    Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2013-2015 Prius and 2014-2017 Prius V vehicles not included in recall 18V-684. Excessive voltage in the Intelligent Power Module (IPM) within the inverter may cause the hybrid system to shut down, causing the vehicle to stall while being driven.

    Risk: An unexpected hybrid power system shut down may increase the risk of a crash.

    Fix: Toyota will notify owners, and dealers will perform a software update for the hybrid system. If the vehicle has experienced an inverter failure with certain hybrid system faults related to this condition, the inverter assembly will be repaired or replaced, prior to software update. All repairs will be performed free of charge. This recall began July 24, 2020. Owners may contact Toyota customer service at 1-888-270-9371. Toyota's number for this recall is 20TB10 / 20TA10.

    Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain model year 2014-2015 Toyota Prius V vehicles manufactured November 7, 2013, to January 7, 2015. The occupant classification system (OCS) that activates or deactivates the passenger seat air bag system may be improperly calibrated. As a result, the passenger air bag may not deploy in a vehicle crash. As such, these vehicles fail to comply with Federal Motor Vehicle Safety Standard (FMVSS) No. 208, "Occupant Crash Protection."

    Risk: If the air bag does not deploy as intended, there is an increased risk of injury in the event of a crash.

    Fix: Toyota will notify owners, and dealers will re-calibrate of the OCS system, free of charge. The recall began February 12, 2015. Owners may contact Toyota customer service at 1-800-331-4331.

  • HYBRID PROPULSION SYSTEM: INVERTERCampaign 15V449000 · 15/07/2015

    Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain model year 2012-2014 Prius V vehicles manufactured August 22, 2011, to June 30, 2014. A component within the hybrid inverter assembly may overheat, causing the hybrid system to reduce output power allowing the vehicle to only drive a short distance. In some circumstances, the hybrid system may shut down causing the vehicle to stop while being driven.

    Risk: The vehicle may enter a fail-safe/limp-home mode that limits the drivability of the vehicle. The hybrid system could also shut down completely resulting in a vehicle stall, increasing the risk of a crash.

    Fix: Toyota will notify owners, and dealers will update the software for the motor/generator control electronic control unti (ECU) and the hybrid control ECU, free of charge. The recall began on July 30, 2015. Owners may contact Toyota customer service at 1-888-270-9371.

Excessive voltage in the Intelligent Power Module (IPM) within the inverter may cause the hybrid system to shut down, causing the vehicle to stall while being driven.

    Risk: If the hybrid system shuts down unexpectedly, it can increase the risk of a crash.

    Fix: Toyota will notify owners, and dealers will update the software for the motor/generator control electronic control unit (ECU) and the hybrid control ECU as necessary, free of charge. The recall began January 22, 2019. Owners may contact Toyota customer service at 1-888-270-9371. Toyota's number for this recall is J0V. Note: The software updates in this recall are needed even if the vehicle had the software previously updated under recalls 14V-053 or 15V-449.

    The Head Gasket failed on my vehicle, and began to burn coolant and overheat my engine. Upon description of the issue, a dealership mechanic was able to diagnose the issue. The sudden failure of my vehicle due to overheating while driving was extremely risky, and I had no indication that the issue was a head gasket until a dealership mechanic was able to look at my vehicle after being towed. I had taken my vehicle to a third-party mechanic, and they could not tell me why my vehicle was burning coolant, but provided a temporary Stop-Leak fix that did not hold. The first appearance of the Check Engine light was about 9 months ago, was assumed to be the spark plugs, which I had replaced, but then my coolant light started coming on with the check engine light for about a week before the engine overheat incident. The vehicle has been inspected by a Toyota Hybird Mechanic. No other inspections have been done. My safety and other's safety was put at-risk as my steering, speed control, and speed monitoring systems immediately shut off once the system detected an overheat, making it difficult to pull over safely and slowly to address the issue/call a tow truck. I required physical assistance from another motorist to move my vehicle off the road to allow others to pass.

    Passenger airbag indicator on/off light was on despite having passenger over the airbag activation weight requirement being present with seatbelt buckled. Brought car to Toyota dealership and after running diagnostics for two hours they found that the airbag itself is faulty.

    TL* THE CONTACT OWNS A 2014 TOYOTA PRIUS. WITHOUT WARNING, THE FRONT PASSENGER SIDE GLASS WINDOW FRACTURED. THE VEHICLE WAS NOT DIAGNOSED BY A DEALER OR AN INDEPENDENT MECHANIC. THE CONTACT REPLACED THE PART DUE TO THE FAILURE. THE MANUFACTURER WAS NOTIFIED AND STATED THAT THIS WAS NOT A KNOWN ISSUE. THE VEHICLE WAS REPAIRED. THE FAILURE MILEAGE WAS NOT AVAILABLE.

    MY 2014 PRIUS HAD A LOSS OF POWER AND CHECK ENGINE LIGHT CAME ON AND MSG CHECK HYBRID SYSTEM APPEARED WHILE STARTED IN THE MORNING AND DROVE OUT OF MY GARAGE. THAT DAY I WENT TO SAN FRANCISCO TOYOTA AND THEY DID A DIAGNOSIS AND TOLD ME THAT MY HYBRID SYSTEM BATTERY WAS BAD AND NEED REPLACEMENT AND QUOTED $3600. I ASKED THAT IF THEY CAN DO THE RECALL BECAUSE I WAS 90% SURE THAT THE PROBLEM WAS DUE TO THE RECALL, BUT THEY TOLD ME THAT THEY CANT UNLESS THE ERROR CODE CLEARS OUT BY REPLACING THE HV BATTERY. I DROVE MY CAR BACK HOME AND THE NEXT DAY I WENT TO A HV BATTERY SPECIALIST AND FOUND OUT THAT MY HV BATTERY IS IN GOOD CONDITION AND DOESNT NEED REPLACEMENT. NOW I AM 100% CERTAIN THAT THE PROBLEM IS DUE TO THE RECALL BUT TOYOTA IS NOT FIXING THE ISSUE THEY ARE SAYING ITS THE HV BATTERY WHICH IS NOT. THE TOYOTA OF S.F NOT ONLY DIAGNOSED MY CAR WRONG THEY TRIED TO RIP ME OFF BY SPENDING $3600 ON A HV BATTERY THAT DOESNT NEED ANY REPLACEMENT! ON TOP OF THAT THEY ARE REFUSING TO DO THE RECALL.

    I PURCHASED THE VEHICLE BRAND NEW FROM A DEALERSHIP. WITHIN A FEW WEEKS, I NOTICED THAT WHEN MY HUSBAND SAT IN THE FRONT PASSENGER SEAT (300LBS) THE NOTIFICATION LIGHT SAID "PASSENGER AIR BAG ON". WHEN MY STEPSON SAT IN THE FRONT PASSENGER SEAT THE LIGHT SAID "PASSENGER AIR BAG OFF". I TOOK A PICTURE TO SHOW THE NOTIFICATION LIGHT AND THE SIZE OF MY 15 YR OLD STEPSON 5' 10" & 175LB-FULLY ADULT SIZE. I CALLED THE DEALERSHIP AND THEY TOLD ME THEY HAD "NEVER HEARD OF THE PROBLEM" AND SAID IT WAS JUST THAT THE PASSENGER AIRBAG NEEDED TO BE RECALIBRATED. AFTER DOING SOME SEARCHING, I REALIZED THAT THE 2012-2013 MODELS HAVE ALREADY BEEN RECALLED FOR THAT EXACT SAME REASON. I AM VERY CONCERNED THAT THE PROBLEM HAS NOT BEEN FIXED WITH TOYOTA, AND THAT NEW CARS ARE BEING SOLD WITH THE SAME PROBLEM. NEEDLESS TO SAY, I AM NOT HAPPY WITH TOYOTA RIGHT NOW AND THIS IS A HUGE SAFETY PROBLEM.     the brakes have not failed yet, but there is a known issue with the brake booster on my model year. I would like to have the government force Toyota to guarantee the brakes for a longer period of time. currently Toyota has extended the warranty out 10 years / 150k miles from date of first use. Since these cars last so long, this should be 20 years / 300k miles from date of first use.

    The ABS and traction control system in my 2014 Toyota Prius 4 are not functioning. They ABS light an the traction control lights are both on. I work in the automotive industry and I used a code reader to pull the codes and got a C1391 which is a brake booster and brake booster pump. That means either the brake booster has failed or the master cylinder is leaking. I contacted Gosche Toyota in Hemet and after waiting for a service writer to contact me (which they never did) I called 5 times till I finally got someone on the phone. They were very familiar with the problem. The problem is so common that SOME vehicles have been recalled. But for others like mine only a "Customer Support Program" was made. That program covers repair "if the parts are available" only for 10 years or 150,000 miles whichever comes first. But for someone like myself who has 170,000 miles Toyota says there's nothing they can do. The cost for me to repair it is $4800. The customer support program bulletin "ZJB" even says that service writers are not allowed to tell customers about the problem or the customer support program. That way when the car doesn't meet the requirements, they won't be responsible for the repairs, even though they know it will eventually fail. There is a lawsuit somewhere regarding this very same problem but for me, I'm screwed. I depend on my car to make money and now me and my family are out of a car. This is a known defect and a safety concern. Toyota is okay with leaving us with nothing.

    MY AIRBAGS DEPLOYED PRIOR TO IMPACT AT A SPEED OF LESS THAN 25 MPH. I APPLIED THE BRAKES VERY FORCEFULLY AND THE AIRBAGS DEPLOYED PRIOR TO IMPACT. I HAD LITTLE FRONT END DAMAGE AND EVERY SINGLE AIRBAG DEPLOYED.

    UNKNOWN. I WAS BASICALLY DRIVING ON THE FREEWAY, WHEN MY CAR'S ENGINE SUDDENLY STOPPED RUNNING. THE CAR WAS AUTOMATICALLY PUT INTO NEUTRAL AND WAS NOT ABLE TO CONTROL THE SPEED OF THE CAR, SO I HAD TO MOVE TO THE SIDE OF THE FREEWAY. AS I WAS SITTING IN THE CAR, I STARTED SMELLING BURNED WIRE AND INTERIOR STARTED TO FILL WITH SMOKE. I IMMEDIATELY TURNED THE CAR OFF AND GOT OIT. AS I WAS STANDING FAR AWAY FROM THE VEHICLE, FLUID STARTED LEAKING AND IT HAD CAUGHT INTO FIRE. THE FRONT BUMPER WAS COMPLETELY BURNED OFF, AS WELL AS SOME OF THE ENGINE AND ELECTRICAL PARTS WERE AFFECTED AS WELL.

    I started noticing the ghosting or tripling of light issue in my windshield. I mostly noticed this during night times, I initially thought I was just tired and my eyes were tired, but I’ve been able to photograph it and verify my experience with other people. This effect is horrible for my eyesight and makes all lights at night so blurry I cannot drive safely.

    WHILE I WAS DOING A SLOW TURN IN A DRIVEWAY, MY BRAKE FAILED AND THE CAR CONTINUED TO ACCELERATE IN A FORWARD MOTION. WHEN I SHIFTED INTO REVERSE, THE CAR REVVED AND ROARED ACCELERATING AT FULL TILT. IT RACED ACROSS A ROAD INTO ANOTHER DRIVEWAY ONLY STOPPING WHEN WEDGED BETWEEN A GARAGE DOOR AND A TREE. TOYOTA TOOK THE TOTALED CAR AND HAD IT INSPECTED BY ENGINEERING ASSOCIATES AND INSURANCE AUTO AUCTIONS. THEIR RESPONSE IS THAT "THERE WAS NO SERIOUS DEFECTS IDENTIFIED." OBVIOUSLY, THE MOMENTARY MECHANICAL OR ELECTRONIC FAILURE OF MY CAR COULD NEVER BE DETECTED, SO ACCORDING TO TOYOTA, THERE WAS NOTHING TO CAUSE THE FAILURE. I FEEL TOTALLY VICTIMIZED AND BELIEVE TOYOTA SHOULD TAKE RESPONSIBILITY AND COMPENSATE ME FOR THE LOSS OF MY CAR.

    WE PURCHASED A NEW TOYOTA PRIUS THAT CAME WITH YOKOHAMA ALL SEASON RADIALS. BECAUSE THE TIRE LIGHT CAME ON, I CHECKED THE AIR PRESSURE IN THE TIRES AND SET THEM TO THE PSI AS RECOMMENDED AT ABOUT 1000 MILES. THERE WAS A MINOR IMBALANCE OF 3-4 PSI IN SOME TIRES, BUT NO SERIOUS UNDER INFLATION. WE NOTICED SOME MINOR ROAD NOISE AND VIBRATION. WE HAD A MECHANIC INSTALL MUD FLAPS AND INSPECT THE TIRES, WHEELS, AND BRAKES AT ABOUT 1500 MILES TO SEE IF THERE WAS A VISIBLE REASON FOR VIBRATION. THERE WAS NOTHING OBVIOUS. WE WERE CONCERNED ABOUT THE ROAD NOISE AND ABOUT 2000 MILES WE TOOK THE CAR BACK TO THE TOYOTA DEALER WHO INSPECTED THE TIRES, CHECKED THE AIR PRESSURE, AND DECLARED THAT THEY DIDN'T SEE ANYTHING WRONG. AT ABOUT 3000 MILES DRIVING AT A SLOW SPEED (35 MPH OR LESS) IN A SCHOOL ZONE, WE HAD A COMPLETE BLOW OUT OF THE LEFT REAR TIRE. FORTUNATELY, THERE WAS A GRASSY SHOULDER AND NO TRAFFIC, SO WE HAD NO INJURIES OR OTHER DAMAGE. THE TREAD APPEARED TO SEPARATE AND THERE WAS A SIDEWALL TEAR STARTING AT THE TREAD. WE RETURNED TO THE TOYOTA DEALER AND ALSO SHOWED THE TIRE TO THREE DIFFERENT TIRE SHOPS (ALL THREE SELL YOKOHAMA TIRES). THE TOYOTA DEALER DID NOT OFFER AN OPINION OTHER THAN TIRE DEFECT - SO WE NEEDED TO DEAL WITH THE MANUFACTURER. TWO TIRE SHOPS THOUGHT IT WAS A DEFECT (EVEN THOUGH THEY SAID IT WAS A HASSLE TO GET A REPLACEMENT PAID FOR BY YOKOHAMA). ONE SHOP REPORTED IT WAS A DEFECT UNLESS THE TIRE WAS UNDER INFLATED, BUT THEY DIDN'T SEE ANY WEAR PATTERN THAT INDICATED UNDER INFLATION. BECAUSE OF THE BLOWOUT AND A SECOND SUSPICIOUS TIRE OF THE FOUR, WE REPLACED ALL FOUR WITH A DIFFERENT BRAND. AS FAR AS WE CAN TELL, THE YOKOHAMA TIRE(S) WERE DEFECTIVE AS NEW ON THE CAR. *TR

    Apparently there is a known defect on the brake actuator/master cylinder/booster on the 2014 Prius that would have been addressed if the car had been driven for less than 10 years or 150,000 miles. We purchased our car in April 2014 and the part just failed yesterday at only 126,000 miles. We understand that there is a pending class action suit (or perhaps one that was settled) but do not know how to become a member of it. It seems as if they knew that the part was defective, but did not offer to do a voluntary recall and remediate this before a failure, they should have, but instead, now we have a $3400 repair bill. Requesting assistance in evaluating whether there was negligence and or liability for them not addressing this issue in a timely manner.

    Brake Actuator Valve Wear all lights relating to brakes are on, Dec. 12th 2024. The warranty for this expensive fix ended March, 2024 or 150k miles, there are only 106k miles on the vehicle. Something that is such a safety hazard SHOULD not have a date. We are experiencing unexpected loss of brake power while driving, accompanied by multiple dashboard warning lights, so we are unable to drive to the Toyota Dealership to have it scanned for issues. Toyota has advised us NOT to drive the vehicle, and we live 40 min from the nearest dealership. We bought the car from a Toyota dealership used, and they should have fixed this before selling the vehicle. Complete negligence and a huge safety issue. We only ask that they warranty be extended for the entire year of Dec. so that we can get this fixed asap.

    AS I TRIED TO MAKE A RIGHT TURN AT A VERY BUSY INTERSECTION, I HEARD A STRANGE NOISE AND TOTALLY LOST CONTROL OF THE STEERING WHEELING. I TURNED THE STEERING WHEEL 360 DEGREES ALL THE WAY, AROUND AND AROUND, BUT THE CAR KEPT GOING STRAIGHT... AFTER A MINUTE OR SO, THE COORDINATION BETWEEN THE STEERING WHEEL AND THE WHEELS RE-ESTABLISHED AND I COULD MANAGE TO DRIVE TO A NEARBY PARKING LOT. THE SITUATION HAPPENED AGAIN IN THE PARKING LOT BEFORE IT CAME BACK TO NORMAL. WE TOOK THE CAR TO A LOCAL TOYOTA DEALERSHIP AND LEFT IT THERE FOR 2 DAYS. HOWEVER, TECHNICIANS SAID THEY COULD NOT DUPLICATION THE ISSUE MEANING THEY COULD NOT FIND ANY PROBLEM WITH IT. THEY TOLD US THAT IT IS PROBABLY THE ELECTRICITY PART! WE ARE VERY UNSATISFIED WITH THE ANSWER AND VERY CONCERNED ABOUT THE UNSAFE CONDITION IT MAY CAUSE.

    TOYOTA ISSUED SAFETY RECALLS E0E AND F0R FOR THE FAILURE OF THE HYBRID INVERTER. IF THE INVERTER HAD NOT FAILED YET, THE TOYOTA'S REMEDY IS A 'SOFTWARE RE-FLASH'. THE SOFTWARE REFLASH 'REMEDY' IS NOT SUFFICIENT. THERE HAVE BEEN AN EXCESSIVE NUMBER OF INVERTER FAILURES AFTER THE SOFTWARE RE-FLASH 'REMEDY' FOR THESE RECALLS HAS BEEN COMPLETED. THIS IS VERY DANGEROUS TO DRIVERS, AS IT LEADS TO UNINTENDED DECELERATION ' THE PRIUS SPONTANEOUSLY ENTERING 'LIMP-HOME MODE' OR EVEN A 'HYBRID SYSTEM SHUT DOWN'. THIS IS DANGEROUS AS IN RESULTS IN DRASTIC REDUCTIONS, IF NOT THE COMPLETE LOSS, OF POWER. THESE UNINTENDED DECELERATION OCCURRENCES PREDOMINANTLY HAPPEN WHILE THE VEHICLES ARE IN MOTION ' MANY TIMES AT HIGH SPEEDS. BETWEEN ONLY TWO DEALERSHIPS, WE HAVE DOCUMENTED APPROXIMATELY 100 INVERTER FAILURES AFTER THE E0E AND F0R SOFTWARE REFLASH 'REMEDY' HAS BEEN COMPLETED. DURING ONE INVERTER FAILURE, WHICH TOOK PLACE AFTER RECEIVING THE E0E SOFTWARE RE-FLASH, EXCESSIVE HEAT WAS GENERATED WITHIN THE INVERTER THAT MELTED STEEL BOLTS AND CREATED AN ELECTRICAL ARC THAT BLASTED TWO HOLES THROUGH THE ALUMINUM CASE OF THE INVERTER ASSEMBLY. THE DRIVER STATED IF SHE HAD EXPERIENCED THIS FAILURE WHILE ON THE FREEWAY, 'SHE WOULDN'T BE HERE ANYMORE.' THE VEHICLE WAS A 2012 MODEL YEAR AND AT THE TIME OF THE FAILURE IT HAD ONLY 31,222 MILES ON IT. INSTANCES OF UNINTENDED DECELERATION WILL CONTINUE TO GROW DAILY UNTIL TOYOTA IMPLEMENTS A SUFFICIENT REMEDY THAT WILL PREVENT THESE LOSSES OF POWER FROM OCCURRING.     Brake light and several other lights suddenly came on. The brakes became spongy. Taken to a repair place close enough for walking for diagnostic work. It showed that the brake booster needs to be replaced. There was a campaign for this problem, so Toyota knows that it is/was a problem. That campaign has apparently ended. This is a very expensive repair on a car that I expected to use the rest of my life. There was no warning by the dealership from which I purchased the car that this was a possible pending problem.

    THE HEADLIGHTS ON PRIUS ARE NOT BRIGHT ENOUGH TO SEE THE ROAD AHEAD OF THE CAR. THE ARE ON BUT SOMEWHAT DIM. OTHER DRIVERS CAN SEE THE LIGHTS, BUT THE LIGHTS ARE NOT BRIGHT ENOUGH TO ILLUMINATE THE ROAD IN FRONT OF THE CAR. THIS OCCURRED AFTER I TOOK THE CAR IN TO THE DEALER TO COMPLAIN ABOUT A SERIES OF ELECTRICAL RELATED PROBLEMS THAT ALL STARTED HAPPENING AT THE SAME TIME. THE DEALER REPLACED THE HEADLIGHTS AFTER THEY DETERMINED THE HEADLIGHTS HAD BURNED OUT. IMMEDIATELY, I NOTICED THAT DURING TIMES WHEN I WAS DRIVING AT NIGHT, THE NIGHT VISIBILITY WAS NOT WHAT IT HAD BEEN PRIOR TO PROBLEM FIRST OCCURRING. I HAVE TAKEN THE CAR BACK TO THE DEALER BU THEY SAY THERE IS NOTHING WRONG WITH THE HEADLIGHTS.

THE SAME PROBLEM HAPPENED TO ALL MY OTHER PRIUS V MODELS AT APPROXIMATELY 200K MILES SO I THINK THIS IS A FAULTY GASKET TOYOTA HAS USED IN ALL ITS PRIUS V MODEL CARS FROM 2012 TO 2016 YEARS THEREFORE THERE SHOULD BE A RECALL OF THESE CARS WHICH TOYOTA SHOULD FIX WITHOUT CHARGE. I SPENT MORE THAN $4000 TO FIX THE PROBLEM IN EACH CAR. DEALERSHIP COULD NOT DETECT THE PROBLEM OR REPRODUCE IT SO, AS INSTRUCTED BY DEALERSHIP, I CALLED TOYOTA CORPORATE NOV. 23, 2015 TO GET CASE NUMBER ASSIGNED. AT ONE POINT, TOYOTA REPRESENTATIVE DEMANDED "WHAT DO YOU WANT FROM TOYOTA?" CASE WAS 'ESCALATED' BUT NO CASE WORKER WAS ASSIGNED UNTIL NOV. 30, 2015. TOYOTA CORPORATE STATED THAT THE TECHNICIAN HAD TO REPRODUCE THE PROBLEM AND IF THEY CANNOT, THERE WAS NO PROBLEM AND DEALERSHIP WOULD RETURN MY CAR TO ME WITHOUT REPAIR. THE DEALERSHIP PROTESTED AND SAID THEY OT REQUIRED TO REPRODUCE THE PROBLEM AS IT WAS AN EPISODIC AND RARE EVENT. FURTHER, THEY WOULD NOT RETURN MY CAR TO ME UNTIL THE PROBLEM WAS FIXED BECAUSE IT WAS A SAFETY ISSUE. WALDORF TOYOTA STILL HAS MY CAR (OVER TWO WEEKS NOW AS TODAY IS DEC. 9, 2015). THEY HAVE NOT RECEIVED ANY FURTHER GUIDANCE BUT ARE WAITING FOR THE FIELD TECHNICAL SPECIALIST TO EXAMINE THE CAR. I FINALLY RECEIVED A CALL FROM CUSTOMER RELATIONS AT THE REGIONAL OFFICE (CENTRAL ATLANTIC TOYOTA) TO AUTHORIZE ACCESS TO THE EDR (ELECTRONIC DATA RECORDER). THIS MEANS OVER TWO WEEKS HAVE PASSED BEFORE EVEN THE FIRST STEP WAS TAKEN BY TOYOTA TO DIAGNOSE THE PROBLEM.

THE MANUFACTURER WAS MADE AWARE OF THE FAILURE AND DID NOT ASSIST. THE FAILURE MILEAGE WAS APPROXIMATELY 80,000.

    My brakes completely stopped working while driving on a busy road. I took it to Toyota and they said it was my brake booster pump assembly. There is a known issue with the booster pump assembly and Toyota has a program to extend their warranty to fix this, but I was never once notified of this issue and never received any mail or notifications. They refuse to cover the repair because my mileage is out of warranty, despite the fact that this is an issue they caused and I almost wrecked and could’ve died. I was on a very busy road and my car was completely uncontrollable due to the brakes not responding. My passenger, others on the road, and myself could’ve died. The Toyota customer service representatives confirmed that this is part of the “customer service” program and it does not qualify due to it exceeding 150,000 miles. No warnings appeared on the dash until the brakes stopped working. The vehicle was inspected by Toyota and was confirmed to have said issue. This needs to be immediately recalled, I almost got into a very serious wreck.

    Yesterday, while driving on the interstate, all of the brake related warning lights on my dashboard lit up- ABS, traction control, brake, caution)- causing me concern, so I looked for the next exit. While exiting and slowing, I noticed my brakes felt different and were not as responsive as usual. I called my mechanic and cautiously headed to his shop a couple of miles away. Today, I met with him and found out that there is a leak in the brake booster line, but the extent of the problem is yet to be determined. I immediately did an internet search for my model year and found that there was a safety campaign on other Toyota hybrids for that year, so I called the Toyota customer service number. I was told that my vehicle had an extended warranty for the brake booster, but it expired 9 months prior, 10 years post purchase, The rep has escalated my issue but I will have to wait to see if someone with more authority can potentially give me an additional extension. Meanwhile, my car is not drivable, as the brakes could fail at any moment. In my estimation, this should not be a time limited coverage, but an actual recall covered by Toyota. This is not normal brake pad rotor wear and tear, but a known issue with the assembly system and a true safety hazard that one cannot foresee without experiencing minor to catastrophic failure of the system.

    The brake master cylinder and brake booster failed. It is available for inspection upon request. My safety was put at risk as braking distance increased significantly. The problem has not yet been confirmed by a dealer or independent service center. The vehicle has not yet been inspected. The warning lights appeared upon failure. Brake warning light, traction control light, and ABS light appeared. Prior to complete failure, the brake booster would cycle on after opening the door and would then cycle every few seconds for 10 minutes or so after parking.

    Something felt wrong with the brakes for many years, but mechanics checked the brake pads and rotors and said they were fine on multiple service visits. I had multiple close calls during quick stops on highways, I though I just needed new tires. I then started to hear a whirring buzz noise become more and more frequent. I researched what it was and found on reddit that there was customer support program ZJB to repair failed brake accumulators, which affects hundreds of thousands of these cars. The problem was that my brakes were only in the process of failing rather than being fully failed enough to send codes and light up the dashboard, which is the only way the customer support program would cover the $4,000 repair. The failure finally happened when I was three hours from home and had half the braking power as usual and had to limp the car home for fear of the brakes going completely out. I can't believe Toyota is letting this many people drive around with a potentially catastrophic brake issue.

THE CAR WAS DRIVING IN THE STRAIGHT LINE AND IMMEDIATELY BEFORE THIS I WAS ABLE TO SUCCESSFULLY BRAKE THE CAR ON THE VERY SAME TRIP. SO THE PROBLEM IS INTERMITTENT. THIS IS THE SECOND TIME WITH THIS CAR, AS MY SPOUSE ALSO EXPERIENCED THE SIMILAR BRAKE FAILURE AT EVEN LOWER SPEEDS (<10 MPH) RESULTING IN TOUCHING THE CAR IN FRONT AND MINOR SCRATCH ON THE BUMPER. The contact stated that while driving approximately 35 MPH, the brake light, tire pressure, traction control, battery, and other warning lights were illuminated. While attempting to depress the brake pedal, there was more resistance than normal, making it difficult to slow the vehicle. The contact was able to drive the vehicle to the nearest mechanic; however, the vehicle was not diagnosed. The vehicle was then taken to the residence. The vehicle was taken to another certified mechanic, who diagnosed and determined that the brake master cylinder and actuator assemblies had failed. The brake master cylinder and brake actuator assemblies were in the process of being repaired. The manufacturer was not informed of the failure. The failure mileage was approximately 220,000.

Toyota previously acknowledged this as a safety-related defect under Customer Support Program ZJB, but my coverage expired because I never received Toyota’s notice dated May 21, 2019. My car has been undrivable for weeks and Toyota refuses goodwill repair.     I was driving on the highway during rush hour when a loud beeping noice began. My 15 year old cousin was in the passenger seat and began to check for the source of the beeping. I was driving 55mph on a busy highway with no safe shoulder stop. As I was preparing for the exit, the brakes suddenly became dysfunctional. The pedal was extremely hard to press down and I found myself unable to slow down the car in enough time to avoid hitting upcoming traffic. I found myself in an urgent, life threatening situation and needed to pull over into the emergency lane located before the exit. I could not just consider my safety, but that of other drivers and my 15 year old passenger. She is traumatized. I was able to get the car to stop with my entire weight on the pedal. Once stopped, I scrambled to find the reason for the sudden brake problem, checking if anything rolled under my foot, if the floor mat was loose, if the gear shift was wrong, even checked if I accidentally hit the emergency brake. Nothing was out of place. No new dash lights. I looked in the manual. I had no information. I went to jiffy lube and they performed a diagnostic test and informed me they are unable see all the codes with hybrids because Toyota’s computer can only see Just in August I went to Toyota for my oil change, nothing was said about the brakes needing to be fixed and how repairing them now would prevent the failure of the accumulator pump system. If I had known paying $400-$500 for the brakes then would prevent having to pay $3000 to replace a pump. I would have made that sacrifice and went outside of my budget. This is paramount information that should be disclosed to all Toyota customers that take their car in for care and pay. After further research, I STRONGLY BELIEVE TOYOTA EMPLOYEES PURPOSELY WITHHOLD INFORMATION FROM THEIR CUSTOMERS NOT INFORMING CONSUMERS ABOUT AVOIDING PUMP FAILURE AND THE NEED REPAIR THEIR BRAKES AND IS COMPLICIT IN ANY INCIDENT CAUSED BY PUMP FAILURE MAY CAUSE.

    Car was diagnosed at a Toyota dealership with a brake booster issue reading code 20TE07 after my vehicles brakes suddenly would not work and the car continued to jolt forward. The brakes wouldn’t stop the car. According to Toyota, my vehicle is three months outside of the warranty period but under the covered mileage so we are left to repair this defect ourselves. This was a very dangerous situation and could have cost us and our children our lives instead of just the $4800 repair cost. This has happened to both of our Prius’s so this is not a rare issue. Toyota needs to have a plan for these parts failing suddenly and they should all be recalled for safety in my opinion. The experience was very scary and we had zero control over the vehicle when the brakes suddenly went out. Where do I join the class action lawsuit?

    Without warning, my brakes completely stopped working and I was fortunate enough to be somewhere I was able to pull over without being killed. Toyota is aware of this issue, but put a 10 year timeline on the complementary repair, which is totally unacceptable, as owners were not notified directly about this program, therefore how could we know it existed, was a problem, and to go get it fixed within their arbitrary deadline? I could have died losing brakes on the freeway, or even could have killed or injured others. This is a known, and severe safety hazard, and Toyota is not taking responsibility. It is unconscionable. Hoping for a class action lawsuit. It was a $5500 repair in California, which was almost the value of the car. Absolutely brutal and horrendous business practices from this company. Shame on them. I hope this agency NHTSA will add this report to the pile of others who have reported this, and actually take meaningful action for those of us who have been put at risk, and ripped off in the process.

    I was parked in my driveway and put my 2014 Toyota Prius in reverse to back up and park in front of my late husband’s workshop about 15 ft. away,on Oct. 10, 2022. The car sped up so fast I could not brake fast enough (no warning).It stopped when it hit the large door of the workshop, and damaged it and the back of my car, costing about $2,000 to the car. I contacted my insurance agent right away, and the person above him at the company the next day, as well as the service manager at the Toyota dealer where we bought the car at the end of 2014. I expect an increase in my premium from my insurance company. I’ll take it to Toyota when it’s done at the auto body shop, but I don’t expect any flaw to be found b/c that is what happened when I had the same kind of accident with this car in Jan., 2022, when I put the car in reverse to back down my long driveway, (which I’d done many times before),and it sped up so fast (SUA) that I couldn’t stop it (no warning). It hit a maple tree in my yard, and it cost about $12,000 to fix the back of my car. I was not injured in either accident, but this flawed accelerator could have caused serious injury to me or others, as it has in many documented accidents in the last 20 years. From the reports I’ve read since my accident last week, thousands of people have experienced similar or worse accidents from a Sudden Unintended Acceleration in their Toyota vehicle. Also, I read in an excellent well-researched and independent report that NHTSA has not researched the accelerator problem thoroughly and has remained noncommittal as to its possible flaws. Toyota wanted to take the “black box” from my Prius in 2022 after my accident, but I didn’t trust them to take it, and tried hard to find an independent engineering group with advanced degrees to study it and identify the flaw in it. I will try again to find such a group. The floor mat was NOT an issue at all—it’s the original one, stuck to the floor about 4” from the accelerator.
