. These conditions increase the likelihood of an accident, particularly at highway speeds. The problem was confirmed by my Chevrolet dealer, who diagnosed the radiator as needing replacement. They initially quoted $1,800, later reducing the cost to $915.90 after acknowledging that approximately 20 customers per month at their dealership alone are experiencing the same issue. This indicates the defect is widespread, at least regionally, and may be systemic across the model. The vehicle has been inspected by the dealer service department, which is an authorized GM representative. No police or insurance inspection has occurred. Prior to the failure, a warning lamp illuminated on the instrument panel, alerting me to a cooling system issue. This was the first symptom I noticed before bringing the vehicle in for service. I believe this premature radiator failure represents a systemic defect in the 2021 Chevrolet Blazer’s cooling system. It occurred well within the 5‑year/60,000‑mile warranty period, yet the dealer claimed the radiator is not covered. I am requesting NHTSA investigate this issue, as it appears to affect many owners and poses a significant safety risk.

P0128, AC not working, Steering Assist Reduced, and Temp Gauge not working. Problem caused by faulty ECT wiring. First the problem is the ECT is a two wire (signal + and -) 26 gauge wire running through the ignition coil wire harness. Ignition coil signals are very high voltage and fairly fast frequency which causes a lot of EMI which is easily picked up on a 2 wire voltage based wire set. The ECT wiring should have been a twisted shielded pair if running next to Coil wiring or they could have used a single wire resistance thermistor, but they should have just routed the signal a different way to begin with. This is a design issue and it should be a recall...maybe a class action lawsuit is needed to force them to fix it correctly. The FSB (field service bulletin) fix blames loose connector terminals and it patches the computer software to not include ECT wire failures in the Steering, AC, and temp gauge inhibit logic (overtemp readings will still fail off these functions). Also the P0128 code can come back occasionally.

- 11/24/2025· STEERING,ENGINE
In September 2023, when getting my vehicle serviced for a routine oil change, I was told there was a leak in my radiator. They replaced the entire thing under warranty. On November 15th, 2025, while driving on the highway heading home from out of town with my children, an error flashed up saying “steering assist is reduced – drive with care” Upon arriving home, the fan on my car was running on high for a solid 10 minutes after turning the car off. No recall but Special Coverage N252508350 covers the thermostat replacement. Upon inspection by a mechanic, I was informed that the radiator is also leaking again in the exact same spot as it was previously. It should be noted I have never seen leaking fluid in my garage/driveway. I’ve driven less than 10,000 miles since the radiator was replaced previously. If there is a known problem with the coolant system which can cause overheating and steering assist issues, why isn't the faulty radiator and thermostat recalled. The radiator is cased in plastic to aluminum. Plastic can easily catch fire if a car overheats. The casing also prevents an owner from even knowing if there is a leak since the plastic is catching leaks - the mechanic informed me that's why I didn't know about both times it leaked. This is a 4yr old car with 46k miles, twice replaced radiator and thermostat. A recall should be issued since both problems could potentially cause a fire from an overheated engine & loss of steering assist while driving. A google search quickly showed me thousands of other Chevy Blazers from 2019-2021 with reoccurring thermostat and radiator issues. I feel unsafe driving in my vehicle but still owe too much to even trade it in for something else. I've owned dozens of vehicles in my lifetime, used and new and keep them 8-10yrs and have never had to replace a thermostat and radiator. This vehicle is dangerous due to GM/Chevy's cheap radiator model and thermostat which could overheat and catch fire
- 11/20/2025· AIR BAGSCrash
The contact owned a 2021 Chevrolet Blazer. The contact stated that while driving at 30 MPH, a 2012 Mazda CX-7, allegedly going at 70 MPH traveled through an intersection and crashed into her vehicle. The Mazda later crashed into another unknown vehicle. All the air bags deployed; however, the driver's side air bag exploded in her face. The air bag dust was in the vehicle. The contact was rendered unconscious due to the incident. All the warning lights illuminated after the crash. OnStar was activated and disabled the vehicle. There were unknown fluids leaking from the vehicle. There was no reported fire. A police report was filed. The contact sustained a severe brain injury, and medical attention was provided at the hospital. The vehicle was towed to her residence. The insurance company later towed the vehicle on a flatbed to a tow lot. The local dealer was not contacted. The vehicle was not diagnosed or repaired. The manufacturer was not notified. The vehicle was deemed a total loss by the insurance agency. The failure mileage was approximately 36,463.
- 11/17/2024· SERVICE BRAKES
The brakes failed on my SUV as I was driving on a city street. Yes it is available for inspection. Losing my breaks almost caused a collision with the car in front of me but due to the distance between the cars and the low speed at the time, I was able to avoid collision and detour into a parking lot where I cruised to a stop with the help of a parking lot space concrete stop. The problem was not reproduced by me but the service center computer diagnostics confirmed break issues. The service center says that the cause was low voltage of the car battery which affected the brakes. This is a very dangerous situation if the vehicle can lose complete braking ability due to a low voltage battery. This is the reason for reporting the issue. They replace the battery and said that should fix the issue. They said the only recourse to avoid this happening again is to replace the battery around every 2 years. The issue was inspected by a service center owned by the manufacturer of my SUV. The warning lamps only occurred as the brake system failed. A message of "Brake System Failure" appeared and the "Check Engine" light came on. Within the previous week of the issue the SUV also had other electrical issues such as the Climate Control interface went out and the rear camera also went out.

Action Required "Steering Assist Warning" drive with caution, Temp gage reads cold, A/C stops working, engine cooling fan runs on high speed (even after the engine is shut off), pending P2018 code, with no Check Engine Light (CEL). Replaced Engine coolant Temperature (ECT) sensor, problem did not go away. Several reviews suggest to replace the thermostat, check fuses for proper insertion, reroute ECT wiring, and possibly replace radiator. Most of these repairs can be very expensive. Chevy Blazer Forum states that the issue is a out of range detection of ECT sensor reading. GM needs to determine a true solution and stop guessing at owner expense/frustration.

The Radiator has a crack in the upper left front of the core causing the fluid to leak and lose pressure causing the engine to overheat. This is a manufacture defect that has not been addressed as of 10/16/2024. The leak was discovered by a certified GM mechanic at a dealership when doing routine maintenance and inspection. The vehicle has never had any indication or advisory light that would alert the operator. As the engine overheats it can cause many of the other components of the vehicle to disable, including but not limited to the ADAS systems as well as power steering and/or collision safety components. This is a manufactures defect with the aluminum radiator that needs to be addressed immediately. Given the information from multiple GM dealerships the amount of newer low mileage Blazers that have been confronted with this problem is unprecedented. This not only creates a danger for the passengers of the vehicle but also every other vehicle on the road.

In March after a short drive, 24 miles, the car temp was pegged at Cold. After turning off the car, the fan continued to run several minutes. After a quick internet search several posts commented on a similar issue with their vehicle. Two potential issues were identified - thermostat failure or programming issue. With less than 40,000 miles on the car, I purchased a diagnostic tool. Running the tool program two errors were identified. After resetting the sensors, the problem went away. In September, yesterday and today the problem occurred again - and the error reset fixes the issue. I believe Chevy needs to fix the issue with a recall to either update programming or replace the thermostat. [Several posts state the thermostat replacement didn't fix the issue and is very expensive.]

Request for Recall Review – 2021 Chevrolet Blazer Steering Assist and Cooling Fan Issue Hello, I am writing to report a recurring issue with my 2021 Chevrolet Blazer and to request that this concern be reviewed for possible recall or warranty coverage. The vehicle frequently displays an “Action Required: Steering Assist is Reduced. Drive with Care” message while driving. Additionally, after shutting off the vehicle, the engine cooling fan continues to run for up to 20 minutes, as if the vehicle is overheating. This issue has occurred multiple times and raises concerns about potential problems with the steering assist system, cooling system, or electrical sensors. I’ve noticed that other Blazer owners have reported similar experiences online, which suggests this could be a broader issue. Could you please confirm whether there are any existing recalls, technical service bulletins (TSBs), or warranty programs addressing this condition? I would also appreciate guidance on the next steps for inspection or documentation. Vehicle Information: Year/Model: 2021 Chevrolet Blazer

When I am accelerating it seems like my car hesitates to get up to speed, at times feels like it is slipping to get traction. I put my car in AWD and didn't seem to notice the issue, when put back into 2WD, the issue happens, it is not an every time thing, but enough that my husband, who doesn't drive my car a lot noticed it and a coworker who has the same year as mine also has the issue. Of course when I took it into the dealership to have it looked at the car didn't do it, so as far as they are concerned there is nothing wrong with it, which is NOT the case. My husband has a 2020 Blazer and it doesn't have this issue. I understand that it is a different year, but if it has the same engine and transmission I would think there shouldn't be much of a difference in how they both drive. It's very frustrating to spend this much on a vehicle that I leased in May of 2020 and it has been in twice for this issue and the parking break issue (which GM's reasoning as to why this turns on by itself and does not turn off is BS, there is still an issue with this as well) and when serviced there is never anything wrong with it. So could it be an issue with the transmission, traction control or possibly the engine?

I went out to start my car and the light said that it was not reading the key fob because of low battery. I went and got the other key fob which had never been used and had the same result. The car was towed by Chevrolet to the dealer and it has been there for over 3 weeks. They found that it wasn't a faulty key fob but it was the receiver that gets the signal and allows the door locks to open and the engine to start. I tried moving the shift lever, stepping on the brakes, trying the remote start on the key fob and nothing would start the car nor would it lock/unlock the doors. It just kept giving me the notice that it couldn't read the key fob. I called ONSTAR and tried the actions they told me but that did not work either and they said to contact my selling dealer. ONSTAR contacted the Chevy warranty number for a tow truck but at 9PM it was too late for them to make a service call. What if I was out in a parking lot or in a remote area and experienced the same problem with no assistance. I have been trying to find out what the dealer is doing to repair my vehicle but I have to call the GM complaint line in order for them to try and contact the dealer about my car. They finally gave me a rental vehicle since the vehicle is less than 1 year old and only has about 6,000 miles on it.

Receiving 3 different errors on dash display. 1) Action Required- No Remote Detected 2) Action Required- No Remote Detected Press Brake to Restart 3) No Remote Key Wa Detected Place Key In Transmitter Pocket Then Start Your Vehicle. All of these errors are intermittent and have appeared with using 2 different Remotes. Batteries have been replaced also. This in itself is not so much a safety hazard (annoying), but what is intermittently happening is the car shuts down while in drive and in reverse. So far it has not shut down on the freeway but has shut down while on City streets. We have taken it to 2 different GM Dealers a total of 7 times in the last 3 months(currently at the dealer now). Each time the dealer is saying they cannot duplicate the errors. We get it back and within 1 (usually the same day)day it is right back with the errors. Clearly these errors are a nuisance and dangerous if the vehicle shuts down. I have additional service documents and photos and videos but due to the maximum I can provide they are not uploaded.

my vehicle is roughly 40k miles. The temp gauge was stuck on C the cooling fan was running continuously even when the car was off and the checked engine appeared. The mechanic at the dealership said the thermostat is stuck in open position causing the computer not to know if the engine is cooling properly. Therefore, the manufacturer needs to look into recalling this issue costing people thousands of dollars. This was my first Chevy and probably my last.
- 09/12/2023· SERVICE BRAKES
The breaks make a squealing noise all the time which is concerning. I was told by Beaver Chevrolet that this is normal as the breaks were changed in newer car models. They have made a squealing noise ever since I bought the car brand new. I took the car into Beaver Chevrolet approximately 4 months ago and was told that they were unable to replace the breaks because they "weren't squealing bad enough". They did spray my breaks down, however they told me that once I hit 36,000 miles if the issue was still occuring (which it will be) that I would have to pay to have the breaks sprayed. The issue persisted and the spray they used only lasted a few days. Typically breaks should not squeal unless they have gone bad, which isn't the case here. I find this issue to be a safety hazard as how are customers supposed to know when their breaks actually do go bad if they squeal all the time. The issue was reproduced, but it is unclear what their standard is for determining the actual noise to breaking ratio.

Auto braking system went off when I was at stop light & began to move forward (was 2nd in line of cars) and I accelerated. On the roadway, there are construction 'steel plates' laying on the ground in the area that all cars must drive over (I have driven over them in my same Blazer before) because we have no choice. One I pressed on gas & began moving over those plates, the Blazer stopped itself VERY abruptly & jerked my head forward & dash said I had to press on brake let off or something in order to move again - it was all very fast & I had a line of traffic behind me at 5:15 pm on a Friday (9/5/25). I'm very luck I did not get hit from behind & car behind me paid attention! That is the 3rd time my car has stopped suddenly; 2 times before it was when going up my very steep driveway so I had no one behind me & I wasn't as shook up - Chevy needs to REALLY look at this! While idling in my driveway, the engine started to shake and the check engine light came on. Got it diagnosed and was told by a private repair center that we had lost compression in cylinder 4 and that it was not worth the repair and we needed to replace the engine. Got it towed to the dealership, Bill Fox Chevrolet, for a 2nd opinion and they confirmed what the private shop had said. Because the vehicle was well maintained with regular preventative maintenance, it was deemed the failure was not the fault of the owner. GM extended a goodwill warranty to help cover the cost of the engine replacement (70% coverage. We were 1 month and 2,000 miles past our warranty). They never told us root cause, only that it would have cost more money to repair it than it would be to get a new engine. The Blazer had been running fine up to that point and was my wife's main mode of transportation. The Blazer only had 62,000 miles on it when the failure occured.

Power Liftgate randomly opens when driving or parked. Even when the power switch is turned to off the liftgate will unlock randomly either parked or driving. I have contacted the dealer and GM no recall currently exists. Dealer recommended replacing the latch mechanism which I have ordered to replace myself.

At routine oil change dealer noticed radiator was leaking. 60,840 miles Took it to an independent repair shop who confirmed radiator needed to be replaced. Has also seen same on other 2021 Blazers. No visible signs of leaking (fluid on garage floor) or warning indicators. No recall issued on this, however, seems to be a common problem on 2021 Blazer based on Chevy forums. Due to safety issues, vehicle is currently in shop for the needed replacement repair.

My name is [XXX] and I was a passenger in our 2021 Chevy Blazer with our young child in the back seat when the brakes suddenly failed while we were driving on the freeway. The brake pedal went stiff and wouldn’t respond. It was terrifying — we truly felt like we were going to crash. I was panicked, and my child was crying. Since then, I’ve been too afraid to drive or ride in this vehicle, even if it gets repaired. This isn’t the first issue with the car, and GM has not resolved it in a timely or helpful way. The vehicle is currently at the dealership and we are paying for a rental car out of pocket. I’m reporting this because I believe this vehicle poses a serious safety risk to families like mine. I’m asking NHTSA to please investigate this issue before someone gets hurt. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)
