Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2013-2015 Prius and 2014-2017 Prius V vehicles not included in recall 18V-684. Excessive voltage in the Intelligent Power Module (IPM) within the inverter may cause the hybrid system to shut down, causing the vehicle to stall while being driven.
Risk: An unexpected hybrid power system shut down may increase the risk of a crash.
Fix: Toyota will notify owners, and dealers will perform a software update for the hybrid system. If the vehicle has experienced an inverter failure with certain hybrid system faults related to this condition, the inverter assembly will be repaired or replaced, prior to software update. All repairs will be performed free of charge. This recall began July 24, 2020. Owners may contact Toyota customer service at 1-888-270-9371. Toyota's number for this recall is 20TB10 / 20TA10.

Toyota is recalling certain model year 2010 through 2014 Prius vehicles. In the affected vehicles, the Intelligent Power Module (IPM) inside the inverter module (a component of the hybrid system) contains transistors that may become damaged from high operating temperatures. If this occurs, various warning lamps will be illuminated on the instrument panel and the vehicle will have reduced power allowing it to only drive a short distance.
Risk: The vehicle may enter a fail-safe/limp-home mode that limits the drivability of the vehicle. The hybrid system could also shut down completely resulting in a vehicle stall, increasing the risk of a crash.
Fix: Toyota will notify owners and dealers will update the software for both the motor/generator control electronic control unit (ECU) and the hybrid control ECU, free of charge. If an owner experiences a failure of the inverter before the vehicle receives updated software, the dealer will repair or replace the inverter assembly with a new one at no charge. The recall began on March 21, 2014. Owners may contact Toyota at 1-800-331-4331.

Note: The software updates in this recall are needed even if the vehicle had the software previously updated under recalls 14V-053 or 15V-449.

Passenger airbag indicator on/off light was on despite having passenger over the airbag activation weight requirement being present with seatbelt buckled. Brought car to Toyota dealership and after running diagnostics for two hours they found that the airbag itself is faulty.

TL* THE CONTACT OWNS A 2014 TOYOTA PRIUS. WITHOUT WARNING, THE FRONT PASSENGER SIDE GLASS WINDOW FRACTURED. THE VEHICLE WAS NOT DIAGNOSED BY A DEALER OR AN INDEPENDENT MECHANIC. THE CONTACT REPLACED THE PART DUE TO THE FAILURE. THE MANUFACTURER WAS NOTIFIED AND STATED THAT THIS WAS NOT A KNOWN ISSUE. THE VEHICLE WAS REPAIRED. THE FAILURE MILEAGE WAS NOT AVAILABLE.

MY 2014 PRIUS HAD A LOSS OF POWER AND CHECK ENGINE LIGHT CAME ON AND MSG CHECK HYBRID SYSTEM APPEARED WHILE STARTED IN THE MORNING AND DROVE OUT OF MY GARAGE. THAT DAY I WENT TO SAN FRANCISCO TOYOTA AND THEY DID A DIAGNOSIS AND TOLD ME THAT MY HYBRID SYSTEM BATTERY WAS BAD AND NEED REPLACEMENT AND QUOTED $3600. I ASKED THAT IF THEY CAN DO THE RECALL BECAUSE I WAS 90% SURE THAT THE PROBLEM WAS DUE TO THE RECALL, BUT THEY TOLD ME THAT THEY CANT UNLESS THE ERROR CODE CLEARS OUT BY REPLACING THE HV BATTERY. I DROVE MY CAR BACK HOME AND THE NEXT DAY I WENT TO A HV BATTERY SPECIALIST AND FOUND OUT THAT MY HV BATTERY IS IN GOOD CONDITION AND DOESNT NEED REPLACEMENT. NOW I AM 100% CERTAIN THAT THE PROBLEM IS DUE TO THE RECALL BUT TOYOTA IS NOT FIXING THE ISSUE THEY ARE SAYING ITS THE HV BATTERY WHICH IS NOT. THE TOYOTA OF S.F NOT ONLY DIAGNOSED MY CAR WRONG THEY TRIED TO RIP ME OFF BY SPENDING $3600 ON A HV BATTERY THAT DOESNT NEED ANY REPLACEMENT! ON TOP OF THAT THEY ARE REFUSING TO DO THE RECALL.

I PURCHASED THE VEHICLE BRAND NEW FROM A DEALERSHIP. WITHIN A FEW WEEKS, I NOTICED THAT WHEN MY HUSBAND SAT IN THE FRONT PASSENGER SEAT (300LBS) THE NOTIFICATION LIGHT SAID "PASSENGER AIR BAG ON". WHEN MY STEPSON SAT IN THE FRONT PASSENGER SEAT THE LIGHT SAID "PASSENGER AIR BAG OFF". I TOOK A PICTURE TO SHOW THE NOTIFICATION LIGHT AND THE SIZE OF MY 15 YR OLD STEPSON 5' 10" & 175LB-FULLY ADULT SIZE. I CALLED THE DEALERSHIP AND THEY TOLD ME THEY HAD "NEVER HEARD OF THE PROBLEM" AND SAID IT WAS JUST THAT THE PASSENGER AIRBAG NEEDED TO BE RECALIBRATED. AFTER DOING SOME SEARCHING, I REALIZED THAT THE 2012-2013 MODELS HAVE ALREADY BEEN RECALLED FOR THAT EXACT SAME REASON. I AM VERY CONCERNED THAT THE PROBLEM HAS NOT BEEN FIXED WITH TOYOTA, AND THAT NEW CARS ARE BEING SOLD WITH THE SAME PROBLEM. the brakes have not failed yet, but there is a known issue with the brake booster on my model year. I would like to have the government force Toyota to guarantee the brakes for a longer period of time. currently Toyota has extended the warranty out 10 years / 150k miles from date of first use. Since these cars last so long, this should be 20 years / 300k miles from date of first use.

The ABS and traction control system in my 2014 Toyota Prius 4 are not functioning. They ABS light an the traction control lights are both on. I work in the automotive industry and I used a code reader to pull the codes and got a C1391 which is a brake booster and brake booster pump. That means either the brake booster has failed or the master cylinder is leaking. I contacted Gosche Toyota in Hemet and after waiting for a service writer to contact me (which they never did) I called 5 times till I finally got someone on the phone. They were very familiar with the problem. The problem is so common that SOME vehicles have been recalled. But for others like mine only a "Customer Support Program" was made. That program covers repair "if the parts are available" only for 10 years or 150,000 miles whichever comes first. But for someone like myself who has 170,000 miles Toyota says there's nothing they can do. The cost for me to repair it is $4800. The customer support program bulletin "ZJB" even says that service writers are not allowed to tell customers about the problem or the customer support program. That way when the car doesn't meet the requirements, they won't be responsible for the repairs, even though they know it will eventually fail. There is a lawsuit somewhere regarding this very same problem but for me, I'm screwed. I depend on my car to make money and now me and my family are out of a car. This is a known defect and a safety concern. Toyota is okay with leaving us with nothing.

I started noticing the ghosting or tripling of light issue in my windshield. I mostly noticed this during night times, I initially thought I was just tired and my eyes were tired, but I’ve been able to photograph it and verify my experience with other people. This effect is horrible for my eyesight and makes all lights at night so blurry I cannot drive safely.

WHILE I WAS DOING A SLOW TURN IN A DRIVEWAY, MY BRAKE FAILED AND THE CAR CONTINUED TO ACCELERATE IN A FORWARD MOTION. WHEN I SHIFTED INTO REVERSE, THE CAR REVVED AND ROARED ACCELERATING AT FULL TILT. IT RACED ACROSS A ROAD INTO ANOTHER DRIVEWAY ONLY STOPPING WHEN WEDGED BETWEEN A GARAGE DOOR AND A TREE. TOYOTA TOOK THE TOTALED CAR AND HAD IT INSPECTED BY ENGINEERING ASSOCIATES AND INSURANCE AUTO AUCTIONS. THEIR RESPONSE IS THAT "THERE WAS NO SERIOUS DEFECTS IDENTIFIED." OBVIOUSLY, THE MOMENTARY MECHANICAL OR ELECTRONIC FAILURE OF MY CAR COULD NEVER BE DETECTED, SO ACCORDING TO TOYOTA, THERE WAS NOTHING TO CAUSE THE FAILURE. I FEEL TOTALLY VICTIMIZED AND BELIEVE TOYOTA SHOULD TAKE RESPONSIBILITY AND COMPENSATE ME FOR THE LOSS OF MY CAR.

WE PURCHASED A NEW TOYOTA PRIUS THAT CAME WITH YOKOHAMA ALL SEASON RADIALS. BECAUSE THE TIRE LIGHT CAME ON, I CHECKED THE AIR PRESSURE IN THE TIRES AND SET THEM TO THE PSI AS RECOMMENDED AT ABOUT 1000 MILES. THERE WAS A MINOR IMBALANCE OF 3-4 PSI IN SOME TIRES, BUT NO SERIOUS UNDER INFLATION. WE NOTICED SOME MINOR ROAD NOISE AND VIBRATION. WE HAD A MECHANIC INSTALL MUD FLAPS AND INSPECT THE TIRES, WHEELS, AND BRAKES AT ABOUT 1500 MILES TO SEE IF THERE WAS A VISIBLE REASON FOR VIBRATION. THERE WAS NOTHING OBVIOUS. WE WERE CONCERNED ABOUT THE ROAD NOISE AND ABOUT 2000 MILES WE TOOK THE CAR BACK TO THE TOYOTA DEALER WHO INSPECTED THE TIRES, CHECKED THE AIR PRESSURE, AND DECLARED THAT THEY DIDN'T SEE ANYTHING WRONG. AT ABOUT 3000 MILES DRIVING AT A SLOW SPEED (35 MPH OR LESS) IN A SCHOOL ZONE, WE HAD A COMPLETE BLOW OUT OF THE LEFT REAR TIRE. FORTUNATELY, THERE WAS A GRASSY SHOULDER AND NO TRAFFIC, SO WE HAD NO INJURIES OR OTHER DAMAGE. THE TREAD APPEARED TO SEPARATE AND THERE WAS A SIDEWALL TEAR STARTING AT THE TREAD. WE RETURNED TO THE TOYOTA DEALER AND ALSO SHOWED THE TIRE TO THREE DIFFERENT TIRE SHOPS (ALL THREE SELL YOKOHAMA TIRES). THE TOYOTA DEALER DID NOT OFFER AN OPINION OTHER THAN TIRE DEFECT - SO WE NEEDED TO DEAL WITH THE MANUFACTURER. TWO TIRE SHOPS THOUGHT IT WAS A DEFECT (EVEN THOUGH THEY SAID IT WAS A HASSLE TO GET A REPLACEMENT PAID FOR BY YOKOHAMA). ONE SHOP REPORTED IT WAS A DEFECT UNLESS THE TIRE WAS UNDER INFLATED, BUT THEY DIDN'T SEE ANY WEAR PATTERN THAT INDICATED UNDER INFLATION. BECAUSE OF THE BLOWOUT AND A SECOND SUSPICIOUS TIRE OF THE FOUR, WE REPLACED ALL FOUR WITH A DIFFERENT BRAND. AS FAR AS WE CAN TELL, THE YOKOHAMA TIRE(S) WERE DEFECTIVE AS NEW ON THE CAR. *TR

Apparently there is a known defect on the brake actuator/master cylinder/booster on the 2014 Prius that would have been addressed if the car had been driven for less than 10 years or 150,000 miles. We purchased our car in April 2014 and the part just failed yesterday at only 126,000 miles. We understand that there is a pending class action suit (or perhaps one that was settled) but do not know how to become a member of it. It seems as if they knew that the part was defective, but did not offer to do a voluntary recall and remediate this before a failure, they should have, but instead, now we have a $3400 repair bill. Requesting assistance in evaluating whether there was negligence and or liability for them not addressing this issue in a timely manner.

AS I TRIED TO MAKE A RIGHT TURN AT A VERY BUSY INTERSECTION, I HEARD A STRANGE NOISE AND TOTALLY LOST CONTROL OF THE STEERING WHEELING. I TURNED THE STEERING WHEEL 360 DEGREES ALL THE WAY, AROUND AND AROUND, BUT THE CAR KEPT GOING STRAIGHT... AFTER A MINUTE OR SO, THE COORDINATION BETWEEN THE STEERING WHEEL AND THE WHEELS RE-ESTABLISHED AND I COULD MANAGE TO DRIVE TO A NEARBY PARKING LOT. THE SITUATION HAPPENED AGAIN IN THE PARKING LOT BEFORE IT CAME BACK TO NORMAL. WE TOOK THE CAR TO A LOCAL TOYOTA DEALERSHIP AND LEFT IT THERE FOR 2 DAYS. HOWEVER, TECHNICIANS SAID THEY COULD NOT DUPLICATION THE ISSUE MEANING THEY COULD NOT FIND ANY PROBLEM WITH IT. THEY TOLD US THAT IT IS PROBABLY THE ELECTRICITY PART! THE HEADLIGHTS ON PRIUS ARE NOT BRIGHT ENOUGH TO SEE THE ROAD AHEAD OF THE CAR. THE ARE ON BUT SOMEWHAT DIM. OTHER DRIVERS CAN SEE THE LIGHTS, BUT THE LIGHTS ARE NOT BRIGHT ENOUGH TO ILLUMINATE THE ROAD IN FRONT OF THE CAR. THIS OCCURRED AFTER I TOOK THE CAR IN TO THE DEALER TO COMPLAIN ABOUT A SERIES OF ELECTRICAL RELATED PROBLEMS THAT ALL STARTED HAPPENING AT THE SAME TIME. THE DEALER REPLACED THE HEADLIGHTS AFTER THEY DETERMINED THE HEADLIGHTS HAD BURNED OUT. IMMEDIATELY, I NOTICED THAT DURING TIMES WHEN I WAS DRIVING AT NIGHT, THE NIGHT VISIBILITY WAS NOT WHAT IT HAD BEEN PRIOR TO PROBLEM FIRST OCCURRING. APPROXIMATELY AT 1500 PM, I PULED THE CAR FROM THE STREET PARKING I-ORDER TO PICK A PASSENGER ABOUT 4 FEET AND THE CAR ACCELERATED BY ITSELF TOWARD RIGHT INTO THE BUILDING COMPLEX ON RIGHT AND FLIPPED ON THE SIDE OF THE DRIVER SIDE. THE CAR WAS ON DRIVE BUT I WAS TRYING TO STOP TO PICK UP A PASSENGER, BUT FAILED TO STOP AND ACCELERATED AND TURNED AND FOUND MYSELF FLIPPED ON THE SIDE AS YOU CAN SEE ON PICTURE.

WHEN THIS HAPPENS AT HIGH SPEEDS ON A HIGHWAY WHERE A PERSON IS DEPENDING ON THE SYSTEM TO GET SOMEWHERE, IT CAUSES A MASSIVE DISTRACTION AND A RISK TO LIFE AND LIMB. My brakes completely stopped working while driving on a busy road. I took it to Toyota and they said it was my brake booster pump assembly. There is a known issue with the booster pump assembly and Toyota has a program to extend their warranty to fix this, but I was never once notified of this issue and never received any mail or notifications. They refuse to cover the repair because my mileage is out of warranty, despite the fact that this is an issue they caused and I almost wrecked and could’ve died. I was on a very busy road and my car was completely uncontrollable due to the brakes not responding. My passenger, others on the road, and myself could’ve died. The Toyota customer service representatives confirmed that this is part of the “customer service” program and it does not qualify due to it exceeding 150,000 miles. No warnings appeared on the dash until the brakes stopped working. The vehicle was inspected by Toyota and was confirmed to have said issue. This needs to be immediately recalled, I almost got into a very serious wreck.

Yesterday, while driving on the interstate, all of the brake related warning lights on my dashboard lit up- ABS, traction control, brake, caution)- causing me concern, so I looked for the next exit. While exiting and slowing, I noticed my brakes felt different and were not as responsive as usual. I called my mechanic and cautiously headed to his shop a couple of miles away. Today, I met with him and found out that there is a leak in the brake booster line, but the extent of the problem is yet to be determined. I immediately did an internet search for my model year and found that there was a safety campaign on other Toyota hybrids for that year, so I called the Toyota customer service number. I was told that my vehicle had an extended warranty for the brake booster, but it expired 9 months prior, 10 years post purchase, The rep has escalated my issue but I will have to wait to see if someone with more authority can potentially give me an additional extension. Meanwhile, my car is not drivable, as the brakes could fail at any moment. In my estimation, this should not be a time limited coverage, but an actual recall covered by Toyota. This is not normal brake pad rotor wear and tear, but a known issue with the assembly system and a true safety hazard that one cannot foresee without experiencing minor to catastrophic failure of the system.

The contact owns a 2014 Toyota Prius. The contact stated that the seat belts would not lock properly. The vehicle was taken to a local mechanic where it was diagnosed with needing the buckles replaced. The manufacturer was informed of the failure and informed the contact that the VIN was not included in a recall. The failure mileage was approximately 88,000.

While driving, suddenly all brake lights and warning lights went on, and brakes stopped working normally. It became difficult to brake (brake took a while to engage - had to press pedal to the floor). After diagnostics at the dealership it looks like the brake booster failed. Apparently Toyota knew about this issue as they did an extended warranty through a Customer Support Program, but unfortunately my issue occurred just past the expiration of that warranty. Since brakes are such a critical component of vehicle safety, if they don't operate well or as expected, it endangers the driver and everyone else on the road. It is wild that Toyota is aware of this issue but that there has been no recall yet.

While driving, I received a warning "hybrid system malfunction". Within seconds, the vehicle powered off and the steering wheel LOCKED. I had no control of the car and came to an abrupt stop in lane 1 of a two lane road. I was fortunate that it was late in the evening with few cars on the road, had I been on the highway during rush hour the outcome, due to loss of power and control (steering), could have been catastrophic and perhaps fatal for me or other commuters. I note that in 2014, Toyota notified NHTSA of a recall to, "rectify software in two of the Prius's electronic control units, one for the motor/generator and the other controlling the hybrid system." Further Toyota said, "In rare circumstances, it could shut down the hybrid system itself, resulting in loss of power and the vehicle coming to a stop". This is exactly what happened to me the other night. Toyota claims, in 2014, that "it has received no reports of accidents or injuries related to the problem." This seems to be an extraordinarily amount of good luck as I could have been easily rear-ended the other night. Curiously, in this NHTSA data base, another owner reported an incident (11439720) on November 8, 2021, that is similar to the experience I had just days prior. And our VINS are eerily close. Coincidence? Not likely. Does the 2014 recall apply to my car and if not, why not given I experienced the exact same symptoms?

I WAS DRIVING AT NIGHT ON BACK ROADS WHEN BOTH HEADLIGHTS WENT OUT. I STOPPED THE CAR, TURNED IT OFF AND RESTARTED IT TO SEE WHAT WOULD HAPPEN. THE LIGHTS DID NOT GO ON, HOWEVER, I FOUND THAT IF I HELD THE SWITCH UP AS THOUGH I WAS TURNING ON HIGH BEAMS, THE HIGH BEAMS WORKED. I COULD NOT GET THE LOW BEAMS TO WORK. I WAS TAKING SOMEONE HOME AND STOPPED AT THEIR HOUSE. WHEN I CAME BACK TO THE CAR AND TURNED IT ON, THE LIGHTS WORKED NORMALLY.

The brake master cylinder and brake booster failed. It is available for inspection upon request. My safety was put at risk as braking distance increased significantly. The problem has not yet been confirmed by a dealer or independent service center. The vehicle has not yet been inspected. The warning lights appeared upon failure. Brake warning light, traction control light, and ABS light appeared. Prior to complete failure, the brake booster would cycle on after opening the door and would then cycle every few seconds for 10 minutes or so after parking.

Something felt wrong with the brakes for many years, but mechanics checked the brake pads and rotors and said they were fine on multiple service visits. I had multiple close calls during quick stops on highways, I though I just needed new tires. I then started to hear a whirring buzz noise become more and more frequent. I researched what it was and found on reddit that there was customer support program ZJB to repair failed brake accumulators, which affects hundreds of thousands of these cars. The problem was that my brakes were only in the process of failing rather than being fully failed enough to send codes and light up the dashboard, which is the only way the customer support program would cover the $4,000 repair. The failure finally happened when I was three hours from home and had half the braking power as usual and had to limp the car home for fear of the brakes going completely out. I can't believe Toyota is letting this many people drive around with a potentially catastrophic brake issue.

THE CAR BRAKES FAILED AT LOW SPEEDS (10-15 MPH) AND BUMPED INTO THE CAR IN FRONT IN A STOP-AND-GO TRAFFIC ON THE WET FREEWAY AFTER A RECENT RAIN. THE BRAKE PEDAL WENT ALL THE WAY DOWN TO TOUCH THE FLOOR, AS IF THE BRAKE PEDAL WAS DISENGAGED FROM THE BRAKING SYSTEM AND THE ABS LIGHT DID NOT TURN ON AT THE TIME OF BRAKING. THE CAR WAS DRIVING IN THE STRAIGHT LINE AND IMMEDIATELY BEFORE THIS I WAS ABLE TO SUCCESSFULLY BRAKE THE CAR ON THE VERY SAME TRIP. SO THE PROBLEM IS INTERMITTENT. THIS IS THE SECOND TIME WITH THIS CAR, AS MY SPOUSE ALSO EXPERIENCED THE SIMILAR BRAKE FAILURE AT EVEN LOWER SPEEDS (<10 MPH) RESULTING IN TOUCHING THE CAR IN FRONT AND MINOR SCRATCH ON THE BUMPER. THE LOW LIGHT BEAMS ON THE CAR BURNOUT EVERY FEW WEEKS. IT MAKES IT VERY DIFFICULT AND UNSAFE TO DRIVE AT NIGHT. My 2014 Toyota Prius (VIN [XXX] , 77,000 miles) displayed Brake, ABS, and Slip warning lights. A mechanic confirmed failure of the Brake Booster and Pump Assembly (DTC C1391). Toyota previously acknowledged this as a safety-related defect under Customer Support Program ZJB, but my coverage expired because I never received Toyota’s notice dated May 21, 2019. My car has been undrivable for weeks and Toyota refuses goodwill repair. This is a serious brake safety issue that should be reviewed for recall or extended coverage since many Prius owners have experienced the same failure. I was driving on the highway during rush hour when a loud beeping noice began. My 15 year old cousin was in the passenger seat and began to check for the source of the beeping. I was driving 55mph on a busy highway with no safe shoulder stop. As I was preparing for the exit, the brakes suddenly became dysfunctional. The pedal was extremely hard to press down and I found myself unable to slow down the car in enough time to avoid hitting upcoming traffic. I found myself in an urgent, life threatening situation and needed to pull over into the emergency lane located before the exit. I could not just consider my safety, but that of other drivers and my 15 year old passenger. She is traumatized. I was able to get the car to stop with my entire weight on the pedal. Once stopped, I scrambled to find the reason for the sudden brake problem, checking if anything rolled under my foot, if the floor mat was loose, if the gear shift was wrong, even checked if I accidentally hit the emergency brake. Nothing was out of place. No new dash lights. I looked in the manual. I had no information. I went to jiffy lube and they performed a diagnostic test and informed me they are unable see all the codes with hybrids because Toyota’s computer can only see Just in August I went to Toyota for my oil change, nothing was said about the brakes needing to be fixed and how repairing them now would prevent the failure of the accumulator pump system. If I had known paying $400-$500 for the brakes then would prevent having to pay $3000 to replace a pump. I would have made that sacrifice and went outside of my budget. This is paramount information that should be disclosed to all Toyota customers that take their car in for care and pay. After further research, I STRONGLY BELIEVE TOYOTA EMPLOYEES PURPOSELY WITHHOLD INFORMATION FROM THEIR CUSTOMERS NOT INFORMING CONSUMERS ABOUT AVOIDING PUMP FAILURE AND THE NEED REPAIR THEIR BRAKES AND IS COMPLICIT IN ANY INCIDENT CAUSED BY PUMP FAILURE MAY CAUSE.

Without warning, my brakes completely stopped working and I was fortunate enough to be somewhere I was able to pull over without being killed. Toyota is aware of this issue, but put a 10 year timeline on the complementary repair, which is totally unacceptable, as owners were not notified directly about this program, therefore how could we know it existed, was a problem, and to go get it fixed within their arbitrary deadline? I could have died losing brakes on the freeway, or even could have killed or injured others. This is a known, and severe safety hazard, and Toyota is not taking responsibility. It is unconscionable. Hoping for a class action lawsuit. It was a $5500 repair in California, which was almost the value of the car. Absolutely brutal and horrendous business practices from this company. Shame on them. I hope this agency NHTSA will add this report to the pile of others who have reported this, and actually take meaningful action for those of us who have been put at risk, and ripped off in the process.

My 2014 prius experienced a failing brake booster actuator assembly. I was driving when suddenly my traction control light came on and my brake pedal turned mushy. I pulled over to a parking lot and put my vehicle in park. As soon as I did this, the ABS and parking brake light both turned on. I had the vehicle towed to the Toyota dealership. They diagnosed the problem as an internal leak in the brake booster actuator assembly. This is a common problem with these vehicles, but instead of Toyota issuing a recall, they only did an extended warranty coverage of which my prius didn't fit the parameters. Now I am stuck with a vehicle that is unsafe to drive and a repair estimate of $3,900 because of one of their parts failing. This should clearly have been made a recall with the volume of prius owners experiencing this problem. What a disappointment after having 3 Toyotas for many years.

On [XXX] my brakes failed while driving, (brake booster and actuator) moving at 25 mph, all of a sudden lights ABS and brakes indicator light started flashing and beep. I was approaching a yellow light and attempting to slow down, my foot went to to floor and the were NO BRAKES. I used the EMG brake slow down me pulled into an empty parking lot. My vehicle is available for inspection at request. My safety was was put at risk due to having no control and coming very close to crashing into another car putting that driver at risks as well, but was blessed I was able to avoid an accident. This problem has been confirmed by Phillips Toyota in Leesburg FL #(352)728-3020. The vehicle components have not been inspected by others besides the dealer. I am 2 months over the 10yr customer support program at 102k miles. I have been complaining for the past YEAR to the dealer regarding strange noises like chirping, air deflating when pressing brakes, constant buzzing sounds from system even when car is off, and being propelled forward when going over gravel or railroad track surfaces of the ABS light goes on. This is on record and each time I’d ask, they’d mention it is the brake booster going but because there was no indicator lights or code coming up, they were unable to do anything. My last service in may 2024, I requested a diagnostic on the brake system, which I paid extra for, knowing the 10yr program was about to be expired in July. The dealer found no codes and assured me it was safe to drive and that if I were to see those warning light come on, that I was to drive the car straight to the dealer to inspect. Well, long story short, the lights came on, and immediately the were NO Brakes and I was completely out of control with no time to drive it safely to the dealer. I had it towed to the dealer and they quoted me $3000 to repair brake booster/actuator. I am working with Toyota corporate for compensation. This is a major safety issue that should be a RECALL! The problem was identified when warning lights came up on the dashboard but there is also a sound coming from the brake actuator that shows a problem. The problem is due to manufacturer error, I have driven the vehicle normally.

WHILE IN AN UNDULATING PORTION OF THE ROAD, DURING ENTRY AND EXIT FROM A DIP AND SLIGHT TURN IN THE ROAD, THE ENGINE STALLED FOR A BRIEF MOMENT, FOR NO MORE THAN A SECOND OR SO THE ACCELERATOR WAS NON-RESPONSIVE, CAUSING ME TO FORCE A STOP BY USING THE BRAKE, AFTER WHICH TIME THE MOTOR RE-ENGAGED AS NORMAL. So my wife is pulling into our driveway with my little ones and notices the ABS, traction control and Brake light come on. I then had the vehicle looked at by a local Toyota dealership and they tell me I'm getting code C1391 and that it's a known issue. They then tell me I'm out of my extended customer support warranty time frame and I will have to pay. According to NHTSA this is a known defect on this vehicle and it affects more than 100k Toyota Prius vehicles. So basically, we've been driving around in a vehicle that the brakes may fail at any moment and potentially kill us all but is only addressed by the manufacturer when the known failure occurs. Even then, they only address it if the error occurs during their set time frame. It seems this has been brought to NHTSA's attention on a few occasions and nothing has been done. I would think brakes are something that warrants a recall on a known defect. Thankfully my wife and kids weren't doing 70mph down the highway when the brakes all of the sudden stopped working.

THE VEHICLE WAS REPAIRED. TWO WEEKS LATER FAILURE AN ABNORMAL SOUND WAS HEARD COMING FROM THE BRAKES. THE VEHICLE THEN WAS TAKEN BACK TO THE SAME DEALER HOWEVER, A DIAGNOSTIC TEST WAS STILL PENDING. THE CONTACT REFERENCED NHTSA ACTION NUMBER: DP19004 (SERVICE BRAKES, HYDRAULIC) 18V684000 (HYBRID PROPULSION SYSTEM) AS POSSIBLE SOLUTIONS TO THE FAILURES HOWEVER, THE VIN WAS NOT INCLUDED. THE MANUFACTURER WAS NOT MADE AWARE OF THE FAILURE. THE FAILURE MILEAGE WAS APPROXIMATELY 112,971.

HE WAS ABLE TO OPEN THE DOOR AND NOTICED THE POWER PUSH BUTTON WAS LIT WHICH INDICATED THE ENGINE WAS ON. THE SCREEN ON THE CENTER CONSOLE STATED NO KEY WAS DETECTED. HE PRESSED THE POWER BUTTON TO TURN THE ENGINE OFF. THE DEALER INFORMED THE CONSUMER, IF HE WALKED AWAY FROM THE VEHICLE WITHOUT TURNING OFF THE IGNITION, THE VEHICLE WOULD STAY ON. THE ENGINE TURNED ON BECAUSE THE VEHICLE WAS IN BATTERY MODE TO REPLENISH THE BATTERY POWER, WHEN THE SYSTEM DETECTS THE BATTERY CHARGE HAS DROPPED BELOW A CERTAIN LEVEL. *JB
