    I was on the exit lane coming from Costco on 12/31/2022 at around 12:20 PM with a speed of about 40 miles . My exit is about 100 yards away. Tesla model 3 that I’m driving suddenly accelerated at high speed and pulled towards right and hit the wall next to freeway exit lane. Then it hit the wall on rear tailgate and rolled to middle of freeway within a fraction of seconds. Two vehicles coming on the freeway hit my car and after second hit my car completely stopped in the middle of freeway. I was able to come out safely and later CHP officers came and towed the vehicle

    WHILE DRIVING, THE DRIVER'S SEAT WILL RANDOMLY START MOVING FORWARD, BACK, UP OR DOWN WITH NO INPUT FROM THE DRIVER. IT IS VERY DISTRACTING AND CAN MAKE IT DIFFICULT TO SEE OUT OF MIRRORS OR REACH PEDALS. NOTIFIED TESLA AND PROBLEM CONTINUES TO HAPPEN OCCASIONALLY AFTER 2 SERVICE VISITS ATTEMPTING TO REMEDY THE ISSUE. HAVE NUMEROUS VIDEOS OF THIS HAPPENING WHILE DRIVING. THIS HAS HAPPENED ON AT LEAST 6 SEPARATE DAYS BEGINNING ON OCTOBER 27TH, 2020.

    The car has message saying "Safety Restraint System issue" . There has been no event to cause this. No recent MVA or any stress or damaging incident to the seat belt or seat in general. Reviewing this on the internet, this is a common defect on Tesla's. The company won't fix it unless you pay them to do so which is crazy considering it is a defect in their safety system. And a well known one to them from my communication with their Tesla service centers which have a monopoly on fixing Tesla's. He said there would have to be a recall. So that is exactly what needs to happen from the NHTSA.

    Recall 21v-00d was registered DEC 2021, notice said waiting for parts. It's been OVER 12 months and no parts yet? Also, the phone number Tesla has listed on the recall notice and their recall info on your website goes to a self-service site that states to use your Tesla app to schedule a repair so there is no way of contacting the company directly to find out about the status of the recall.

    I was driving my vehicle. It said there was an issue with code "RCM_a056 Front Passenger Safety Restraint System Fault." Many older model 3's are having this issue and with the quote from tesla, they would need to perform a retrofit for the electrical wiring system.

    This vehicle has the phantom braking issue whether you are using adaptive cruise or autopilot, and has happened at least 15 times in the past two years. Because of the sudden braking, we almost had a collision with a semi-truck coming up behind us on I-5 heading south the last time and the driver thought I was brake-checking him, making him very angry and giving obscene gestures at us for it. We reported it to the Tesla service center, and they basically said it happens, and they disclosed it in the operator manual. That is unacceptable. This is very dangerous, and lives could be lost in the circumstance where someone is following close behind. Tesla basically is saying it can happen and there is no fix which absolutely unacceptable and this car is dangerous to drive.

    Tesla issues an update to software that forces drivers to go through a jumble of menus to do important tasks like turning on defrosters and windshield wipers. These used to be one touch items on the home screen, but now require drivers to take their eyes off the road for a dangerous amount of time.

    Tesla pushed an update (v. 2021.44.25.2) to my Model 3 a couple of days ago that has dramatically altered the usability of the car, and potentially has safety implications. There was no warning prior to the update being applied that such changes to the car's user interface were included, nor is there any option to revert to the prior user interface. Major issues with the changes are outlined below: - 1-button, single-click shortcuts to seat heating, defrost/defog functionality have been removed; when driving in bad weather where the screen may fog up rapidly, you're now required to take your eyes off the road, and click twice on a small target on the touch screen to activate the required functionality. - 1-button, single-click access to the wipers has been removed, now requiring one to either rely on the auto-sensing functionality (which works properly about half the time), or click through the HVAC menu to adjust the wiper speed. - The quick access driver profile selection dropdown in the previous interface has been removed. My spouse and I differ significantly in height, and about half the time, the car selects the wrong profile when we're both getting into the car (despite our phone keys being linked to our specific profiles). With the earlier UI, it took one tap on the screen to switch to the correct profile. Now, I have to dig through menus to select the correct profile, while my knees are being slowly ground into the steering column/dash. - When driving, the speedometer, gear selection and navigation look significantly different than in the earlier UI. This is likely to lead to more distracted driving while we get used to the new interface. I filed a service request with Tesla, requesting that they revert our car back to the prior version of the interface; this request was denied and the appointment was automatically canceled with a generic message about firmware updates being rolled out on a schedule.

    While operating TACC with lane keeping assist, I experienced multiple iterations of phantom braking when the road and weather was clear ahead. This phantom braking could potentially cause other drivers to collide with the vehicle that unexpectedly braked in the middle of the highway. This problem has been a common issue with multiple people taking their cars to the service center to get checked out with no problem identified. There are no warning lights or indicators that the Tesla is going to phantom brake on the highway. Recommend having Tesla provide information regarding this phantom braking.

MY CAR ACCELERATED GENTLY TO 70 AND THEN AS I APPROACHED AN OVERPASS THE CAR SUDDENLY BRAKED (OR WENT TO REGENERATION) AND SLOWED INSTANTLY TO ABOUT 40 MPH WHEN I WAS ABLE TO APPLY THE ACCELERATOR AND RETURN TO SURROUNDING TRAFFIC SPEED AND MOVE RIGHT TO THE NEXT LANE AND RECOVERED FROM THE SHOCK AND SURPRISE OF WHAT OTHER TESLA OWNERS REFER TO AS "PHANTON BRAKING". I WAS FORTUNATE THAT NO OTHER CARS WEREN'T FOLLOWING ME OR THEY LIKELY WOULD HAVE CRASHED IN THE BACK OF MY CAR. Ever since the update, the vehicle, cruise control and auto pilot systems no longer function properly, and while I was in, cruise control and auto pilot traffic stopped ahead and the auto pilot system disengaged while I was waiting while it was slowing down, which has never happened in four years of ownership and over 60,000 miles of driving. The new recall software update has made the vehicle unsafe and it should be retracted and corrected. What once was a safe feature is now unsafe. Please investigate this with Tesla to prevent possible injury or death resulting from this programming failure.

    An incident occurred in April of 2023 while running FSD Beta 11.3.6 firmware. I have been unable to retest this location on more recent software, so this concern may have been mitigated with a subsequent update. As seen in the video, the yellow lane marking on the left transitioned from the roadway up onto the movable center median. Once on the bridge, the left repeater camera and the center display visualization both illustrate the lane centering was definitely biased towards the left side of the lane, a possible indication the vision-based lane centering was miscued by the yellow line now appearing on the barrier instead of the road surface. This left bias brought the vehicle uncomfortably close to the barrier, which resulted in a manual takeover. It is difficult to judge based on the low resolution of the repeater camera view, but my estimation is that the tires were approximately 12 inches away from the protruding bottom lip of the barrier. [XXX] This same left lane marking concern also occurs at the entrance to the Oakland, California Caldecott tunnel eastbound bore. The Google maps link provided shows the yellow line which defines the left lane edge disappears at the tunnel entrance, at which time the vision-based lane centering seemed to be cuing off of the curb as the lane edge, rather than there being a marking that would delineate a lane edge further away from the curb. An FSD beta user (name and date unavailable) reported contacting the curb at this location at approximately the same date as the GGB recording. [XXX] The GGB matter was brought to the attention of the Bay Area Toll Authority with the suggestion they contact Tesla. I have been unable to verify that the Toll Authority followed up on that suggestion. The release notes for Beta 11.4.4 suggest the issue might have been addressed, to wit, "Improved offset consistency when controlling for static obstacles."     I recently purchased a 2020 Tesla Model 3 Long Range AWD (VIN [XXX] internal systems and service records show that all recalls for this vehicle have been completed and closed. However, the NHTSA VIN lookup still shows seven open recalls. This is causing a major issue because I am participating in a California clean transportation incentive program (Replace Your Ride), which requires that all recalls be shown as closed specifically on the NHTSA website. Tesla states that the recalls are complete but has not updated the recall status with NHTSA. NHTSA customer service informed me that recall data can only be updated by the manufacturer. I am requesting that NHTSA notify Tesla’s regulatory or safety compliance team so they can update the recall reporting for this VIN.     I was trying to avoid a car parked on the side of highway on my right going between 45 to 55 miles an hour and slightly steered the car to the left, and the car swerved back and forth at the slightest steering wheel movement as if skidding in the snow. The road was dry (no rain) but the steering was not stable and the car ended up hitting a truck on the left. It caused a very serious accident and we are lucky that there was just a major car damage and no one was hurt. The steering seems to be uncontrollable and loose, and causes major swerving if one has to steer away at a high speed.

    During a month-long road trip in the US, I experienced frequent "phantom braking" incidents while using adaptive cruise control. In some cases, Autosteer was also active. By "phantom braking" I refer to sudden hard braking by the car itself in clear daylight conditions on interstate highways with no vehicles visible ahead of me for at least 1/2 mile. By "frequent" I mean that this behavior occurred multiple times per hour over multiple days. I decided to cease using cruise control for the remainder of the trip after the following episode occurred. With cruise control active on a four lane interstate, I was changing lanes to the right to allow a pickup truck to pass me on the left. I had just passed a semitrailer, who was in the right lane. I could see him in my rear-view mirror as I began the lane change. The road was empty and visible for at least a mile ahead on a sunny morning. We were all traveling about 75 mph in a 70 mph zone. While straddling the lanes, the car suddenly slammed on its brakes, requiring both vehicles behind me to take evasive action. I over-rode the car’s action by pressing the accelerator pedal swiftly and firmly. No accident occurred. I stopped using cruise control entirely for several months, and I have only rarely used it since then, and only when the road behind me is empty. In those few recent trials, I have not experienced any phantom braking. Finally, I speculate that the braking may have been triggered by "mirages" of what look like puddles on the road. We humans are familiar with this phenomenon, but the camera AI may have interpreted those as obstacles.

On December 6, 2020, she was on the way out of the parking gate and suddenly the car accelerated, hitting the side wall of the gate. The car flew over a two-way street, resulting in a compound fracture of her right hip requiring internal fixation and transfusion. The vehicle was inspected by the police. There were no warning lamps, messages, or other symptoms prior to failure.

    Phantom breaking occurred twice while driving east on highway sr 152. Our car was traveling straight with no cars in front of us. I no longer use cruise control. Apparently there is no fix. I have two relatives that have experienced same problem with their Teslas. I will suggest that they report to NHTSA.

    My Tesla Model 3 is experiencing premature suspension failures involving the lower lateral links and stabilizer links. These components were already replaced once, and they are failing again in less than 30,000 miles. This is not normal wear for suspension parts on a modern vehicle. These failures cause clunking, instability, and looseness in the front end—conditions that affect the vehicle’s ability to track straight and maintain control, especially over uneven pavement or during emergency maneuvers. This isn’t an isolated case. There are extensive reports from other Tesla owners across multiple models (Model 3 and Model Y in particular) showing the same premature wear on these suspension links. Many owners have parts failing even earlier. Once the car is out of warranty, Tesla charges thousands of dollars to replace these components, despite clear indications that the suspension was under-engineered for the weight of the vehicle. Tesla knows this is a recurring issue but refuses to issue a recall or goodwill repairs. A repeated failure of critical suspension links in such low mileage suggests a defect. These parts should not be wearing out this fast, and the resulting instability poses a real safety risk. I’m requesting NHTSA investigate these premature failures.

    Currently occurring with FSD Beta firmware V.2022.27.12 but the same has occurred with previous firmware versions also. Normal operation of the vehicle with the ADAS engaged puts the windshield wipers into AUTO mode. At times the windshield wipers are activated when the windshield is dry. It isn't raining or misting and there's nothing on the surface of the windshield at the location of the front-facing cameras. "No rain" occurrences seem to correlate with times when visibility ahead has low contrast and there are no highlights and shadows to sharply define the scene, (ie, a little "muddy"). Observed when it is overcast but not raining, around sunrise when the sun is low in the sky and the light levels ahead are relatively low and headlights aren't helping illuminate the road ahead, and at times there are high-contrast light and shadows immediately ahead but farther down the road there is mist or fog, creating the same low contrast lighting conditions some distance away. It is as if the ADAS is aware that the scenery is not sharply defined and is "wiping its eyes" in an attempt to gain a more sharply-defined view. Occasionally, but rarely in my experience, the windshield washer is also engaged. Having the ADAS activate the wipers at these times is not a safety hazard, per se, but it is distracting and concerning as it gives the driver the impression that perhaps the ADAS is malfunctioning. I've read in user forums reports from a fair number of FSD Beta drivers complaining that the wipers go on when the windshield is dry and the ADAS is engaged. I haven't observed the same when the ADAS is not engaged.

There is a specific spot on US Highway 183 north in Austin where the speed is set at 65 and the Tesla M3 will rapidly brake without any warning. There is a danger of being rear ended. I manually disconnect the ADAS in anticipation. I shouldn’t have to. In general, phantom braking is dangerous and could be attributed to using vision only sensors?

    Unknown issue with front passanger safety restraint system. Car suddenly notified me of an issue with the restraint system. Before the fault the car was simply just parked in my driveway, then I went into the car and the safety issue appeared. The seat was adjusted for my mom's height the day prior, this might have caused a wire to become pinched and caused the fault due to how the wiring under the seat is set up from manufacturing. Online search results show this to be a common occurrence for cars from 2019-2021 in Tesla.

WHILE IN THIS COMPLETELY INOPERATIVE STATE: THE AUDIO SYSTEM WOULD EMIT A HIGH PITCH NOISE; ANY ABILITY TO TELL SPEED; TURNING SIGNAL STATUS; ANYTHING WAS COMPLETELY IMPAIRED; THE CLIMATE CONTROL BLASTS ON HIGH FAN A TORRENT OF COLD AIR. AT 2000 MILES; THIS HAS HAPPENED SEVERAL HUNDRED TIMES - ON AVERAGE ONCE EVERY 10 MILES DRIVEN; THOUGH SOMETIMES TWICE WITHIN A FEW MOMENTS (BOOTS UP ONLY TO RESET AGAIN) AND SOMETIMES IT CAN GO AS FAR AS 30 MINUTES WITHOUT FREEZE; BLACK; RESET. THIS IS EASILY THE MOST EXPENSIVE CAR I'VE EVER PURCHASED; AND HAD NEARLY 0 MILES UPON PICKUP - IT IS EASILY THE MOST FRUSTRATING EXPERIENCE I'VE HAD WITH A CAR. TO NOT BE IN CONTROL OR HAVE ANY VISIBILITY TO THE MACHINE DRIVING AT HIGHWAY SPEEDS WHEN EVERYTHING GOES COMPLETELY BLACK AND THE COMPUTER TAKES MINUTES TO RESET. THE REPAIR FACILITY HAS ESTIMATED $2400 WHICH THEY'RE ATTEMPTING TO CHARGE ME.

    While driving my Tesla Model 3 with self-driving engaged in excellent conditions, the car suddenly braked to a full stop. There was no apparent reason to do so and the driver did nothing. The road ahead was free of traffic, and no obstacles, pedestrians or animals were visible. It was possible to resume driving by turning self-driving off. Tesla forums online show many instances of others reporting similar problems in various Tesla models over several years. A rear end collision will ultimately be the result of this flaw if it hasn’t yet occurred.

    The contact owns a 2020 Tesla Model 3. The contact stated while driving 10 MPH and making a left turn, the steering wheel seized, and the front passenger's side of the vehicle lowered to the ground and made an abnormal grinding sound, before coming to a stop. The contact stated that upon exiting and inspecting the vehicle, it was noticed that the tie rod had detached from the front passenger's side wheel, and the front passenger's side wheel had turned inward and was pinned by the wheel well. The vehicle was towed to a Tesla Service Center, where it was diagnosed and determined that the front passenger's side steering assembly needed to be replaced. The contact was charged a fee for the repair. The manufacturer was notified of the failure. The failure mileage was approximately 36,000.

  • 11/21/2022· FORWARD COLLISION AVOIDANCE

    "Phantom braking" issue. When car is in autopilot or enhanced autopilot mode (an upgrade I purchased after I bought car), will brake suddenly and firmly at high speed for no apparent reason. Until September 2022 car would very occassionally brake too easily in autopilot mode (perhaps 3 times in two years), but it was not sufficiently forceful or frequent to cause alarm. However, on 9/26/22 and 9/30/22 during daytime driving car violently braked on highway at least 6 times over approx 500 miles of driving. Fortunately, these incidents caused no damage, collision, or injury. There was little additional traffic on the road at the time. However, items in car, and two cats in carriers on back seat were thrown forward and displaced, and the incidents were very frightening. When I realized this was happening in autopilot modes, I stopped using feature completely and have not since. At 11/19/22 service appt for issue at Alhambra Tesla dealer I was told this was a "known issue" with no current solution; however, I was told that the more I used autopilot and hit accelorator during phantom braking, the faster car would "learn" not to do this. Given that taking the risk of continuing to use autopilot at all poses a serious risk of injury, I am truly shocked at the suggestion that I continue to use this feature and essentially train my car not to be dangerous. Via Tesla app I gave information and dates to Tesla prior to service appt and was told that they would attach vehicle log to file for service visit.

The local dealer was contacted. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. The manufacturer was made aware of the issue.
