    Southeast Toyota Distributors, LLC (SET) is recalling certain 2015-2018 Toyota 4Runner vehicles equipped with an accessory Billet Grille Overlay installed by SET or provided by SET and installed by SET dealers. The U-nut that secures the overlay grille may fracture, allowing the Billet Grille Overlay to detach from the vehicle.

    Risk: If the Billet Grille Overlay detaches while driving, it can become a road hazard, increasing the risk of a crash.

    Fix: SET will notify owners, and dealers will replace the securing U-nuts with upgraded hardware, free of charge. The recall is expected to begin January 26, 2018. Owners may contact SET customer service at 1-866-405-4226. SET's number for this recall is SET17C.

    Risk: An incorrect label may result in the vehicle being overloaded, increasing the risk of a crash.

    Toyota Motor Engineering & Manufacturing (Toyota) is recalling one 2016 Toyota 4Runner vehicle manufactured November 1, 2016 and one 2017 Lexus GX460 vehicle manufactured November 16, 2016. The affected vehicles are equipped with Takata PSPI-LD front passenger air bag inflators that may have been manufactured without ignition booster tablets. As a result, in the event of a crash, the air bag may not fully inflate during deployment of the primary stage. As such, these vehicles fail to comply with the requirements of the Federal Motor Vehicle Safety Standard (FMVSS) number 208, "Occupant Crash Protection."

    Risk: The improper inflation of the passenger air bag may increase the risk of injury in the event of a crash.

    Fix: Toyota dealers have replaced the passenger side air bag modules, free of charge. Toyota notified the affected dealers on December 15, 2016. Owners may contact Toyota customer service at 1-800-331-4331. Toyota's number for this recall is G06, GLR.

These vehicles had their driver or passenger frontal air bag inflators previously replaced under a prior recall using inflators of the same design. The inflators may explode due to propellant degradation occurring after long-term exposure to high absolute humidity, temperature and temperature cycling.

    Risk: An inflator explosion may result in sharp metal fragments striking the driver or other occupants resulting in serious injury or death.

    Fix: Toyota will notify owners. DROVE IT A FEW MORE DAYS AND EVENTUALLY LIGHTS WENT OUT. FAST FORWARD A WEEK AND LIGHTS CAME BACK ON. SAME ROUTINE WITH THE OB2 BUT THIS TIME THE CODE P2443 CAME UP. GOOGLED IT AND " SECONDARY AIR INJECTION SYSTEM SWITCHING VALVE STUCK CLOSED BANK 2". FOUND OUT THERE IS A HUGE HISTORY WITH THIS PROBLEM NOT ONLY WITH MY 4RUNNER BUT WITH TACOMAS, TUNDRAS AND SEQUOIAS. WITH CERTAIN YEARS OF TUNDRAS AND SEQUIOAS, TOYOTA HAS PAID FOR THIS FIX. WHY NOT THE 4RUNNERS AND TACOMAS??SAME PROBLEM!! THIS WHOLE THING IS A BIG SHAM . IT WILL COST ABOUT $2000 PLUS FOR THIS ISSUE TO BE FIXED. WHERE I COME FROM, THIS IS ROBBERY.     BOUGHT A USED 2016 4RUNNER. HAD IT 3 WEEKS AND A RODENT CHEWED THE WIRES ON THE MAIN WIRING HARNESS. TOYOTA (THAT I HAVE 2 WARRANTIES THROUGH) DOES NOT COVER RODENT DAMAGE EVEN THOUGH THE MATERIAL THAT THEY USE ATTRACTS RODENTS APPARENTLY FROM MY RESEARCH. I HAVE $3500-$5500 IN DAMAGE AND MY CAR HAS BEEN AT THE DEALERSHIP WAITING FOR REPAIR FOR 3 WEEKS NOW. THE INSURANCE COMPANY IS FIGHTING THEM ON THE REPAIR AND I AM STUCK WITHOUT THE VEHICLE. I DO NOT HAVE THAT KIND OF MONEY TO REPAIR IT MYSELF. TOYOTA SHOULD BE RESPONSIBLE FOR REPAIRING AND REPLACING THE WIRES THAT THEY USED SINCE IT IS ENCOURAGING THE DAMAGE!     MY VEHICLE WAS PARKED IN FRONT OF MY HOME AND WHEN I WENT OUT IN THE MORNING TO TAKE MY CHILDREN TO SCHOOL MY VEHICLE STARTED MISFIRING ALL THE CHECKS ENGINE LIGHTS WERE LIGHTED UP EVERY WARNING LIGHT ON MY PANEL LIT UP. I HAD MY VEHICLE TOWED BACK TO THE DEALER. COME TO FIND OUT THAT ALL MY WIRING UNDER THE HOOD HAD BEEN EATEN OR CHEWED UP BY RATS. IT COST ME $500.00 DOLLARS TO HAVE IT FIXED (TAPED) LESS THAN 6 MONTHS LATER IT HAPPENED AGAIN . AND LESS THAN 3 MONTHS AFTER THAT IT HAPPENED AGAIN! I READ SOMEWHERE THAT YOUR VEHICLES ARE KNOWN TO ATTRACT RATS AND THAT I'M NOT THE ONLY CUSTOMER TO HAVE HAD THIS PROBLEM. IS THERE ANY WAY YOU GUYS ARE PLANNING TO FIX THIS PROBLEM ANY TIME SOON ?     THROTTLE CONTROL SYSTEM FAILURE ON 2016 TOYOTA 4RUNNER. WHILE DRIVING THE VEHICLE THE ORANGE SERVICE ENGINE AMD TRAC ORANGE LOGHT ILLUMINATED. ONCE THAT HAPPEN D THE THROTTLE BODY BECAME TOTALLY NON-RESPONSIVE. NO MATTER HOW MUCH I PRESSED THE THROTTLE IT WAS NON-RESPONSIBILVE. I WAS ABLE TO SAFELY PULL OFF THE ROAD. I HAD TO RESTART THE VEHICLE TWICE, ON THE SECOND RESTART BUT THE WARNING LIGHTS REMAINED ON BUT THROTTLE APPEARED TO BE FUNCTIONING NORMAL. THE DEALERSHIP WAS ABLE TO REPLICATE THE ISSUE AND ADVISED THE VEHICLE WAS NOT SAFE TO DRIVE AND PROVIDED A LOANER. THE SERVICE TECH AND SERVICE ADVISOR EXPLAINED HOW RARE THIS IS ESPECIALLY ON A 12 MONTH OLD VEHICLE WITH UNDER 30K MILES ON IT. THEY ORDERED A NEW THROTTLE BODY AND WE'LL SEE IF IT FIXES THE ISSUE. GIVEN TOYOTA'S RECENT HISTORY WITH 'SUDDEN ACCELLERATION' I THOUGHT I SHOULD REPORT THIS JUST IN CASE.

    I HIT A CAR THAT PULLED OUT IN FRONT OF ME ON THE HIGHWAY. I WAS GOING NORTH BOUND AND THEY ATTEMPTED TO TAKE A LEFT FROM A SIDE STREET TO GO SOUTH BOUND. I WAS GOING 45 MILES PER HOUR AND THE OTHER CAR WAS AT A DEAD STOP IN FRONT OF ME. I HIT THEM JUST BEHIND THE DRIVERS SEAT. MY VEHICLE IS NOT DRIVEABLE, BUT YET THE AIRBAGS DID NOT DEPLOY.

NOW THEY SAID I NEEDED NEW BRAKES AND THEN THE ABS, BRAKE, AND SKID CONTROL LIGHTS KEEP COMING ON AND OFF WHICH COINCIDES WITH A COIL/CLICK FEELING UNDER THE BRAKE AND GAS PEDAL. CAR STILL SHAKES AND SQUEAKED. TOYOTA KEEPS SAYING MY CAR IS FINE.

    I have a 2016 Toyota 4Runner at 143,011 miles. When it hit 125,000 mile mark the backup camera stopped working intermittently. The screen in the car goes blank. Turning the car on/off or changing gears does not affect this issue. It’s random and seems to becoming more frequent as more miles are put on. I will take it in however when I called them they said since it’s not a known issue I will have to replace it and it’s not cheap!

    THE WIRING INSIDE OF THE ENGINE WAS CHEWED THROUGH BY RODENTS. I DRIVE THE VEHICLE EVERY DAY AND IT WAS SERVICED TWO DAYS BEFORE. AFTER PERFORMING SOME RESEARCH, I FOUND OUT THAT TOYOTA AND OTHER COMPANIES USE SOY BASED PRODUCTS TO MAKE WIRING. THIS APPARENTLY ATTRACTS RODENTS AS IT IS BASICALLY FOOD FOR THEM.THE VEHICLE WAS PARKED IN FRONT OF MY HOME IN A MARKED PARKING SPOT. I HAVE PARKED OTHER (OLDER) VEHICLES IN FRONT OF MY HOME FOR APPROXIMATELY 10 YEARS WITHOUT THIS PROBLEM. I HAVE ONLY OWNED MY TOYOTA FOR ONE YEAR AND THE WIRING WAS DESTROYED BY RODENTS TO THE TUNE OF NEEDING A NEW WIRING HARNESS.

    It is a common complaint on the 4runner forums, the low beam head lights don't generate enough light to see at night. One video i saw measured the out put at less then 250lumns. The average halogen bulb put out 700lumns of light. I am constantly having to use high beams to see down the road. Consumers are forced to purchase and install after market head lights. It posses a safety risk not only to the drive but to anything on the road. I had my 2016 4runner a month before the need to drive at night arose, and I was shocked at how poor the illumination was.

    THE 2016 4RUNNER TRD PRO IS A LIMITED PRODUCTION MODEL. TOYOTA SOURCE THE SUSPENSION COMPONENTS FROM AN OUTSIDE VENDOR. MY RIGHT FRONT SPRING FAILED, BREAKING I FOUR PLACES. THE CAR HAS 41,000 MILES AND HAS ONLY BEEN USED IN NORMAL HIGHWAY DRIVING -- NO OFF-ROAD. THE FRONT SPRING DESIGN IS DEFICIENT. THERE IS NO TAPER AT EITHER END OF THE COIL SPRING, LEAVING A KNIFE EDGE TOUCH THE ADJACENT COIL -- RIGHT WHERE THE SPRING BROKE. THERE ARE NUMEROUS EXAMPLES OF THE SAME PROBLEM ON THE INTERNET. THE PART WAS BACK ORDERED REQUIRING MORE THAN A WEEK TO GET IT, SUGGESTING MANY OTHERS HAVE HAD THE SAME PROBLEM. THE LEFT SPRING WILL LIKELY FAIL, AND THE RIGHT WILL PROBABLY FAIL AGAIN. THIS IS A SAFETY ISSUE AFFECTING STABILITY.

    2016 TOYOTA 4RUNNER WITH LESS THAN 5000 MILES EVERY ONCE IN A WHILE I HAVE NOTICED A LACK OF RESPONSE TO THROTTLE WHEN MAKING A RIGHT HAND TURN. BY THIS I MEAN NO INCREASE IN ENGINE RPM'S PROPORTIONAL TO THROTTLE POSITION. I HAVE SHRUGGED IT OFF AS MY IMAGINATION. NOW I HAVE HAD TWO OCCASIONS WHERE AFTER COMING TO A STOP AT A STOP SIGN ON A STEEP INCLINE WHERE WHEN I RELEASED THE BRAKE PEDAL AND STEPPED ON GAS THE VEHICLE ROLLED BACK EVEN THOUGH I HAD AT LEAST 1/4 THROTTLE APPLIED. The contact received notification of NHTSA Campaign Number: 23E051000 (Suspension) however, the contact stated that after calling and leaving messages with the manufacturer regarding the recall, the contact had not received a call back. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. Parts distribution disconnect.

THE DEALER DIAGNOSED THAT THE INNER CONSTANT VELOCITY BOOT LEAKED OIL AND NEEDED TO BE REPLACED. THERE WAS NO WARNING INDICATOR ILLUMINATED. THE CONTACT WAS INFORMED THAT THE PART WAS NOT COVERED UNDER WARRANTY. THE MANUFACTURER WAS MADE AWARE OF THE FAILURE AND STATED THAT THE PART WAS NOT COVERED BY THE WARRANTY. THE VEHICLE WAS NOT REPAIRED. THE FAILURE MILEAGE WAS 18,445.

    SUNROOF "EXPLODED" WHILE DRIVING AT LOW SPEED ON A DESOLATE ROAD. CAR WAS PARKED AT A BEACH RENTAL FOR 3 DAYS IN THE HOT SUN AND THIS WAS THE FIRST TRIP, HEARD A POPPING NOISE. SINCE THERE WERE NO VEHICLES WITHIN AT LEAST 1/2 MILE AND NO BUILDINGS WITHIN 50 YARDS DID NOT THINK THIS NOISE WAS CAUSED BY A ROAD HAZARD. LOOKED OVER THE CAR AND COULD NOT FIGURE OUT WHAT THE NOISE WAS. WHEN WE ARRIVED BACK AT THE HOUSE AND WERE SITTING ON THE DECK WE NOTICED THE SUNROOF LOOKED STRANGE, A BUBBLE IN THE CENTER . FURTHER INSPECTION FOUND THE SUNROOF SHATTERED, STILL IN PLACE. LUCKY WE HAD THE COVER DRAWN CLOSED BUT EVEN LUCKIER IT STAYED INTACT.

    THE VEHICLE HAS HAD A MULTITUDE OF ISSUES. I AM ON MY 3RD DIFFERENT SET OF TIRES THIS YEAR (INCLUDING THE ORIGINAL TIRES). NO MATTER HOW MANY TIMES THE TIRES ARE BALANCED, THE STEERING WHEEL SHAKES RATHER VIOLENTLY AT CERTAIN SPEEDS. THE TIRES HAVE BEEN BALANCED AND RE-BALANCED (INCLUDING ROAD-FORCE BALANCED, ETC.). ON-LINE FORUMS INDICATE THAT MANY OTHER TOYOTA OWNERS ARE EXPERIENCING THE SAME ISSUE. ALSO THIS WEEK I HAVE LOST THE ABILITY TO STEER THE VEHICLE (THE STEERING WHEEL IS LOCKING AND BINDING UP). APPROXIMATELY 2 WEEKS AGO THE RIGHT FRONT CALIPER FROZE UP AND THE VEHICLE NEARLY WENT OFF THE ROAD. ALSO THE REAR DIFFERENTIAL WAS NOISY FROM THE DAY THE VEHICLE WAS NEW. TOYOTA EVENTUALLY REPLACED THE REAR DIFFERENTIAL (I BELIEVE A BAD PINION GEAR). SHORTLY AFTER THE REAR DIFFERENTIAL WAS REPLACED THE VEHICLE STARTED TO GO IN AND OUT OF 4-WHEEL DRIVE ON ITS OWN, AND WARNING LIGHTS AND CHIMES STARTED GOING OFF. THE DEALER THEN MADE REPAIRS BUT THIS HAS HAPPENED AT LEAST ONCE OR TWICE SINCE THEN AND THE DEALER HAS REPAIRED IT EACH TIME. ALSO IN THE LAST YEAR THE TRANSMISSION STOPPED WORKING LEAVING ME STRANDED IN THE MIDDLE OF A 4-LANE ROAD. I EVENTUALLY WAS ABLE TO GET THE VEHICLE TO MOVE BUT THEN IT STARTED TO GO IN AND OUT OF 4-WHEEL DRIVE ON IT'S OWN AGAIN WITH WARNINGS AND CHIMES GOING OFF. WHENEVER THE VEHICLE STARTS TO GO IN AND OUT OF 4-WHEEL DRIVE ON ITS OWN THE SHIFTER LEVER USUALLY STARTS TO FREEZE UP AS WELL (AND YOU MAY NEED TO USE BOTH HANDS TO FORCE THE VEHICLE INTO GEAR). THE VEHICLE HAS ALSO HAD A STRONG ODOR OF ANTIFREEZE SINCE NEW COMING FROM THE RIGHT FRONT CORNER. THE DEALER HAS ADVISED THEY CANNOT LOCATE THE CAUSE. ALSO VARIOUS HEAT SHIELDS HAVE EITHER BROKEN OR FALLEN OFF SINCE THE VEHICLE WAS NEW. PLEASE NOTE THAT I DO NOT GO 4-WHEELING WITH THE VEHICLE AND IT'S METICULOUSLY MAINTAINED INCLUDING ALL SERVICE RECORDS AND RECEIPTS.

    Toyota has a major safety issue (that they know about) relating to door lock actuators on their 4runners. Both of mine (passenger and driver side) failed within the first 55,000 miles, and there are a LOT of others who have had the same problem. The forums are littered with people reporting. However, Toyota has discouraged people from reporting the problem to them by charging an exorbitant amount for repair of such an inexpensive item (about $800 per side). When that becomes known to owners, people will just repair the items themselves. This is obviously unethical. Door lock actuators pose an obvious safety issue, both if the door remains unlocked when it is supposed to be locked, and when the door remains locked when it is supposed to be unlocked. In the latter case, following an accident where a passenger is incapacitated and the door lock cannot be reached by hand (and there's no external key slot), the only way to unlock the door is via the electronic fob. If the door lock actuator is broken, then that incapacitated passenger will be trapped in the vehicle. For the former case - and this has personally happened to me - when the door was supposed to be locked, I had an individual actually open my passenger side door and enter into my vehicle. (He was under the influence of something, and fortunately nothing happened other than a very tense situation). But the safety issue remains. I expect that the post-accident scenario is more relevant to the NHTSA's interests, however, since a KNOWN design defect could very easily lead to the death of an individual trapped in a vehicle.

    Swivel motion has started to jam when half locked, base will show half red/half green and refuse to lock. Unlock handle becomes hard to lift to swivel the car seat back to attempt to lock it again. I’ve contacted Evenflo, but waiting to hear back.

    The entire exterior body clear coat is peeling off. This started about 1.5 years ago on the tailgate. It has now spread to the hood and the top of the vehicle. I called Toyota customer care, and they indicated this a known widespread problem with this vehicle make and model. So far, only the pearl white color has been recalled as the result of numerous consumer complaints. My vehicle color is nautical blue- Toyota indicated there has not been enough consumer complaints yet for this color for it to be recalled. Toyota suggested I log a complaint here in hopes it will result in a recall.

    THE DAY AFTER WE PURCHASED THIS 4RUNNER IN OCTOBER 2015 THE 4 WHEEL DRIVE STOPPED WORKING AND WHEN WE TOOK IT BACK TO THE DEALER THEY SAID THAT RODENTS HAD CHEWED THROUGH THE WIRE HOUSING CONNECTED TO THE DRIVE TRAIN. INITIALLY THEY WERE NOT GOING TO WARRANTY THE ISSUE BUT AFTER FURTHER AGGRESSIVE DISCUSSION BY ME IT WAS COVERED. APPARENTLY TOYOTA USES A SOY BASED SPRAY OR COATING ON THE WIRING THAT ATTRACTS RODENTS. THE VEHICLE WAS OPERABLE BUT THE 4WD WARNING LIGHT WAS FLASHING AND 4WD DID NOT ENGAGE. WE HAVE ALSO HEARD FROM A NEIGHBOR WITH A 2015 4RUNNER THAT A SIMILAR SITUATION HAPPENED TO THEM BUT IT AFFECTED THEIR WHOLE ELECTRICAL SYSTEM AND THAT THE VEHICLE WAS NOT OPERABLE AND HAD TO BE TOWED TO THE DEALER.

AT 12,000 AND 30,000 MILES, THE WIRES WERE PREMATURELY DAMAGED BY RODENTS. THE MANUFACTURER PROVIDED NO SOLUTION OTHER THAN SUGGESTING THAT THE CONTACT REPLACE THE WIRES TWICE. THE DEALER (DURANGO TOYOTA, 1200 CARBON JCT, DURANGO, CO 81301, (970) 385-4822) INSPECTED THE VEHICLE. THE MANUFACTURER WAS NOTIFIED OF THE FAILURE. THE VIN AND FAILURE MILEAGE WERE NOT AVAILABLE.

    SINCE NEW, I HAVE DOCUMENTED AT LEAST 20 OCCURRENCES OF LACK OF ACCELERATION, SEVERAL TIMES ALMOST RESULTING IN AN ACCIDENT. ALL OCCURRENCES HAPPEN FROM A STOPPED POSITION, THEN APPLYING AT LEAST HALF ACCELERATOR PEDAL, THE TRUCK WILL INITIALLY ACCELERATE, THEN PAUSE FOR SEVERAL SECONDS, VERY SCARY WHEN CROSSING TRAFFIC OR GOING THROUGH AN INTERSECTION. HAPPENS ON TURNS AND WHEN GOING STRAIGHT.

    This vehicle transmission is demonstrating transmission shuddering problems, We have returned this car three time for the same repair. The highly reputable transmission has cited direct purchases of this part from Toyota has constantly have quality issues with the torque converter, This problem was inspected and confirmed by dealer and independent service center. They both concluded the same diagnose, There were no warning messages. We fear travelling on interstate roads is hazardous.

    Wiring harness to rear window defogger melted causing the rear window to crack and shatter. Safe lite replaced the window and recommended the harness not be reconnected. Told me it was not uncommon. Vehicle was not being used at the time. It was in my driveway when the window blew outward. Inspected by Safe lite, Reported to Liberty Mutual. No warning light, rear defogger was not in use, car was off.

    TOYOTA USES ENGINE WIRING THAT HAS A SOY BASED COATING. THIS ENCOURAGES RODENTS TO CHEW ON THEM. WE WERE CAMPED (PARKED) AND WHEN WE TRIED TO DRIVE THE CAR IT SHUDDERED AND THE CHECK ENGINE LIGHT WAS ON ALONG WITH SEVERAL OTHER INDICATORS. THE CAR WAS RUNNING ON FIVE CYLINDERS. WE MANAGED TO GET IT TO A TOYOTA DEALER WHO TOLD US OF BITE MARKS AND DROPPINGS. IN RESEARCHING THIS ON LINE WE FOUND REPORTS OF THIS HAPPENING IN TOYOTAS AND HONDAS. WE COULD HAVE BEEN STRANDED IN THE DESERT IN 110 DEGREE HEAT IF THE DAMAGE WAS ENOUGH TO DISABLE THE ENGINE COMPLETELY.

    TL* THE CONTACT OWNS A 2016 TOYOTA 4RUNNER. WHILE THE VEHICLE WAS STATIONARY, THE INTERIOR OF THE VEHICLE BECAME COMPLETELY RUSTED. THE CONTACT FOUND SEVER RUST UNDERNEATH THE SEATS, THE STEERING WHEEL, AND THE SEAT BELT LATCHES. THE SEAT BELTS DID NOT LATCH DUE TO THE SEVERE RUST ON MORE THAN ONE OCCASION. IN ADDITION, UNDERNEATH THE SEAT CUSHION OF THE THIRD ROW SEATS, THE ENTIRE SEATING FRAMEBOARD WAS RUSTED SEVERELY. THE RUST HAD AN UNPLEASANT ODOR. READOR-DYKES TOYOTA OF PLAINVIEW, TEXAS WAS CONTACTED ABOUT THE ISSUE AND STATED THAT THERE WAS NOTHING WRONG WITH THE VEHICLE, AND THE CONTACT COULD CONTINUE TO BE DRIVEN. THE MANUFACTURER WAS MADE AWARE OF THE ISSUE AND STATED THAT THERE WAS NO WARRANTY COVERAGE ON THE VEHICLE AND THAT IT WAS SOLD "AS IS". THE FAILURE MILEAGE WAS 25,000.

    THE DOOR LOCK ACTUATOR ON THE PASSENGER SIDE DOOR IS GOING OUT. THIS IS CLEARLY A COMMON PROBLEM WITH THIS MAKE AND YEAR MODEL. SEVERAL REVIEWS I HAVE READ STATE THAT THIS HAS BEEN AN ISSUE SPECIFICALLY WITH THE FRONT DRIVER AND PASSENGER DOORS. I'M NOT SURE WHY THERE HAS NOT BEEN A RECALL ON THIS ISSUE. THIS IS OBVIOUSLY A MANUFACTURER ISSUE VS A OWNER/USER ISSUE. TOYOTA REALLY NEEDS TO CONSIDER A RECALL BECAUSE THIS IS NOT A CHEAP FIX.

    - The Roll Over Side Curtain Airbags deployed and hit my head. Both sides deployed. - I was in 4Wheel Low Range. Driving with a group of trucks. The obstacle was a set of mud ruts. - All witnesses did not understand, nor have seen this happen. - Toyota sent me a report based on their review of the vehicle. They said the Roll Over sensors detected a roll-over event. They advised that it is a "predictive" system. That at some point in my driving through the mud ruts, the system thought I was rolling over... I was not anywhere near a roll-over. - I requested to know why the RSCAS button was removed. Toyota advised that it was governed by NHTSA to be removed from models produced after a certain year. - I do not want to have my vehicle repaired to "factory specifications" given the experience I had. Toyota could not negotiate that request on my behalf. This is my request to NHTSA.

    We were traveling down the mountain of Mt. Nebo in Dardanelle, AR, around 1:08pm on Friday, May 22, 2026. It had been misty rain for a few hours. As we were traveling down the switchbacks of the mountain, we pressed on the brake to slow the vehicle; however, the vehicle did not slow down any. Since we were coming to the next curve in the switchback, the decision was made to drive the car into a tree vs. risking flipping the car over the side of the mountain. When the vehicle collided with the tree, none of the air bags deployed. The point of impact was the front driver corner of the car. The car was traveling around 20 MPH at the time of impact (to the best of our knowledge). Problem #1-brake system failure Problem #2-Air bags failed to deploy -What component or system failed or malfunctioned, and is it available for inspection upon request? The brake system & air bags. Both are available for inspection. -How was your safety or the safety of others put at risk? Risk of death associated with the crash. -Has the problem been reproduced or confirmed by a dealer or independent service center? Unavailable at this time due to the extensive damage caused from the crash. -Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? Not to my knowledge since this happened a few hours before a holiday weekend. -Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? None that we seen.

    I WAS GOING STRAIGHT DOWN A HIGHWAY WHEN MY SUNROOF SUDDENLY EXPLODED AND SHATTERED. I DID NOT SEE ANY DEBRIS HIT IT. IT WAS EARLY MORNING AND THERE WERE FEW CARS ON THE ROAD AND I WAS NOT IN A CONSTRUCTION ZONE. THERE WAS NO EVIDENCE OF IMPACT TO THE SUNROOF OR BODY/ROOF OF THE VEHICLE. THE SUNROOF WAS BULGING OUTWARD, AS IF SOMEONE PUNCHED IT FROM THE INSIDE BUT I WAS THE ONLY PERSON IN THE VEHICLE AND HAD THE SUNROOF SHADE CLOSED. AFTER INVESTIGATING, THERE HAVE BEEN INCIDENTS JUST LIKE THIS HAPPENING WITH OTHER PEOPLE'S SUNROOF AS WELL, SO THIS WAS NOT JUST A ONE RANDOM OCCURRENCE. THERE NEEDS TO BE HIGHER SAFETY STANDARDS ESTABLISHED FOR SUNROOFS IN THE FUTURE. LUCKILY, IN MY SUDDEN SHOCK AND PANIC, I DID NOT CAUSE AN ACCIDENT TRYING TO GET OFF THE ROAD WHILE PIECES OF GLASS WERE FLYING OFF AND INTO THE VEHICLE AFTER I OPENED THE SHADE TO SEE WHAT HAPPENED.
